04 · Who can prescribe

Who can prescribe Mounjaro in the UK

Mounjaro is a prescription-only medicine, so it can only be supplied on the decision of an appropriately qualified prescriber. In the UK, the right to prescribe attaches to an individual's prescribing qualification and professional registration, not to a job title. A nurse practitioner may or may not hold a prescribing qualification, and two people with the same title can work within quite different limits.

That is why a flat yes or no is unhelpful. Some nurse practitioners prescribe a wide range of medicines within their area of competence. Others work in roles where prescribing is done by a colleague. Prescribers are expected to work within their own competence, so a prescriber who does not routinely manage obesity or diabetes may reasonably pass your request to someone who does. This is the same for doctors: a GP can prescribe it, but a given practice may or may not choose to offer weight-management prescribing locally.

The medicine itself brings its own requirements, whoever signs the prescription. The prescriber needs to check that your BMI and any weight-related conditions fit the licence, review your medical history, and check what else you take.1 The BNF sets out indications, dosing and safety information for tirzepatide that prescribers refer to.11

05 · What Mounjaro is

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). It is not a replacement for diet and activity. The medicine works alongside a reduced-calorie diet and increased physical activity, and your prescriber will expect you to commit to both.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 This matters for any prescriber conversation about stopping or changing treatment, because the effects do not switch off the day after the last injection.

06 · Eligibility

Eligibility: what any prescriber will check

A prescriber will only offer Mounjaro if you meet the licensed criteria, and the criteria are the same whoever you see. For private prescribing, Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1

It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1 That list of three things is what a consultation is for. Your height and weight give a BMI. Your history shows whether you have a weight-related condition and whether anything makes the medicine unsuitable. Your current medicines show whether anything needs adjusting.

The NHS uses a tighter threshold because it is rationing a limited rollout. NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.4 That is why someone with a BMI of 32 can be licensed for private treatment yet not currently eligible on the NHS: the licence and the NHS policy answer different questions.

Some practical points about the consultation itself:

  • Be accurate about your weight and height; the prescriber's decision rests on them.
  • List every medicine you take, including insulin or other diabetes treatments, because doses of some may need adjusting.
  • Mention contraception. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
  • Say if anything about your health has changed since you last saw a clinician.

In practice, who prescribes on the NHS can vary by area. It might be your GP, another prescriber in the practice, or a specialist weight-management service. Wegovy's NHS access runs through specialist weight-management services; ask your GP. A nurse practitioner who works in your surgery may be part of that conversation, but whether they personally issue the prescription depends on their qualifications and the local arrangement.

Where does that leave you? There are three sensible paths:

  • Ask your GP or practice nurse whether you fall into the current NHS tier and how to be considered.
  • If you are not in the tier yet, ask what the local timeline looks like.
  • If you meet the licensed criteria and do not want to wait, consider regulated private treatment.

Where an NHS prescription charge applies in England, it is £9.90 per item. That is the NHS charge only; it tells you nothing about private prices, which are set by pharmacies and vary.

08 · Results to expect

What the trial evidence shows

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

To make this concrete, for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. Those are straight arithmetic from the trial percentages, not a forecast for you. Trial averages do not predict your own result, and your prescriber cannot promise a specific outcome.

The same trial reports how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Why does this matter for a nurse practitioner conversation? Because a good prescriber will use it to set realistic expectations rather than overpromise. Ask your prescriber:

  • Which dose are we aiming for, and how will we review progress?
  • What happens if I do not respond as expected?
  • How will we decide on stopping or maintaining treatment?
