An in-use Mounjaro pen can stay out of the fridge; an unopened pen should not.
For a pen you have started, the licensed position is clear. 03 · In-use pens
How long can an in-use Mounjaro pen stay out of the fridge?
An in-use pen can stay out of the fridge at a temperature not above 30°C for up to 30 days after first use.1 After that it should be discarded, even if some medicine appears to remain.
Two practical points follow. First, the 30 days is a ceiling, not a target. Most people finish a pen long before it, because each pen holds four weekly doses. Second, the limit is about storage conditions, not about how many doses you have taken. If a pen is still in use when the window ends, it should not be kept going.
You do not have to take the pen out of the fridge between doses. Many people keep an in-use pen in the fridge anyway and let it warm up a little before injecting. That is a personal preference rather than a requirement, and the licensed room-temperature allowance simply gives you flexibility for trips, busy weeks and forgotten steps.
Keep the cap on whenever the pen is not being used, because it protects the medicine from light.1 A pen left uncapped on a sunny windowsill is not being stored properly, whatever the day count says.
Labels differ between countries, and some figures circulating online come from other markets. 04 · Unopened pens
Does Mounjaro stay in the fridge before first use?
Yes: an unopened Mounjaro pen should be stored in the fridge at 2–8°C and never frozen.1 Stored that way, it keeps until the expiry date printed on the box.13
The in-use allowance does not automatically stretch to pens you have not started. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.13
When you contact the pharmacy, have the following ready so they can advise quickly:
- The batch number and expiry date from the box.
- Roughly how long the pen was out and where it was kept.
- Whether it was in direct sun, a hot car or near a heater.
- Whether the pen looks clear and colourless.

05 · Left out by mistake
What if I forgot to put my Mounjaro in the fridge?
Put it back in the fridge now, then work out whether the pen was in use or unopened. That decides everything else.
If the pen was already in use, you are very likely still inside the licensed allowance. Count the days since you started it. If you are within 30 days, the pen has been kept not above 30°C and it has not been frozen, you can carry on as normal.1
If the pen was unopened, do not guess. Check with the pharmacy before using it rather than relying on a remembered day limit.13 A short call or message is a small price compared with injecting a medicine whose condition you cannot vouch for.
While you wait for advice, inspect the pen. The solution should be clear and colourless. Do not use a pen that has been above 30°C, has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days in use.1
Should you miss the injection day because you are waiting for an answer, the missed-dose rule applies. Take the dose as soon as you can within 4 days (96 hours) of the scheduled time. 06 · Heat, cold and light
What happens if Mounjaro gets warm or cold?
Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days, and the allowance only applies while it stays not above 30°C.1
A pen that has been somewhere hotter than the label permits is outside the tested window, and its sterility and potency are no longer assured. Typical examples are a car parked in summer sun, a bag next to a radiator, or a beach holiday with the pen in a pocket. Do not rely on how the pen looks, because a pen can look normal and still have been stored badly. Contact the pharmacy and describe what happened.
Cold is the opposite risk. Never freeze Mounjaro, and never use a pen that has been frozen.1
Light is the quiet third factor. Keep the cap on to protect the pen from light.1 Keeping the pen in its box in the fridge covers that automatically.

07 · Travelling
Travelling with Mounjaro: flights, holidays and hot climates
You can fly with Mounjaro, and the in-use allowance means most holidays need no special arrangements.1 Plan around three habits.
First, carry pens in hand luggage, never in the hold. Hold luggage can fall below freezing, and a frozen pen must not be used.1 Second, pack the pen ideally in a cool bag with the prescription label or a copy of your prescription. Third, check the destination's rules for travelling with prescription injectables before you fly, as they differ from place to place.
In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch.1
If you are travelling with an unopened spare pen, keep it in the fridge at your accommodation where you can. If that is not possible, speak to the pharmacy before you go rather than improvising on arrival.

08 · Is it still good?
How to tell if Mounjaro has gone bad
Look at the solution, the expiry date and the storage history. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.1
Do not use a pen that:
- has been frozen;
- looks cloudy or discoloured;
- has particles in it;
- is past its expiry date;
- has been out of the fridge for more than 30 days in use, or above 30°C.
If you cannot account for where a pen has been, treat it as doubtful and ask the pharmacy.
Also think about provenance. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.12 Our own pens come through official LillyDirect supply, directly from Eli Lilly's UK distribution, and we are registered with the GPhC under 9012878. Everyday pen handling that protects the medicine
Good habits matter more than any single storage figure. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Each KwikPen contains four weekly doses.
Build a simple routine:
- Store unopened pens in the fridge at 2–8°C and never freeze them.
- Keep the in-use pen with its cap on, in the fridge or out of it at a temperature not above 30°C for up to 30 days.
- Write the date you started the pen on the box so you can see the in-use window at a glance.
- Rotate the injection site with each dose.
- Dispose of the pen at the end of its use, using the sharps bin supplied with your order.

Other questions about Mounjaro
Best time of day. There is no best time of day. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1 Many people choose a weekend morning so early side effects fall on days off.
Starting dose. Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly.1
Realistic pace. Weight loss is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest, and most measurable progress builds from 5mg onward.1 In the SURMOUNT-1 trial, people on tirzepatide lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity.2

12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
