Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Frozen pens are a bigger practical risk than warm ones, because the back of a fridge, a cool bag with an ice pack or an aircraft hold can all drop low enough to freeze the solution.

03 · In-use pens

In-use pens: the 30-day rule explained

The 30-day figure applies once a pen is in use, meaning you have taken the first injection from it. It does not apply to a sealed pen sitting in its box.1 The leaflet gives the same message to patients, so the number you read on the label and in the box is the one to follow.13

Count from the day of your first injection with that pen. Writing the start date on the box or setting a phone reminder removes any guesswork.

Mounjaro is injected once a week, on the same day each week, at any time of day, with or without food.1 Because each pen holds four weekly doses, a normal schedule uses the pen up comfortably inside the in-use window. If you are taking a break or have missed injections and the pen is approaching the end of its in-use period, speak to your prescriber rather than guessing.

04 · Unopened pens

Unopened pens: fridge first, then ask

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.1 That is the whole rule for a sealed pen. The in-use window is a different thing and should not be applied to a pen you have not started.

If a sealed pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.1

Practical steps if it happens to you:

  • Put the pen back in the fridge once you notice, and do not inject yet.
  • Note roughly how long it was out and whether the room was warm, near a radiator or in direct sun.
  • Check the leaflet in the box, then contact the pharmacy that dispensed it with those details.
  • Keep the box and batch number to hand when you call.

06 · Signs a pen is spoiled

How to tell if Mounjaro has gone bad

Check the liquid, the dates and the history of the pen. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.1 Each part of that sentence is a separate reason to stop.

A simple routine before each injection:

  • Look at the solution: it should be clear and colourless.
  • Check the expiry date printed on the box and pen label.
  • Think back: has the pen frozen, or been in a hot car, a bag in the sun or a warm window?
  • Check the start date: are you still inside the in-use window?

You can fly with Mounjaro, and most holidays need no special arrangements. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1 In-use pens tolerate up to 30 days not above 30°C, so a normal break fits inside the window.

For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.1 Rules vary by country, and airlines can ask to see the label, so keep the pen in its original box.

Planning tips that fit the product information:

  • Start or continue a pen so it falls inside its in-use window for the whole trip.
  • Pack any spare, unopened pens in a cool bag and put them in the fridge as soon as you reach your accommodation.

Keep pens in a fridge at 2–8°C, and never freeze them.1 Keep pens in their original box so light is blocked, and keep the cap on the pen as well.

Keep the box away from the freezer box and from cooling vents, since pens that freeze must not be used.

09 · Common mistakes

Common storage mistakes to avoid

The usual errors are heat, freezing and miscounting. None of them needs a complicated fix once you know the rules.

Leaving a pen in a hot car is the commonest slip. The in-use rule is tied to staying below the labelled limit, so a car on a summer day can take a pen outside what the product information supports. If this has happened, do not inject, and contact the pharmacy.

Freezing is the opposite problem. Never use a pen that has been frozen.1 Pens pushed against a freezer wall, or carried in a cool bag with ice packs touching them, are at risk.

Miscounting the in-use window is another. Write the first-injection date on the box. If you cannot remember when you started the pen, treat that as a reason to ask your pharmacy, not as a reason to carry on.

10 · If a dose is delayed

Missed doses and pen timing

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

This matters for storage because a long gap can leave a part-used pen sitting in your bag or cupboard while its in-use window runs down. Pen life and dose timing run separately: skipping an injection does not pause the 30-day clock.

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 If you are planning a holiday, that flexibility can help you fit injections around travel while keeping the pen within its window.

If you have stopped treatment, tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.1 A leftover pen from a stopped course should not be kept past its in-use window or used again without talking to a prescriber.
