A quick overview

What to know
before you start.

  • Memory loss is not a listed side effect in the Mounjaro SmPC or patient leaflet.
  • If you feel foggy or forgetful, tell your prescriber rather than putting up with it.
  • Report any suspected side effect, listed or not, through the MHRA Yellow Card scheme.
  • Sudden or marked memory problems, confusion or personality change need medical help straight away, whatever medicines you take.
In this guide

01 · The core answer

Does Mounjaro cause memory loss?

Memory loss does not appear among the listed side effects in the Mounjaro SmPC or the patient information leaflet.113

It does not mean that nobody taking Mounjaro will ever feel forgetful. People are forgetful for many reasons, and starting a new treatment, changing how you eat and altering your daily routine can coincide with changes in how you feel.

02 · What should you do?

What to do if your memory seems different

Speak to your prescriber if you notice new memory lapses, trouble concentrating or a general sense of mental slowness while on Mounjaro. Do this even if you suspect an everyday cause, because a clinician can look at the whole picture, including your other medicines, sleep, mood and general health.

If you are a customer, our prescribing team handles monthly reviews and side-effect support as part of aftercare. You can raise a new symptom there at any point; you do not have to wait for a scheduled check-in.

Whether or not a symptom is a listed side effect, you can report it through the Yellow Card scheme.19

Get medical help straight away if memory problems come on suddenly, or if they come with confusion, severe headache, weakness, difficulty speaking or a marked change in personality. For broader background on treatment, see our weight loss clinic guide.

03 · What the label says

What the product information says

Memory loss is not among the side effects listed in the SmPC and patient leaflet.113

For the symptom you are actually experiencing, the question to ask your prescriber is not only "is this a side effect?" but "what is the most likely explanation for me, and what should we check?" That framing leads to a more useful conversation.

04 · How Mounjaro works

How Mounjaro works, and why that matters here

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1 It activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is the intended effect.

If your intake has dropped sharply, tell your prescriber. Treatment is meant to be used alongside a reduced-calorie diet.1

Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber. It works alongside a reduced-calorie diet and increased physical activity, and individual results vary.

05 · Brain fog explained

What about brain fog, focus and tiredness?

Brain fog is a description people use, not a diagnosis, and it is not a listed Mounjaro side effect in the SmPC or leaflet.1 Feeling tired is listed as a very common side effect in the UK patient leaflet (it may affect more than 1 in 10 people). It is most noticeable in the first weeks and after dose increases, and for most people energy returns to normal within a few weeks. Tell your clinical team if tiredness is severe or not settling.

Our guide on whether Mounjaro can cause sleepless nights may help if sleep is a concern.

If you take insulin or a sulfonylurea, tell your prescriber about any new symptoms. Doses of those medicines may need reducing to avoid low blood sugar.

Brain fog that is persistent, getting worse or interfering with work, driving or daily tasks deserves a proper assessment. Reporting it via the Yellow Card scheme helps regulators too.19

06 · Dementia, mood and personality

Dementia, mood and personality questions

Our related guide on whether Mounjaro causes aggression goes into the behavioural side.

The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

A personality change that appears suddenly, especially in an older person or alongside confusion, needs medical help straight away.

07 · Long-term and the brain

Long-term effects and what Mounjaro does to the brain

Mounjaro acts on gut-hormone receptors that help regulate appetite, and this is how it supports weight management.1

Your prescriber decides what is right for you.

If you stop, tirzepatide takes roughly four to five weeks to clear the system, and there is no way to speed this up, and nothing needs doing while it clears.1 So if you are wondering whether a symptom is linked to the medicine, stopping on your own will not give a quick answer, and it is better to talk to your prescriber before changing anything.

08 · Checking your own situation

Is it the medicine, or something else?

Jot down when the problem started relative to your first dose or a dose increase, what exactly you are forgetting, and whether it is worse at certain times of day.

Note what has changed around the same time: how much you are eating and drinking, how you are sleeping, any new medicines, supplements or alcohol, and any new stress.

Be careful about where your treatment comes from. Mounjaro should only be supplied by a registered pharmacy after a consultation and prescription, and you can check any UK pharmacy on the GPhC register.12 Counterfeit and unregulated products are a real concern, and the MHRA publishes alerts about them.7 If a product or its source is uncertain, tell a clinician before judging any symptom.

We supply Mounjaro through official LillyDirect supply, sourced directly from Eli Lilly, as genuine KwikPens, and every order is reviewed by a GPhC-registered prescriber. Our registration number is GPhC 9012878.

Other symptoms people ask about include whether Mounjaro can affect memory and breathlessness on treatment.

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

10 · Who should speak up

When to contact your prescriber or a doctor

Contact your prescriber if the symptom is new since you started Mounjaro or since a dose change, or if it bothers you. That is a low threshold on purpose. You are not wasting anyone's time, and our prescribing team offers side-effect support.

Get medical help straight away for sudden confusion, collapse, severe or unusual headache, loss of speech, weakness on one side, or any rapid change in alertness.

Tell your prescriber about every other medicine you take, including those bought over the counter. The patient leaflet is the right place to check the full list of side effects and when to seek advice, and we recommend reading it fully before you start.13

Before you begin treatment, a good prescriber conversation covers your medical history and anything that already affects your thinking, sleep or mood. If you notice such problems before you start, say so; that gives a baseline to compare against later.
