The SmPC and patient leaflet are the reference documents for what is recognised, and irritability is not listed as a side effect in the UK patient leaflet.113

That does not make your experience imaginary. Starting any new injectable treatment, changing how you eat and adjusting to dose increases is a lot to absorb. Mood can shift for reasons that sit next to the medicine rather than inside it.

It is relevant if the change in temper started around the time you began treatment or went up a dose. Note when it happens, how long it lasts and whether it follows meals, poor nights or nausea. A simple diary makes a conversation with a prescriber far more useful.

Possible indirect causes of irritability

Several everyday factors can make anybody more irritable, and some may overlap with starting Mounjaro. None of these is a proven effect of tirzepatide; they are things to rule out.

Mounjaro works on two gut-hormone receptors, GIP and GLP-1, so that most people feel fuller sooner and less hungry between meals.1 If that tips into eating far too little, tiredness and low energy can follow, and a short fuse often comes with tiredness. Eating regular, balanced meals matters even when appetite is quiet.

  • Eating too little: a very small intake leaves many people tired and on edge.

  • Digestive discomfort: ongoing nausea or an unsettled stomach wears people down.
  • Changing relationship with food: if eating was how you coped with stress, you may need new routines, and that adjustment can feel frustrating.
  • Life stress: work, family and health worries continue regardless of treatment.

04 · Blood sugar and other medicines

Blood sugar, insulin and other medicines

If you take insulin or a sulfonylurea alongside Mounjaro, doses may need adjusting because of the risk of low blood sugar.1 Low blood sugar can cause shakiness, sweating and confusion, so speak to your prescriber about any mood or other changes.

If you do not take diabetes medicines, speak to your prescriber if you notice mood changes. If you take any other regular medicine, check the patient leaflet and ask your prescriber or pharmacist about the combination.

05 · Anger, rage and personality

Anger, rage and feeling like a different person

Sudden anger or a marked change in personality is not listed in the UK patient leaflet, and it should never be brushed off. Treat it as a new symptom that needs a conversation with a clinician.

06 · Low mood and mental health

Low mood, anxiety and when to seek urgent help

Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Tell your prescriber about any history of depression.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

For milder changes in mood, seek help promptly and tell your prescriber about anything that feels out of character.

Practical steps if you feel more irritable

Start by looking at the basics, then involve your prescriber. These steps are general good practice rather than treatment for a recognised side effect.

  • Keep a short diary of mood, meals, sleep and injection day.
  • Eat regular meals, even when you feel only a little hungry, and drink enough fluids.
  • Do not skip doses, double up or alter your dose to try to fix a mood symptom.
  • Tell your prescriber about any new symptom, and report it through Yellow Card.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
