No cyst type appears among the listed side effects of Mounjaro in the UK product information.113 That is the plain answer for ovarian, breast and pancreatic cysts alike. The honest wording is that cysts are not an established effect of the medicine, which is different from saying a link has been ruled out for every person.

Starting a new medicine often coincides with other changes: a different body weight, a different diet, a different routine, and sometimes a first proper look at your health. Lumps and pains that appear around that time deserve to be examined on their own merits. The question to ask is not "did Mounjaro do this?" but "has a clinician looked at it?"

If you notice something new after starting treatment, tell your prescriber even if you think it is unrelated. 04 · What the label says

What the product information says about cysts

The Mounjaro SmPC and patient information leaflet do not list cysts as a side effect.113 If an effect is not in them, it is not listed as a side effect in the UK patient leaflet.

It does not mean nobody has ever developed a cyst while taking Mounjaro. Cysts can appear in anyone, and some people will inevitably be on any widely used medicine when one is found. Coincidence in time is not the same as cause. Equally, it does not mean the evidence is closed forever.

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in strengths from 2.5mg up to 15mg. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Nothing in that mechanism description refers to cyst formation.

How to treat unlisted symptoms

When a symptom is not in the leaflet, the right approach is the same as for any new symptom. Tell your prescriber. Describe it clearly. Let a clinician decide whether it needs investigation. If your prescriber advises a pause, follow that advice.

05 · Ovarian cysts and hormones

Ovarian cysts and female hormones

People often ask whether the medicine affects female hormones, and the careful answer is that its documented action is on gut-hormone receptors, appetite, stomach emptying and insulin response.1

If your periods change noticeably after starting treatment, that is worth mentioning to your prescriber and your GP.

Pelvic symptoms that need a clinician

Persistent pelvic pain, a sense of swelling, pain that changes with your cycle, or unusual bleeding should all be assessed. A scan arranged through your GP is how ovarian cysts are normally identified. If the pain is sudden and severe, get medical help straight away rather than waiting for an appointment.

Contraception and pregnancy

There is one hormone-related point that is well established. Mounjaro can reduce absorption of oral contraceptives because it slows stomach emptying.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Ask your prescriber or pharmacist how to protect against pregnancy while you are on treatment, particularly when you start and after dose changes.

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.13 If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Always give your full medical history during consultation, including conditions you have been told about such as PCOS, so the prescriber can weigh them properly.

06 · Breast cysts and lumps

Breast cysts and breast lumps

Breast cysts are not a listed side effect of Mounjaro, and the patient leaflet does not describe breast lumps as an expected reaction.13 So when someone asks whether Mounjaro can cause breast cysts, the accurate reply is that this is not listed as a side effect in the UK patient leaflet, and that any new breast change should be examined regardless.

This matters because a lump in the breast is something you should never explain away. Whatever medicines you take, a new lump, a change in size or shape, skin dimpling, nipple changes or a localised area of pain should be seen by your GP. A clinician can examine you and arrange the right tests.

Why timing misleads

People who start treatment often begin paying closer attention to their bodies, and weight change can make existing lumps easier to feel. That does not mean the medicine created them. It also does not mean the lump is harmless. The only reliable way to know is an examination. Continue your routine breast awareness and any screening you are invited to, and mention your medicines when you attend.

What to tell your prescriber

Let your prescriber know about the lump and about the outcome of any assessment. They can record it as a new symptom, consider whether treatment should continue, and support a Yellow Card report if appropriate.19 If you are asked to stop treatment while investigations take place, follow that advice.

Remember the clearance point too. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That means stopping the injections does not instantly remove the medicine from your body, and symptoms should be assessed in their own right rather than used as a test of whether the medicine is responsible.

07 · Pancreatic cysts and the pancreas

Pancreatic cysts and pancreas warnings

Pancreatic cysts are not a listed Mounjaro side effect, but the pancreas does feature in the safety information through a different condition: pancreatitis.1 It is easy to confuse the two, so it helps to separate them clearly.

Pancreatitis is inflammation of the pancreas. The warning sign is severe, persistent stomach pain that may spread to your back, with or without vomiting: if that happens, stop using Mounjaro and get medical help straight away.1 Do not wait for your next scheduled review, and do not take another dose until a clinician has assessed you.

Who needs extra care

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13 That is why the consultation asks for your full medical history and medicine list. If you have ever been told you have a pancreatic cyst or any pancreatic condition, say so plainly.

Gallbladder signs

While we are on upper abdominal pain, note the related warning. Pain in the upper right abdomen, fever or yellowing of the skin or eyes can point to gallbladder problems and also needs medical help straight away.1 These are uncommon, but they are the symptoms to act on quickly.

Mild stomach upset is different

Many people feel more nausea or fullness in the early weeks or after a dose increase, and large portions can sit heavily because the stomach empties more slowly. That is a very different picture from severe, persistent pain. If you cannot keep fluids down, get medical help straight away. Smaller, slower meals and regular fluids usually make treatment easier.

If a scan finds a pancreatic cyst, tell the team treating you which medicines you take, including Mounjaro, and let your prescriber know the result so they can review your treatment with the full information in front of them.

08 · Rare and long-term effects

Rare and long-term side effects that are documented

The documented rare and longer-term concerns are specific, and cysts are not among them.

Serious but uncommon

Seek immediate medical help for severe persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1 If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, such as shakiness, sweating or confusion, because your prescriber may need to adjust those medicines.

Thyroid

In rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.1

Kidneys and liver

Mounjaro is not known to damage the kidneys or liver directly. The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys, so keep fluids up and contact your clinical team if you cannot keep fluids down.1

Mood

The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review.17 Any big change in weight, appetite and routine can affect how you feel, so seek help promptly if your mood drops or you have distressing thoughts.

In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

09 · Lumps and how to describe them

Finding a lump: what to note and who to see

If you find a lump anywhere on your body while on Mounjaro, the first step is a clinical examination, not a search for a cause. Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days; contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.

Needles, a sharps bin and injection guidance are included with every order. If you are unsure about technique, ask the team before your next dose rather than guessing.

A short checklist before you call

  • Where exactly is the lump, and is it on one side or both?
  • When did you first notice it, and has it grown or changed?
  • Is it tender, red, warm or leaking anything?
  • Does it vary with your menstrual cycle, meals or activity?
  • Which dose are you on, and when was your last dose or dose change?
  • Do you have any fever, unexplained tiredness or other new symptoms?

That list gives a GP or prescriber what they need to decide how urgent it is. Keep a dated note on your phone, ideally with a photo if the lump is visible and it is appropriate to take one.

Why a clinician needs to check

Lumps have many possible causes, a medicine is only one of them, and a clinician is the person who can tell which applies to you.

Feeding back to your prescriber

Repeat orders re-check suitability, and there is no auto-shipping, so your prescribing team has a regular chance to hear about new symptoms. Use it. If your GP finds something, pass that on. If a clinician advises you to stop treatment, follow their advice and tell us so your record is accurate.

10 · Eating, drinking and what to avoid

What to eat, drink and avoid on Mounjaro

No food is banned on Mounjaro, including bananas, but some choices make treatment easier and more effective. Prioritise protein at each meal, vegetables and fibre, and regular fluids. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better.2 Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.

If side effects are bothering you, these habits are the ones most worth trying first: smaller portions, eating more slowly, stopping when you feel full, and keeping water close. They will not treat a lump or a cyst, and they are not a substitute for a clinical assessment, but they can make the early weeks easier.

Alcohol and caffeine

There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and an emptier stomach can hit harder than usual, and alcohol adds calories. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Coffee and tea are fine, although strong coffee on an empty stomach can aggravate nausea or reflux for some people. Social media "coffee rules" have no clinical basis.

Supplements

No supplement is required, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist. Products sold as "natural GLP-1 boosters" are not equivalents and are not substitutes for prescribed treatment. Oral supplements may be absorbed more slowly when you start treatment.

Muscle, movement and fluids

Some of any rapid weight loss comes from lean tissue, so include a protein source at every meal and add resistance exercise. Regular moderate activity such as brisk walking is part of the licence, not an optional extra. Drinking enough matters more than usual because reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. Treat inability to keep fluids down as a reason to contact your clinical team.

Medicines to discuss

Which drugs should not be taken with Mounjaro depends on your full list, so share it during consultation. Two points are specific: insulin and sulfonylurea doses may need adjusting because of low blood sugar risk, and oral contraceptives can be absorbed less well, so switch to a non-oral method or add a barrier method for 4 weeks after starting and for 4 weeks after each dose increase.7 Your prescriber or pharmacist can check the rest.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

NHS or private

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment. Whichever route you use, report new symptoms the same way.
