01 · The core answer
Can you take NAD+ and tirzepatide together?
The honest answer is: it is not established either way. Tirzepatide is a licensed prescription medicine with a detailed Summary of Product Characteristics and patient leaflet.1 NAD+ products are a different category, often sold as supplements, injections or clinic infusions. No trial tests the two together, so any page that says it is definitely fine, or definitely a "metabolism stack", is going beyond the evidence.
You may see the claim that there are "no known interactions". It does not show the combination is safe for you. It shows the question has barely been asked.
If you already take both and feel well, you do not need to panic. Do tell your prescriber, so it is on your record and can be weighed against anything else you take. If you are only thinking about it, the sensible default is to wait and talk it through first.

What you are actually deciding
Most people asking this question are really asking one of three things. Each has a different answer.
- "Will it harm me?" We cannot rule it out, because the combination is untested. Your prescriber can check it against your health history and other medicines.
- "Will it help my weight loss?" There is no evidence that NAD+ improves tirzepatide results. Weight loss on treatment works alongside a reduced-calorie diet and more physical activity.2
- "Will it fix my tiredness?" Tiredness has many causes. Pinning it on a missing coenzyme without an assessment is guesswork, and it can delay finding the real cause.

What to do before adding NAD+
Start with a conversation, not a purchase. Tell the prescriber the product name, the form (capsule, nasal, injection or infusion), who supplies it and how often you plan to use it. Ask whether anything in your history makes it a poor idea. If a seller offers to supply an injectable product without a consultation or prescription, treat that as a warning sign.

04 · What tirzepatide does
What is Mounjaro (tirzepatide)?
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1
Tirzepatide acts on appetite and digestion. Anything you take by mouth shares a stomach that is emptying more slowly, which is one reason your prescriber wants to know about every product you use.13
05 · What NAD+ is
What is NAD+?
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in the cells of the body. It takes part in the way cells handle energy, which is why wellness marketing describes it as an energy or anti-ageing molecule.
That biology is real, but it is a long step from "cells use NAD+" to "taking more of it improves how you feel". Products vary widely: capsules, powders, nasal sprays, injections and clinic infusions all exist, and they are not interchangeable. Dosing is not standardised, and quality depends on the supplier.
Importantly, NAD+ products are not the same as licensed medicines such as Mounjaro. A licensed medicine has been assessed for quality, safety and effectiveness for specific uses. A supplement or wellness treatment has not been put through the same process for weight management. That is not a reason to assume harm, but it is a reason to be sceptical of strong promises.
06 · Why people stack them
Why people combine NAD+ and tirzepatide
The idea is simple and appealing: tirzepatide reduces appetite and so intake, and some people feel tired while eating less, so NAD+ is pitched as a way to "support energy". Sellers then describe it as a metabolism stack.
There are problems with that logic. First, tiredness on a lower intake may simply mean you are eating less, or that your dose is climbing faster than your body is comfortable with. Second, it assumes NAD+ would fix the tiredness, which no evidence shows. Third, it treats a symptom instead of finding the cause.
Tirzepatide's headline results do not rely on any add-on. In the trial, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary.2 Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

08 · What not to mix
What not to mix with tirzepatide or NAD supplements
Do not self-assemble a stack. The safest approach is to treat every addition as a medicines question and check it against the leaflet and with your prescriber.13
Some specific points apply to Mounjaro:
- Oral contraceptives. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows.
- Insulin and sulfonylureas. Doses may need adjusting because of the risk of low blood sugar. They are not simply forbidden with Mounjaro, but the change should be supervised.1
- Other injectable weight-loss products. Do not combine Mounjaro with other GLP-1 medicines or unlicensed products. If a product is not prescribed to you, do not use it.
For NAD+ itself, the practical risk is not necessarily the molecule. It is the supply chain: unclear ingredients, inconsistent strength and sellers who promise dramatic weight-loss claims. 09 · Tiredness on treatment
If you feel tired on Mounjaro
Treat tiredness as a message to investigate, not a gap to fill. Begin with the basics: are you eating enough, drinking enough and sleeping properly? Reduced appetite can make it easy to under-eat, and that alone can leave you flat.
Next, look at timing. Tiredness that appears around a dose increase may settle, but you should say so at your review. Our team offers ongoing monthly reviews and dose-titration follow-ups for exactly this reason.
Finally, rule out other causes. Low mood, poor sleep and other conditions can all cause fatigue and have nothing to do with tirzepatide. A NAD+ product could mask the picture without addressing any of them.
If a new symptom worries you, tell your prescriber and consider a Yellow Card report. For severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing, seek immediate medical help.19

Why am I not losing weight on tirzepatide? Results vary from person to person, and trial averages do not predict your own result.2 Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. If your progress has stalled, raise it at your review before adding products. Dose, intake, medicines and sleep are all worth checking.
Do these medicines mess up metabolism? Tirzepatide works by activating GIP and GLP-1 receptors to help regulate appetite, slow stomach emptying and improve how the body responds to insulin.1 That is its intended action, not damage. Anything beyond this is a conversation for your prescriber.
Do I have to stay on tirzepatide forever? Whether and when to stop is a decision between you and your prescriber. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14 If you do stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

Getting tirzepatide through a regulated route
If you are not yet on treatment, start with the licensed medicine, not the add-ons. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46
