Can you take 5mg and 2.5mg of Mounjaro at the same time?

No. Mounjaro is licensed as a single weekly injection at your prescribed strength, and the strengths available are 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 There is no licensed instruction to add two injections together to reach a dose in between, such as 7.5mg, and no instruction to take two 2.5mg injections to make 5mg.

It is a tempting idea, particularly if you hold a spare pen or the next strength is delayed. But the dose ladder exists for a reason. Most side effects appear in the first days after starting or increasing a dose and settle as your body adapts, which is why Mounjaro uses a gradual ladder.1 Stacking injections skips that adjustment and puts a larger step on your stomach in one go.

04 · How the step up works

How the move from 2.5mg to 5mg works

The move from 2.5mg to 5mg is a change of pen, not a combination of doses. Everyone starts Mounjaro on 2.5mg once a week.1 This starting dose is about letting your body adjust, not about weight loss itself.1

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Note the words "at least" and "minimum". Four weeks is the earliest point, not a target, and many people stay longer on a dose if they need to.

Each KwikPen contains four weekly doses.1 That means one starter pen covers the first four weeks on 2.5mg, and the next pen is the 5mg strength.

05 · Why not combine pens

Why combining or doubling up is a bad idea

Combining pens is a bad idea because it removes the gradual adjustment that the dose ladder is designed to give. Side effects are most likely when starting treatment or moving up a dose.1 A self-made higher dose lands that risk on your stomach at once.

There are practical reasons too. A pen is supplied for a single strength, and the instructions with it describe one injection per week at that strength. Mixing strengths muddles your record of what you have actually taken, which makes it harder for your prescriber to judge how you are tolerating treatment and when to step up. Because each pen holds four weekly doses, it can feel wasteful to leave a partly used pen. But a pen is for weekly use over its four doses, not for catching up or accelerating.

Some people hope that doubling a low dose is a safe halfway house between steps. It is not a recognised option. If you feel 2.5mg is not doing enough, the proper question for your prescriber is whether you are ready to move on, not how to get more from each week.

06 · Side effects of stepping up

Side effects when moving from 2.5mg to 5mg

Most side effects when you step up from 2.5mg to 5mg are digestive, and they are usually temporary. Like all medicines, Mounjaro can cause side effects, and most are digestive.13 The most common — affecting more than 1 in 10 people — are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.13

The timing matters. Most side effects follow a pattern: they appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts.1 Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13

In the trials supporting the weight-management licence, nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo.2 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.2 Individual results vary, and these are trial averages, not a prediction of your own experience.

Managing the step up

Practical steps that help most people: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.1 If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.1

If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up — speak to your clinical team rather than pushing through severe symptoms.1 Tiredness is also common, most noticeable in the first weeks and after dose increases.1 For a wider look at the starter strength, see whether the 2.5mg dose is safe.

Serious effects need urgent help

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). Also seek immediate medical help for pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1 If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 You can report any suspected side effect through the Yellow Card scheme.19

07 · If you took too much

What to do if you have taken too much Mounjaro

If you have taken extra Mounjaro, do not inject again and talk to a doctor immediately. This applies whether you took a 5mg and a 2.5mg together, doubled up 2.5mg, or simply injected twice in a week by mistake.

Trouble is most likely to show up in the digestive system. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.1 Severe, persistent stomach pain that may spread to your back needs immediate medical help — get medical help straight away.1 If you use insulin or a sulfonylurea, watch for low blood sugar.1

Practical steps in the meantime:

  • Do not take your next scheduled dose until you have spoken to a prescriber or pharmacist, who will tell you when to resume.
  • Sip fluids and keep meals small and plain while you wait for advice.
  • Keep the pen, packaging and a note of what you took and when, so you can describe it accurately.
  • If you become seriously unwell, get urgent medical help instead of waiting for a call back.

The same rule holds if you accidentally injected a higher strength than intended, for example a 10mg dose when you were prescribed less. Tell your clinical team what you took. Never take two doses within 3 days of each other.1 Report suspected side effects through the Yellow Card scheme.19

08 · Timing, early and missed doses

Injecting early, late or more than once a week

Mounjaro is a once-weekly injection and should not be taken more than once a week. It is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 That is why questions such as injecting every few days, or taking your dose a few days early, come down to the same rule: leave a proper gap between doses.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

If you want to change your day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Taking a dose early simply because you want a bigger effect is not covered.

Rotate the site with each dose. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.1

09 · How long 2.5mg stays in your system

How long does 2.5mg Mounjaro stay in your system?

The active ingredient stays in your body for several weeks after an injection. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

This explains why a dosing mistake can be felt for longer than a day or two, and why you should talk to a clinician rather than wait for it to pass.

10 · Pen strengths and storage

Pens, strengths and storage

Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Our pens are genuine KwikPens supplied through official LillyDirect supply, direct from Eli Lilly's UK distribution, and all six strengths are in stock. Each order includes needles, a sharps bin and injection guidance.

Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 Keep the cap on to protect the pen from light, and never use a pen that has been frozen.1
