01 · The core answer

Can you start Mounjaro below 2.5mg?

No, because Mounjaro comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Nothing lower exists as a licensed product, and we cannot supply one.

Attempts to get a smaller amount mean tampering with the pen. Altering doses outside the licence means unknown dosing accuracy and no safety data, so we do not advise it. Our prescribers will not support it.

02 · What to do instead

What if 2.5mg feels like too much?

Tell your prescriber before your next dose. They can review how you are getting on and talk through side effects. Do not make changes yourself.

The 2.5mg strength is the starting step, and the dose increases after at least 4 weeks, depending on how you respond and tolerate it. 03 · Why pens are fixed

Why the pen strengths are fixed

Each KwikPen is made and licensed at a set strength. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.

A dose you cannot measure reliably is a dose your prescriber cannot plan around. If you want a gentler route, ask about your pace of increases instead. Our dose explainer sets out the licensed ladder.

04 · Combining pens

Using several pens or skipping steps

Using extra pens of a lower strength to reach a higher one is not an approved way to dose. Each strength is designed to be used as a single weekly injection.1 The dose does not jump straight from 2.5mg to 7.5mg: after at least 4 weeks it increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals, depending on how you respond and tolerate it. If you use more Mounjaro than you should, talk to a doctor immediately.

What do trials show about licensed doses?

The SURMOUNT-1 trial of 2,539 adults tested licensed strengths: people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary.

Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

07 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Can I take a smaller dose of Mounjaro than 2.5mg?
No. The lowest licensed Mounjaro strength is 2.5mg, and the pen cannot be safely adjusted to deliver less. Changing a pen is off-label and we do not advise it. If 2.5mg is a worry, talk to your prescriber about your plan and how you are feeling.
Will a very low dose of Mounjaro work?
The published results, such as SURMOUNT-1, relate to licensed strengths used with diet and exercise, and individual results vary. A prescriber can advise on your own plan.
Can you start tirzepatide at a lower strength than 2.5mg?
Not within the UK licence. Mounjaro pens come in six strengths from 2.5mg to 15mg, and 2.5mg is the starting step. If you struggle at the start, your prescriber can review your treatment rather than you altering the pen.
Can I take two 2.5mg pens instead of a 5mg pen?
No. Each strength is made to be used as a single weekly injection, and using extra pens to reach another strength is not an approved way to dose. Ask your prescriber about moving up to 5mg when you are ready.
Can I take half of a 5mg Mounjaro dose?
No. Splitting a dose means tampering with the pen, which risks damaging it and delivering an untested amount. If you feel 5mg is too much, contact your prescriber to talk through your options.
What happens if I take an extra dose of Mounjaro?
If you use more Mounjaro than you should, talk to a doctor immediately. Report any side effects you notice, and do not take further doses unless you have been advised to.
Can you go from 2.5mg to 7.5mg on Mounjaro?
