04 · What GLP-1 does to your body

What does GLP-1 treatment do to your body over time?

It acts on the gut-hormone system that regulates appetite and blood sugar. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the point worth holding on to: the medicine makes the diet easier to follow, it does not replace it.

The effect depends on the medicine being in your system. That is why long-term questions and stopping questions are really the same question seen from two sides.

Weight is not the only thing that moves. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.2

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. 05 · Long-term safety

What do we know about long-term safety?

Mounjaro has specific, named cautions, and your prescriber and regulators keep watching, which is why reviews matter.

First, who should not take it at all. Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.1

This is why full medical history matters at every repeat order, not just the first. Always share your full medical history and medicine list during consultation, and tell the team if anything changes while you are on treatment.13

06 · Thyroid, kidneys and liver

What about thyroid, kidneys and liver?

Thyroid. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2, as they will weigh this in your assessment. Routine thyroid monitoring is not required for most people.1

Kidneys and liver. Mounjaro is not known to damage kidneys or liver directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment. The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys. Keep fluids up and get medical help straight away if you cannot keep fluids down. People with severe impairment need individual prescriber assessment.1

For long-term users, the takeaway is practical. Dehydration during a bout of stomach upset is the thing to act on early, whatever your length of time on treatment.

07 · Mood and mental health

Does long-term GLP-1 use affect mood?

Regulators have looked at this specifically and have not found a causal link. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and low mood or suicidal thoughts, and the MHRA keeps this under continuous review.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

That said, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17 Changing your relationship with food can be emotionally significant, so it is reasonable to mention it at review even when you feel broadly fine.

08 · Side effects over time

Long-term side effects: what to watch for

Serious side effects are uncommon but need urgent attention, and they matter just as much in your second year as in your first week. Seek immediate medical help for any of these:

  • Severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis): stop using Mounjaro and get medical help straight away.
  • Pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems): get medical help straight away.
  • Swelling of the face or throat and difficulty breathing (possible severe allergic reaction): get medical help straight away.
  • If you take insulin or a sulfonylurea alongside Mounjaro, shakiness, sweating or confusion (possible low blood sugar). Your prescriber may need to adjust those medicines.

For new or lasting symptoms, tell your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19

09 · Stopping GLP-1

What happens if you stop?

Stopping should always be planned with your prescriber. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Your prescriber can discuss what to expect for you, with diet and activity habits ready to carry the load.

What you can do is plan. Before stopping, it helps to settle on how you will manage meals, portions and activity, and to agree a check-in date. If you only want a pause, tell the clinical team so the restart dose can be set safely rather than guessed. If you are pregnant or planning a pregnancy, Mounjaro must not be used during pregnancy and should be stopped at least one month before trying to conceive; if you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

10 · Lower doses long term

Can you stay on a lower dose long term?

Mounjaro comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment starts low and steps up, and the aim is to find the dose that works for you with side effects you can manage.

Some people settle on a mid-range dose, and some continue to a higher one. In SURMOUNT-1, different strengths produced different average results, with higher strengths producing higher averages, but individual results vary and a higher dose is not automatically better for you.2 The 2.5mg pen is a starting dose, so ask your prescriber before treating it as a maintenance choice. 11 · Access, cost and the NHS

Cost, access and the NHS for long-term treatment

Long-term use raises a practical question: can you keep it going? Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you go privately, think about the whole year, not just the first month. As of October 2026, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg, at standard prices; a new customer's first order is £40 less and every later order £25 less. Ozempic teeth, breasts and other changes

If you notice a change in your teeth, gums, body shape or anything else after starting a GLP-1 medicine, tell your prescriber. A symptom that is not on the UK patient leaflet list is not a listed side effect.

What to do is simple. Discuss any new symptom with your prescriber and report it through the Yellow Card scheme.19 Rapid weight change can alter how you look and feel, which is a reason to eat well, keep up good dental care and attend your reviews.
