Is it possible to lose 20kg in 3 months on Mounjaro?
It is not what the evidence tells you to expect. Weight loss on Mounjaro is gradual by design, and the first weeks on 2.5mg are an adjustment phase where losses are usually modest.2 Most measurable progress builds from 5mg onward, month by month.
Three months in, most people are still on the lower rungs of the dose ladder. In SURMOUNT-1, average weight loss built month by month, and the full averages were measured at 72 weeks, not 12.2 A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.

02 · The evidence
What the trials actually measured
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. The table shows what those percentages look like as a worked example for someone starting at 100 kg.

Notice the timescale: these are 72-week results, not 3-month results. In the same trial, half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight by 72 weeks.2 For someone starting at 100 kg, 20% would be 20 kg. So a 20kg loss is something a share of people reached over 72 weeks, not in three months.
Trial averages do not predict your own result. They describe groups of people who followed a reduced-calorie diet and increased activity alongside the medicine.
03 · Your first 3 months
What three months on Mounjaro usually looks like
The first three months are mostly about adjusting to the medicine and moving up the dose ladder. Experiences differ widely: some people notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal.2 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
Doses are increased in steps, with at least 4 weeks on each dose, depending on how you respond.1 Your prescriber decides when a step-up is appropriate. 04 · Planning and cost

05 · Why speed is not the goal
Why chasing a fast result is a poor plan
Pushing for a very large loss in a short time raises the stakes without improving the odds. Mounjaro is licensed for use alongside a reduced-calorie diet and increased physical activity, and your prescriber sets the dose pace. A plateau, or the wish for faster results, is not a reason to double doses or add unlicensed products.
Side effects are also most likely when starting treatment or moving up a dose.13 The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects, up to 1 in 10, include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions. For most people they ease within days to a few weeks as the body adjusts.
Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13 If you take insulin or a sulfonylurea, watch for low blood sugar (shakiness, sweating, confusion); your prescriber may need to adjust those medicines.13 You can report any suspected side effect through the MHRA Yellow Card scheme.19
06 · A healthy timeline
What is a healthy timeline for losing 20kg?
Think in months, not days. Trials measured results at 72 weeks, and judging progress by trend rather than individual weigh-ins is the sensible approach.2 How long 20kg takes depends on your starting weight, your dose, how you respond and how consistently you follow the diet and activity plan, so your prescriber is the right person to set a target with you.
For context, 20kg is a bigger share of a smaller body. For someone starting at 100 kg, 20kg is 20%, which is close to the 19.5% to 20.9% SURMOUNT-1 averages at 10mg and 15mg. For someone starting at a higher weight, the same 20kg is a smaller percentage. That is why a goal expressed as a percentage of your starting weight is usually more useful than a fixed number of kilograms.

07 · If progress slows
What to do if progress is slower than you hoped
Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2 Within the licence, the sensible responses are:
- Review your food intake honestly, as portions drift back up.
- Protect protein and activity.
- Check the basics, such as sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at the maximum dose.
A plateau is not a reason to double doses or add unlicensed products.
Your prescriber also reviews your response over time. 08 · Loose skin and visible change
Will 20kg be noticeable, and what about loose skin?
How noticeable a 20kg change is depends on your starting weight, height and body shape, so there is no single answer. For someone starting at 100 kg it is a fifth of body weight, which most people notice in how clothes fit and how they feel. Your waist measurement and how you feel day to day are often better markers than the scales alone.
Loose skin can follow significant weight loss; skin commonly adapts gradually after weight stabilises. Ask your prescriber about it before or during treatment so you can plan with realistic expectations. "Ozempic face" is not a drug effect; it is what losing a significant amount of weight quickly can do to facial volume, on any method. Discuss it with your prescriber if it concerns you.
09 · Beyond the scales
Progress that is not on the scales
In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

10 · Who it is for
Who Mounjaro is licensed for
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.
Mounjaro is also available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment.
