Can you get Mounjaro with a BMI of 28?

You can if you have at least one weight-related condition. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.1 At 28 you are inside that second band.

The qualifying conditions named in the licensed criteria include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes.1 Treatment must be used alongside a reduced-calorie diet and increased physical activity.1

04 · Qualifying conditions

Which conditions count at BMI 28?

The licence lists examples of weight-related conditions: high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes.1 The prescriber assesses your BMI, medical history and current medicines before deciding whether treatment suits you.1

Many people at BMI 28 do not know they have raised blood pressure or cholesterol. A recent GP or pharmacy check can settle that question. Bring any readings to your consultation, as they help the prescriber make a sound decision.

Sleep apnoea is one example where Mounjaro has been studied specifically, in the SURMOUNT-OSA trial.16 If you snore heavily or wake unrefreshed, mention it to your GP.

05 · Ethnic background adjustment

Does your ethnic background change the threshold?

It can. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.1

In practice, this means the prescriber may assess you against a lower BMI bar than the standard thresholds. Tell the prescriber your background during consultation so it can be considered properly.

06 · NHS position

What about the NHS at BMI 28?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6

A BMI of 28 is below those NHS thresholds, so NHS prescribing for you is unlikely for now. Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Your GP can talk through local options.

07 · Who should not take it

Who should not take Mounjaro?

Meeting the BMI rule does not make it suitable. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.1 In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or MEN 2.1 If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Stop Mounjaro at least one month before trying to conceive. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.

What results can you expect?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary.

For someone starting at 100 kg, 20.9% is about 21 kg. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

09 · Understanding BMI 28

Is a BMI of 28 overweight?

Yes, a BMI of 28 falls in the overweight range, below the threshold of 30 used for obesity. It is not an emergency figure, but it can still sit alongside conditions such as high blood pressure that matter for your health.

BMI is only a screening number. It does not measure fat distribution, and it is one reason a prescriber looks at your whole history rather than one figure.
