01 · The short answer

Can you get Mounjaro at a BMI of 29?

You can get Mounjaro at a BMI of 29 only if you also have at least one weight-related condition. The licence covers adults with a BMI of 30 or above, or a BMI between 27 and 30 with a condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.111

It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.1

04 · BMI 29 explained

Is a BMI of 29 concerning?

A BMI of 29 is just under the obesity line, but it is a screening measure, not a diagnosis. It cannot tell a prescriber where you carry weight or how your health is affected, which is why the licence asks about conditions as well as the number.

BMI is your weight in kilograms divided by your height in metres, squared. For example, at 1.70 m tall, a BMI of 29 is about 84 kg, a BMI of 25 is about 72 kg and a BMI of 30 is about 87 kg. These are plain calculations, not treatment targets.

What matters more than the number is whether you have a weight-related condition. High blood pressure, high cholesterol and prediabetes often cause no symptoms. If you have not had these checked recently, a GP or pharmacy health check is a sensible first step.

The same logic answers a common question: how big is someone with a BMI of 30? It depends on height. At 1.70 m, BMI 30 is about 87 kg, and a taller person would weigh more at the same BMI. That is why BMI is used as a starting point rather than a verdict.

05 · Ethnic background adjustment

Does your ethnic background change the BMI threshold?

Yes, for many people it does. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.4

As a worked example, the 30 line becomes 27.5 and the 27 line becomes 24.5. A BMI of 29 would then sit above the adjusted 27.5 line. The prescriber decides how to apply this in your case, so tell them your background during the consultation.

06 · NHS versus private

What does the NHS offer at a BMI of 29?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4

The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.6 Full rollout is expected to take years. That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

The licence and the NHS criteria are different tests. The licence says who can be prescribed Mounjaro; NICE guidance says who the NHS should prioritise. Meeting the first does not mean you meet the second. If you are unsure where you stand, ask your GP, who can explain local arrangements.

07 · Safety checks

Who should not take Mounjaro, whatever their BMI?

Meeting the BMI rule is not enough on its own. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it must not be used during pregnancy; stop it at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.113

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems. Always share your full medical history and medicine list during the consultation.1

In rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.1

If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar. Mounjaro can also reduce absorption of oral contraceptives, because stomach emptying slows.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Report any suspected side effect through the Yellow Card scheme.19

08 · What to expect

What results can you expect on Mounjaro?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

For someone starting at 100 kg, 20.9% is about 21 kg. That is arithmetic on a trial average, not a forecast for you. Trial averages do not predict your own result, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2

09 · If you do not qualify

What if you have a BMI of 29 and no qualifying condition?

If you do not qualify, the useful steps are lifestyle support, a health check and, if circumstances change, a fresh assessment. A BMI of 29 without a weight-related condition falls outside the licence.

To move from 29 towards 25, the evidence-based foundations are the ones every programme uses: a reduced-calorie diet, more physical activity and support from your GP or a weight-management service.

10 · Natural alternatives

Can you mimic Ozempic or Mounjaro naturally?

No supplement or "trick" is an established substitute for a prescription GLP-1 or GIP medicine. Products sold as "nature's Ozempic" are low-cost supplements with no clinical oversight, and their dosing is not standardised. Dramatic weight-loss claims on them should be treated with caution.

What is established is the basics: a reduced-calorie diet, regular physical activity, adequate sleep and professional support. These are also what the licence requires alongside Mounjaro. A prescriber can discuss whether medicine is right for you, and we are happy to explain the options.
