What the 2.5mg starting dose actually does

The 2.5mg pen is the first rung of a six-strength ladder, and it is an adjustment dose. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

At 2.5mg, your body meets the medicine at the lowest licensed strength. That is why this dose is described as a tolerance-building phase rather than a full treatment dose.

Who can buy the 2.5mg starting dose

You can buy the 2.5mg pen if a prescriber decides Mounjaro is suitable for you. Mounjaro is licensed for weight management in adults with a BMI of 30 or above. It is also licensed at a BMI between 27 and 30 if you have at least one weight-related condition, such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.¹¹ That is the job of the consultation. It is a short health questionnaire reviewed by a GPhC-registered prescriber, not an automated score. What the titration and first year cost

The 2.5mg pen is only the first of several, so plan for the whole ladder rather than one box. If you step up after 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £879.96 at standard prices, before any discount. A new customer's first order is £40 less, and every later order is £25 less.

If you settle at 7.5mg, 10mg or 12.5mg, work from that dose's monthly price: £244.99, £274.99 or £289.99. Higher is not automatically better

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.¹

07 · NHS or private

NHS or private: which route to the starting dose

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.⁴⁶ Full rollout is expected to take years.⁶

The practical result is that many people who meet the licensed criteria choose regulated private treatment instead of waiting. Private treatment follows the same dose ladder. You start on 2.5mg either way.

How to decide

Start by asking your GP whether you fall into a current NHS group. What NHS and private share

Both routes involve a prescriber, a suitability check, a licensed pen and a plan to step up the dose. On the NHS the prescription charge in England is £9.90 per item where it applies, and rollout groups decide who is offered treatment first. 08 · Using the pen

How to use the 2.5mg pen

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.¹ There is no best time of day. It works the same morning or evening, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.

Injection sites

Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.¹³

Setting the dose

You set the KwikPen by turning the dose knob until the indicator shows the correct dose in the window. Follow the pen's instructions for the injection itself.

If you miss a dose

Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

Changing your injection day

You can change your weekly injection day if at least 3 days (72 hours) have passed since your last dose.¹

09 · First weeks on 2.5mg

What to expect in the first weeks on 2.5mg

The 2.5mg weeks are about adjustment, and experiences differ widely. Some people notice an appetite change and early loss in week one. Others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over several weeks matters more than any single weigh-in.

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.¹³ If symptoms are severe or persistent, contact your prescriber. For severe, persistent stomach pain, swelling of the face or throat, or difficulty breathing, get medical help straight away. You can also report a suspected side effect through the MHRA Yellow Card scheme.⁷

Think in months, not days

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so judge progress by trend rather than individual weigh-ins.

What the trials measured

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages and individual results vary. As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Note that these figures describe the higher maintenance doses, not the 2.5mg starter, which is an adjustment step.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.² Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Your prescriber reviews response

10 · Storage and travel

Storing and travelling with your pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once a pen is in use, it can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips. After that it should be discarded.¹ Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Unopened pens

Unopened pens should be kept in the fridge, and they keep until the expiry date printed on the box.¹³ If an unopened pen has been out of the fridge, check with the pharmacy before using it. Do not rely on a remembered day limit. Figures circulating online often come from other countries' labels, which differ from the UK product information.

Flying and holidays

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. Ideally use a cool bag, and carry the prescription label or a copy of your prescription. Because in-use pens tolerate up to 30 days not above 30°C, most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹

Checking a pen before you inject

A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and holds a clear, colourless solution. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use. If anything about supply or packaging seems wrong, contact the pharmacy before injecting.¹³

When your delivery arrives

Our parcels travel in temperature-controlled packaging by tracked next-day delivery. Put the pen in the fridge as soon as it arrives, unless you are about to inject. Check the batch number and expiry date on the box. If a parcel is damaged or the wrong item arrives, contact us within 3 days.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.¹

11 · Avoiding fake pens

How to avoid fake Mounjaro pens

Only buy Mounjaro from a pharmacy you can verify on the GPhC register. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

Never buy from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation. A seller offering to supply without a consultation or prescription is a warning sign in itself.

How to check a pharmacy

Search the pharmacy's registration number on the public GPhC register.¹² EverydayMeds is GPhC-registered under number 9012878, and you can look it up yourself. Signs a pen is not right

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it or is past its expiry.¹³ Missing batch details or unusual packaging are also reasons to stop and check with the pharmacy before injecting.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
