In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).¹ The pen does not replace food choices or activity. It makes a lower-calorie routine easier to keep up.

05 · Dose ladder

The dose ladder: how treatment starts and builds

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.¹ That answers a common question: you may notice reduced appetite early, but the starter dose is not designed to deliver the full effect.

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹ Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹

How quickly does it work, and how much will I lose?

Results depend on your starting point, your dose, your diet and your activity, and trial averages do not predict your own result.

Because everyone begins on the starter dose, the first weeks are mostly about adjustment. Many people notice they feel full sooner, but a noticeable change in a fortnight is not what the licence is built around. The dose rises gradually, and the effect builds as it does.¹

A fast early drop is not the goal either. Losing weight very quickly can affect facial volume and skin. Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.¹

Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.¹ If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.¹

Practical steps that help most people: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.¹

Hair loss was reported by 4.9% of people taking tirzepatide in the weight-management trials, compared with 1.0% on placebo.¹ Temporary shedding is linked to rapid weight loss itself rather than a direct drug effect, and for most people it settles as weight stabilises.¹

08 · Serious warning signs

Serious side effects and when to get help

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.¹ If you use the contraceptive pill, Mounjaro slows stomach emptying, which can reduce how well it is absorbed: switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting and for 4 weeks after each dose increase.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.¹

On mood, regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.¹⁷ Any big change in weight, appetite and routine can still affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment; if you have thoughts of harming yourself or ending your life, get medical help straight away. 09 · Using the pen

Using the pen: injecting, missed doses and storage

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.¹ Follow the pen's instructions for the injection itself. Needles, a sharps bin and injection guidance are included with every order.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹
