How Mounjaro works: appetite, digestion and insulin

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 This is why the treatment is often described as an appetite-regulating medicine rather than a fat burner.

What does that feel like day to day? Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 The medicine does not do the dieting for you. It changes how strong the pull towards food feels, so that a reduced-calorie diet becomes more manageable.

The dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, sold as Wegovy and Ozempic.1 Wegovy is also an effective licensed treatment, and we explain how it differs on our Wegovy weight loss injections page. Which one suits you is a decision for you and your prescriber, based on your medical history, current medicines and how you tolerate treatment.

Another practical point is how long the medicine stays in your system. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear.1 There is no way to speed this up, and nothing needs doing while it clears.

You will also hear the word 'burn' used about injections in marketing. Anyone promising dramatic weight-loss claims from a single injection, or offering to supply without a consultation or prescription, should be treated with caution. Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber, and it is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

05 · What trials show

What the SURMOUNT-1 trial shows

The clearest evidence for Mounjaro in weight management comes from SURMOUNT-1, a trial of 2,539 adults.2 People taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Averages hide a range, so the trial also reports how many people crossed certain thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To make those percentages concrete, here is a worked example. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg, while the 3.1% seen with lifestyle changes alone is about 3 kg. Your own starting weight, dose, diet, activity level and how you respond will all change the picture, and trial averages do not predict your own result.

Notice what the control comparison tells you. Lifestyle changes alone produced a modest average loss in the trial, and tirzepatide added substantially to it. But the trial participants were all following diet and exercise advice, so the medicine is best understood as a support to those changes, not a replacement.2

Two cautions when reading headlines. First, do not set one trial's numbers beside another's as if they were a head-to-head contest. Different trials recruit different people and measure over different periods, so only a trial that compares two drugs directly can say which does more.

Another point is that the higher doses are not simply 'better'. Many people settle at 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1

06 · Realistic pace

How fast do results arrive?

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 The SURMOUNT-1 trial measured results at 72 weeks, so think in months, not days.2

Experiences in the first weeks differ widely. Some people notice an appetite change and early loss in week one; others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Why is the pace deliberately slow? Everyone starts on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of four-week intervals to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

That structure is why a single week's scale reading tells you little. Judge progress by the trend across several weeks, and weigh yourself under similar conditions if you choose to track at all. Your treatment is also reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

If you are expecting a fast result because a seller claimed an injection would 'burn' weight quickly, reset that expectation. The licensed pathway is steady, and it is built around tolerability.

Planning the money alongside the pace helps. The first four pens cost £959.96 at standard prices, before a new customer's first order is £40 less and every later order £25 less. Think of your plan in terms of the full year, not just the first pen.

07 · Who qualifies

Who is eligible for weight loss injections in the UK?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you. That assessment is the point of the consultation. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

What about the NHS? Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.6 Full rollout is expected to take years.

That gap is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The licensed criteria are wider than the current NHS priority groups, so you might qualify for private prescribing while not yet being in the first NHS cohort. If you are unsure where you sit, ask your GP about the local position. NHS access to Wegovy runs through specialist weight-management services; ask your GP.

08 · Dosing and injecting

Dose ladder and how to inject

Everyone starts Mounjaro on 2.5mg once a week. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at minimum four-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly.1 Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.

There is no 'best' time of day. Mounjaro works the same morning or evening. What matters is the same day each week. Many people choose a weekend morning so that early side effects fall on days off.

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Follow the pen's instructions for the injection itself.

Missed a dose? Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 The BNF carries further dosing information for prescribers.11

09 · Storage and travel

Storing Mounjaro and travelling with it

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

For unopened pens, the rule is simple: store them in the fridge until first use, and they keep until the expiry date printed on the box.13 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and they are the wrong market for a UK KwikPen.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.

A pen left in a hot car or a sunny windowsill is outside its tested conditions, and sterility and potency are no longer assured. Treat the pen as you would any temperature-sensitive medicine.

Your delivery arrives in temperature-controlled packaging, via free next-day tracked delivery, so the pen is protected on its journey. Put it in the fridge when it arrives unless you are about to start using it. Saturday delivery is £14.99 if you need it, and same-day dispatch applies when ordered by 3pm.

Practical checklist for the week of an injection: take the pen out of the fridge according to the leaflet's instructions, check the dose window, choose and rotate your site, inject, and put the used needle in the sharps bin supplied with your order. 10 · Supplements and myths

Natural alternatives, 'poor man's Ozempic' and other myths

The phrase 'poor man's Ozempic' or 'nature's Ozempic' is usually attached to supplements such as berberine. These are low-cost supplements with no clinical oversight, and their dosing is not standardised.

The comparison matters for safety as much as effect. A licensed GLP-1 or GIP and GLP-1 medicine is supplied after a prescriber has checked your BMI, history and current medicines.1 A supplement bought off the shelf has none of that review. Cheap and easy to buy are not signs that something works the same way.

Is there an injection that burns fat? The documented effect of tirzepatide is on appetite, stomach emptying and insulin response.1 Be wary of any seller using fat-burning language for a prescription product, particularly one that offers to supply without a consultation or prescription.

11 · Safety and what to avoid

What to avoid while taking Mounjaro

The most practical 'avoid' list comes straight from the dosing and storage rules. Do not take two doses within 3 days of each other. Do not freeze pens or use one that has been frozen. Do not keep an in-use pen beyond 30 days, even when stored not above 30°C. Do not put pens in hold luggage, where temperatures can fall below freezing.1

Also avoid altering the dose yourself. The licensed ladder already starts low at 2.5mg for tolerability. Altering doses outside the licence means unknown dosing accuracy and no safety data.

Do not treat the treatment as a substitute for diet and activity. It is licensed for use alongside a reduced-calorie diet and increased physical activity.1 And do not buy from sellers who skip the consultation. Mounjaro is a prescription-only medicine, and a legitimate supplier will have a prescriber review your suitability first.

Is Ozempic or Mounjaro safer? Ozempic is semaglutide licensed for type 2 diabetes, not a UK weight-loss medicine; prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg), and your prescriber decides what suits you. Is there a newer drug better than Mounjaro?

Is there a newer drug better than Mounjaro? No licensed medicine is best for everyone; your prescriber decides what suits you. Mounjaro, tirzepatide, is a licensed once-weekly treatment that activates both GIP and GLP-1 receptors.1 Newer options exist or are being developed, and they have their own product information.

Wegovy (semaglutide) is the established alternative among injections. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 Mounjaro's own ladder runs from 2.5mg to a 15mg maximum. These are different medicines with different dose ladders, and neither schedule tells you which will suit you.

Wegovy tablets and Foundayo are oral options with their own product information. Trials of different medicines are not head-to-head evidence, so their results should not be set against Mounjaro's. Liraglutide, sold as Saxenda and in products such as Erly, is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy. Your prescriber can advise on liraglutide's results.

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

What is the new drug better than Mounjaro?

What is known as the "poor man's Ozempic"?
The nickname is usually applied to low-cost supplements such as berberine, sometimes called 'nature's Ozempic'. These are low-cost supplements with no clinical oversight, and their dosing is not standardised. They are not licensed weight-management medicines and are not equivalent to prescription tirzepatide or semaglutide, which are supplied after a prescriber has reviewed your BMI, medical history and current medicines. If you are considering one, speak to a pharmacist or prescriber first.
Which is safer, Ozempic or Mounjaro?
Ozempic is semaglutide licensed for type 2 diabetes, not for weight loss, so using it for weight loss is off-label. Mounjaro and Wegovy are the licensed weight-management options, and suitability depends on your medical history and current medicines. A GPhC-registered prescriber should assess that before supplying either. Read the patient information leaflet, tell your prescriber about new symptoms, and report suspected side effects through the Yellow Card scheme.
What should I avoid while taking Mounjaro?
Avoid taking two doses within 3 days of each other, freezing pens or using a frozen pen, keeping an in-use pen beyond 30 days, or above 30°C, and packing pens in hold luggage where temperatures can fall below freezing. Use Mounjaro alongside a reduced-calorie diet and more activity, and buy only through a registered pharmacy after a prescriber review.
Why are people stopping Mounjaro?
People stop any medicine for varied reasons, including side effects, cost, health changes or reaching a goal. For Mounjaro, stopping should be discussed with your prescriber, who reviews your response. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.
How much does a Mounjaro weight loss injection cost in the UK?
