The thyroid cancer precaution, explained

The thyroid warning is the cancer-related line in the Mounjaro information, and it is easy to misread. In rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials.1

The practical consequence is a screening question. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.1 Any new symptom deserves a conversation with your prescriber or GP.

Breast cancer and GLP-1: what to know

The Mounjaro product information does not say whether GLP-1 treatment raises, lowers or does not change breast cancer risk. If you have had breast cancer, speak to your oncology team and your prescriber.

If you are currently being treated for breast cancer, or you are in follow-up, the right people to ask are your oncology team. They know your tumour type, your hormone therapy or chemotherapy, and your plans for the coming months. That is a decision for your clinicians, not a web page.

06 · Colon and pancreatic concerns

Colon and pancreatic cancer

The information does cover pancreatitis, which is inflammation and not cancer. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13 Do not confuse the two. A warning about pancreatitis is not a statement about pancreatic cancer.

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Also seek immediate medical help for pain in the upper right abdomen, fever or yellowing of skin or eyes, which may be a gallbladder problem.113 These symptoms need medical help straight away whatever the cause.

The same goes for bowel changes. Some digestive upset is common when starting treatment, and most people find it settles. If you have severe, persistent stomach pain, get medical help straight away; for other bowel changes that persist or do not settle, tell your prescriber or GP. If you have a family history of bowel disease, or you are in a screening programme, keep attending your appointments. Treatment for weight does not replace screening.

07 · How the medicine works

How tirzepatide works, and why it matters here

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 If you are having an operation or procedure under sedation or general anaesthetic, tell the surgical team you take Mounjaro — because it slows stomach emptying, they may give specific fasting or medication-pause instructions before the procedure. Tell your prescriber and your specialist that you are on it, so they can plan together.

Weight loss on treatment is a separate subject from cancer risk. In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2

08 · Who should not take it

Who should not take Mounjaro, and what to disclose

Mounjaro is not suitable for everyone, and your history matters more than anything you read online. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.13 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.13

For cancer specifically, the thyroid precaution is the one written into the information: tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.1 Beyond that, we ask you to disclose any cancer diagnosis, past or present, any current cancer treatment, and any planned surgery or scans.

Disclosure is protective. A prescriber who knows your whole picture can make a safer decision, and may suggest you check with your specialist first.

Also tell your prescriber about the other medicines you take. If you use insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, such as shakiness, sweating or confusion, as those medicines may need adjusting.1 People who are going through cancer care often take several medicines, so a complete list helps.

Mood is another thing to mention. Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 A cancer diagnosis can be a heavy emotional load. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

09 · NHS and private access

NHS and private access if you have a cancer history

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.46 Full rollout is expected to take years.

That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Whether your prescriber is an NHS clinician or a private one, the same history needs to be shared: thyroid cancer, MEN 2, other cancers, and your medicine list.

If you are in cancer care, your GP or specialist may be best placed to discuss NHS weight management options and the timing relative to your treatment. Ask them whether a weight-management service is appropriate for you and when.

10 · Staying well on treatment

Staying well on treatment and reporting concerns

Good habits protect you whatever your cancer history, and they make treatment easier. Treatment is licensed alongside a reduced-calorie diet and increased physical activity.1 No food is banned. Prioritise protein at each meal, vegetables and fibre, and regular fluids. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better.

Protecting muscle matters. Some of any rapid weight loss comes from lean tissue. The two levers are adequate protein, with a source at every meal, and resistance exercise on a regular weekly basis. If you are recovering from cancer treatment, ask your team what activity is right for you before you start.

Drink enough. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class. Treat inability to keep fluids down as a contact-your-clinical-team issue, not something to push through. Mounjaro is not known to damage the kidneys or liver directly.1

No supplement is required, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net. Discuss anything beyond that with a pharmacist. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures. These are trial-average outcomes, not promises.1
