05 · Spotting fake pens

How to avoid fake Mounjaro online

The biggest risk in buying online is not paying too much. It is receiving a fake. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7

The rule that protects you is simple: only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.7,12

When your pen arrives, check it. A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. 06 · Who can get it

Who can get Mounjaro online

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1,11

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. That is why the consultation matters, and why a provider that skips it is a warning sign. If you are declined, you are refunded automatically in full.

It helps to know what the medicine is. Mounjaro is the brand name for tirzepatide, a once-weekly injection made by Eli Lilly. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 It activates two gut-hormone receptors, GIP and GLP-1, which helps regulate appetite and slows how quickly the stomach empties. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1,13

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, and individual results vary.2

07 · NHS or private

NHS or private: which route suits you

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4,5,6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you have type 2 diabetes, Mounjaro is also licensed for that condition, and your GP or diabetes team can tell you whether it is appropriate for you on the NHS.8 Private treatment is an option alongside this, not instead of talking to your GP, particularly if you take other medicines.

A sensible way to decide: ask your GP whether you fall into a current NHS group. If you do not, or the wait does not suit you, a regulated online pharmacy is the alternative. Do not turn to unregulated sellers because the NHS route is slow.

08 · Mounjaro or Wegovy

Mounjaro or Wegovy: which to ask about

Both are licensed once-weekly injections, and the choice is a clinical one made with your prescriber. Wegovy is semaglutide, which acts on one gut-hormone receptor, whereas tirzepatide acts on two: GIP and GLP-1.1 Semaglutide is available through a regulated pharmacy in the same way, after a consultation. On efficacy, SURMOUNT-5 was the first large head-to-head trial of the two, and you should not set results from separate trials side by side. Within the separate trials, STEP 1 tested semaglutide and SURMOUNT-1 tested tirzepatide, each in its own population, so their averages do not rank the two medicines.9,15

The Wegovy dose ladder runs 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after 4 weeks on each of the first four doses, so the 2.4mg maintenance dose starts from week 17.18 Your first Wegovy pen costs £99.99 as a first-order price, while your first 2.5mg Mounjaro pen is £129.99. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle. Talk to your clinical team rather than self-adjusting.1

10 · After your first pen

What good aftercare looks like

Good aftercare means a clinician is still reachable after the first pen arrives. Mounjaro is started low and stepped up, so someone needs to review how you are responding before each dose change. Our prescribing team provides ongoing monthly reviews, dose-titration and follow-up assessments, and side-effect support. Repeat orders re-check suitability, and nothing is shipped automatically.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. Nothing needs doing while it clears, and nothing speeds it up.1 This matters if you ever decide to pause: speak to your prescriber first.

Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
