The upper arm is only suitable if someone else gives the injection, because you cannot reliably reach the back of your own arm. Mounjaro has three licensed injection areas, and understanding them removes most of the guesswork. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, and it comes as a pre-filled multi-dose pen (KwikPen).1 It is injected under the skin, not into muscle, which is why the sites are areas with a layer of soft tissue.
Why keep clear of the belly button
The navel area is not on the licensed list, so the safe approach is to treat it as off limits. The labelled distance acts as a buffer. When you are first learning, it helps to look at your stomach in a mirror and picture a generous circle around the navel that you will not use, then choose from the skin outside it.
Choosing healthy skin
Choose skin that looks and feels normal. Avoid spots that are tender, red, hard or marked from a previous injection.
Why the stomach is popular
Most people use the stomach simply because it is visible and reachable. You can see what you are doing, you can sit or stand comfortably, and you do not need anyone else to help. Those are convenience advantages, not clinical ones. The thigh is just as licensed, and for some people it feels more comfortable, particularly if they have a sensitive abdomen while their body adjusts to the medicine.

05 · Rotating your sites
How to rotate your stomach injection sites
Rotate the site with each dose.13 That is the instruction in the product information, and it is the single habit that matters most after staying clear of the navel. Because you inject only once a week, there is plenty of time for a site to settle before you return to the same region.
A simple rotation pattern
Imagine the licensed part of your stomach as a grid. Use the lower left on one injection day, the lower right the next, the upper left after that, then the upper right, always staying outside the buffer around your navel. Because a pen holds four weekly doses, this four-spot cycle lines up neatly with one pen. When you start a new pen, you begin the pattern again, but shift a little further out or in so you are not landing on exactly the same skin as before.
Mixing sites
You are not limited to the stomach. Some people alternate between the stomach and the front of the thigh, which spreads the load across larger areas of skin. This is allowed, because all of these are licensed sites.1 If you use the back of the upper arm, remember it only works if someone else gives the injection.
Keep a log
Memory is unreliable over many weeks. A note on your phone, a line in a diary, or the GLPTrackr companion app can record the date and the site. That makes it easy to see where you went last time and to avoid any area that became sore. It also gives your prescriber useful detail if you ever need to discuss a site reaction.
If a site is sore
If the spot you planned to use looks bruised or feels tender, simply choose another licensed area that day. You should not skip or delay your weekly dose because of a sore patch when other licensed sites are available. 06 · Stomach, thigh or arm
Stomach, thigh or upper arm: which suits you?
All three sites are licensed, so the best one is the one you can use correctly and comfortably every week. The question many people ask is whether Mounjaro works better in the thigh or the stomach. Nothing in the product information says the site changes how well the medicine works. What differs is practicality.
Stomach
The stomach is easy to see and reach, and it offers a large area, which makes rotation straightforward. Remember the buffer around the belly button.1 If you often feel digestive discomfort after a dose increase, you may prefer to avoid pressing on your abdomen at that time, and moving to the thigh is a legitimate choice.
Front of the thigh
The front of the thigh is easy to reach while seated. It works well if you prefer to keep your injection away from a bloated or sensitive stomach, and it lets you alternate between left and right legs for rotation. It is licensed in the same sentence as the stomach.1
Back of the upper arm
The back of the upper arm is licensed only if someone else gives the injection. This suits people who have a partner, carer or relative helping them. It is not designed for self-injection because you cannot properly see or reach it.
How to decide
Think about three things. First, can you reach and see the area clearly? Second, is the skin there healthy and free from marks? Third, does it feel comfortable on the day? If the answer to all three is yes, that site is a good choice. Many people use a mix of stomach and thigh.

07 · Injection day routine
Injection day: timing, food and preparation
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.13 That answers two common questions at once. Whether you inject in the morning or the evening makes no difference, and you do not need an empty or full stomach. Choose a time you will remember and that fits around your week.
Preparing the pen
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Letting the pen warm briefly out of the fridge reduces stinging.13

Should you pinch the skin?

If you miss a dose
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
Changing your injection day
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 If you move day, pick a new day you can keep and continue rotating your site as usual.
Dose changes
Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 Your prescriber guides the pace of increases, so the injection technique stays the same while the dose changes.

Injection-site reactions and what to do
Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days.13 In the weight-management trials, injection-site reactions were reported by 8.0% of people taking tirzepatide in the weight-management trials, compared with 1.8% on placebo.1 So they do happen, but they are generally mild.
Reducing the chance of a reaction
Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.13 Beyond that, choose healthy skin away from the navel buffer, and avoid injecting straight back into a spot that is still tender from the previous dose.
When to contact your clinical team
Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.13 These are signs worth checking rather than waiting out. If you are unsure whether what you are seeing is normal, a short message to the prescribing team is a reasonable step.
Signs that are not about the site
Some symptoms have nothing to do with where you injected. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13 Do not assume stomach pain after an abdominal injection is just a site reaction if it is severe and persistent.
Reporting side effects
If you notice any new symptom, discuss it with your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19 Reports help regulators monitor medicines across the whole population.
Bruises and lumps
A small bruise can happen with any injection. It is not a reason to stop treatment, but it is a reason to choose a different patch next time. Keep notes of where bruising occurred so you can avoid that region for a while. If you see a lump that does not settle, or a reaction that keeps recurring in the same type of spot, mention it at your next review so your prescriber can advise.

09 · Stomach side effects
Injecting into your stomach versus stomach side effects
Many people worry that injecting into the abdomen will make nausea or stomach upset worse. The two are separate: the digestive effects of Mounjaro come from how the medicine works, not from the place you inject. Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties.1 That slowing is why most people feel fuller sooner, and it is also why digestive effects occur.
What to expect
Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13
The trial picture
In the trials supporting Mounjaro's weight-management licence, nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo. Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.2
Practical steps
Practical steps that help most people: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.13 If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
Constipation
Constipation is very common on Mounjaro for weight management (it may affect more than 1 in 10 people), because food moves through the gut more slowly and many people eat and drink less. Fluid intake comes first, then fibre from vegetables and whole grains, and daily movement. If it persists, a pharmacist can recommend a suitable laxative.13
Dose timing and tolerance
Most side effects appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts. This is one reason Mounjaro uses a gradual dose ladder. If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up, so speak to your clinical team rather than pushing through severe symptoms.13 Changing your injection site will not fix these effects, but staying in touch with your prescriber can.

Food, fizzy drinks and getting the most from treatment
Mounjaro is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.1
What foods to avoid
There is no list of banned foods in the product information. What the guidance does say is to favour plain lower-fat foods while adjusting, eat smaller meals slowly, and stop at the first feeling of fullness.13 If a particular food leaves you feeling unwell, that is a reason to eat less of it for now. Rich or very large meals are more likely to feel uncomfortable while your stomach empties more slowly.
Chocolate and fizzy drinks
The practical logic follows from the digestive guidance: if sweet, rich or fizzy items bring on nausea, reflux or bloating for you, cut back while your dose settles. Reflux and burping are listed among the common effects, so people who find fizzy drinks aggravate those symptoms often choose still water.13 Staying hydrated matters throughout.
Speeding up weight loss
There is no safe shortcut in the product information. Higher is not automatically better when it comes to dose, and your prescriber guides the right pace.1 What helps is the supportive basics: balanced meals with adequate protein, resistance exercise, regular activity and enough fluid. Energy and hair
Fatigue is a recognised common side effect, most noticeable in the first weeks and after dose increases, so keep meals balanced, stay hydrated and keep gentle activity going. Getting Mounjaro safely: NHS and private routes
If you are reading about injection sites, you may already have Mounjaro or be about to start it. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
