Is Mounjaro the best weight loss medicine?

Mounjaro is one of several licensed weight-management injections, and whether it is best depends on your health, your tolerance and your prescriber's judgement. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

What makes it distinctive is its mechanism. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

Mounjaro has a dual mechanism, but it is not automatically right for everyone. Some people prefer a different medicine, some cannot tolerate it, and some are not suitable for any of these treatments. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Who prescribes Mounjaro in the UK

Mounjaro can be prescribed by a suitably qualified prescriber, which includes your GP, a specialist weight-management service or a GPhC-registered prescriber working for a regulated online pharmacy. The route differs, but the questions should be similar: your weight and height, your medical history, your current medicines and whether you could become pregnant.

The licence matters. Mounjaro is licensed for weight management in adults, used alongside a reduced-calorie diet and increased physical activity, and for type 2 diabetes.1 A prescriber uses the product information and clinical guidance, such as the BNF, to judge whether it suits you.11 BMI thresholds of 27, 30, 35 and 40 kg/m² often come up in discussions of eligibility, and which one applies depends on the route and on whether you have weight-related health conditions. A prescriber will explain which applies to you rather than leaving you to guess.

NHS or private: which route suits you?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

If you think you may qualify on the NHS, the sensible first step is a conversation with your GP. NHS treatment costs you only the usual prescription charge if you pay one, which in England is £9.90 per item, though check how the charge applies in your situation. The trade-off is timing: you may be placed in a later group and wait.

Private treatment is faster to start and open to adults who meet the licence and are judged suitable by the prescriber. You pay the pharmacy price, and quality varies between providers, which is why supplier checks matter so much. 09 · Safety and what to avoid

Safety: what to avoid while taking Mounjaro

The safest approach is to take Mounjaro only as prescribed, tell your prescriber about every other medicine, and report anything unexpected.

First, contraception. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only.7 Second, other diabetes medicines. If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar. That is a conversation for your prescriber, not something to change on your own.1

Next, do not use the pen in ways it was not licensed for. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose and take the next one on your normal day; never take two doses within 3 days of each other.

Regulators have also looked at mood. The EMA's safety committee reviewed the evidence on GLP-1 medicines and low mood or suicidal thoughts and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Seek help promptly if your mood drops or you have distressing thoughts during treatment, and tell your prescriber about any history of depression.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

For any new or lasting symptom, the right steps are the same. Discuss it with your prescriber, and report it through the MHRA Yellow Card scheme.19 If something is not a listed side effect, that does not mean you should ignore it.

Supply safety matters as much as clinical safety. Avoid sellers that offer to supply without a consultation or prescription, unbranded pens, powders or vials, and anyone pushing dramatic weight-loss claims. Report suspected counterfeits to the MHRA.7

Lastly, there is no way to speed up clearance if you stop. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

10 · Alternatives compared

Alternatives to Mounjaro in the UK

Mounjaro's main alternatives are other licensed weight-management medicines, and each has different dosing and a different evidence base. Wegovy (semaglutide) is the closest comparison. It is a once-weekly pen with a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg; the steps rise after at least four weeks, reaching the 2.4mg maintenance dose from week 17, and 2.4mg is the licensed maintenance dose for weight management.18 The first-order price of Wegovy's starting pen is £99.99.

Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy. Some people find a weekly routine easier to maintain, but that is a personal preference rather than a clinical rule. Figures for liraglutide vary, and your prescriber can advise.

Tablets are an emerging option. Wegovy tablets and Foundayo (orforglipron) are oral GLP-1 medicines with their own product information.2021

What about the so-called poor man's Ozempic? That nickname is used for low-cost supplements, such as berberine, sold as a natural substitute. They are available cheaply without assessment, dosing is not standardised, and they are not an equivalent to prescription GLP-1 medicines.

Which is safer, Wegovy or Mounjaro? Both are licensed prescription medicines with their own safety profiles, and neither is simply safer. Your health history and tolerance determine suitability, which is why a prescriber chooses with you.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
