01 · The decision
If you want an injection, what are the real options?
The real options are licensed prescription injections, not berberine. In the UK, the once-weekly injections for weight management are Mounjaro (tirzepatide) and Wegovy (semaglutide), and both need a prescriber to confirm they are suitable for you. That difference matters for the decision you are making. A supplement has no prescriber checking your BMI, your medical history or your other medicines. A prescription injection does. Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition, and it must be used alongside a reduced-calorie diet and more physical activity.1 A prescriber assesses your BMI, history and current medicines before deciding.11

What does the licensed alternative cost?
The standard price of one 4-week Mounjaro pen starts at £169.99 for 2.5mg and is £218.99 for 5mg. A new customer pays £129.99 for a first 2.5mg pen with code FIRST40, and every later order is £25 off the standard price (code DOSE25). The first discount applies at any dose, so someone switching to us at 15mg pays £289.99 for a first order.
If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. 04 · What berberine is
What is berberine, and is there a berberine injection?
Berberine is a plant-derived compound sold as an oral supplement, and it is not a weight-loss injection. It is extracted from plants such as barberry, goldenseal and Oregon grape, and it is sold in capsules, powders and liquids. We are not aware of any licensed berberine injection in the UK, so the phrase "berberine weight loss injections" describes something that does not exist as a regulated product.
The confusion comes largely from social media. Berberine has been nicknamed "nature's Ozempic", a catchy label that suggests it does what prescription GLP-1 injections do. It does not follow that it does. The comparison is a marketing device, not a clinical finding. Prescription injections such as Mounjaro are different substances, with a different mechanism, delivered by a different route, and supplied only after a prescriber has reviewed your health.
Supplement dosing is not standardised. If you are considering one, a community pharmacist or your GP is the right person to ask, especially if you take other medicines or have a long-term condition.
The word "injection" matters here for a practical reason. Anyone advertising a berberine jab, a berberine pen or a "natural Ozempic injection" is selling something outside the regulated medicines system. A genuine prescription injection is dispensed by a registered pharmacy against a prescription. It also helps to remember what a supplement label is and is not. A label tells you what a product claims to contain; it does not tell you that a prescriber has judged it suitable for you, or that its effects have been measured in a trial of thousands of people. Licensed medicines carry a Summary of Product Characteristics and a patient leaflet, which set out the indications, dosing and safety information. Supplements do not go through that process for weight management.
None of this means you must never speak about berberine. It means you should treat it as a low-cost supplement with no clinical oversight, not as an alternative to a treatment that has been through licensing. If your goal is meaningful, medically supervised weight loss, the more useful question is whether you are eligible for a licensed treatment and which one fits your health history.
05 · How Mounjaro works
How does a licensed weight-loss injection actually work?
Mounjaro works by activating two natural gut-hormone receptors at once: GIP and GLP-1. Tirzepatide is the first treatment of its kind to do this. Together these receptors help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 Mounjaro is the brand name for tirzepatide, a once-weekly prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
This is the key contrast with berberine. A prescription injection has a described, licensed mechanism that is set out in the product's Summary of Product Characteristics. When someone says a supplement works "like" a GLP-1 injection, remember that the GIP and GLP-1 mechanism belongs to the licensed medicine, not to a supplement.
Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4 to 5 weeks to clear the system; there is no way to speed this up, and nothing needs doing while it clears.1 That slow, steady profile is part of why the injection is taken once a week, on the same day each week, rather than daily.
The injection is used alongside lifestyle change, not instead of it. Mounjaro must be used with a reduced-calorie diet and increased physical activity.1 Think of the medicine as making that plan easier to follow, because you feel less hungry between meals, rather than as something that removes the need for the plan.

It is also why a berberine "injection" makes little sense even in principle. A made-up injectable of a different substance would have none of that engineering, testing or dose accuracy behind it.
06 · What the evidence shows
What does the trial evidence show?
For licensed tirzepatide, the headline evidence is the SURMOUNT-1 trial. In 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.
Looking at the proportion of participants, at 72 weeks 85% on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.
If a seller quotes dramatic weight-loss claims for a supplement, the right response is to ask where the trial is, how many people were in it and what the comparison group was.

What about pace? Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.1
Response is also reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That built-in review is another thing a supplement bought off the shelf cannot offer: no one is checking whether it is working or whether it is right for you.
07 · Safety and supplements
Is berberine safe, and what should you check first?
The honest position is that a supplement has no prescriber looking at your liver, kidneys, other medicines or health conditions before you start. Questions such as whether berberine is hard on the liver and kidneys, what not to mix it with, and what happens if you take it every day all belong with a pharmacist or your GP, who can see your full history.
Several general principles apply to any supplement. Tell a pharmacist or prescriber about everything you take, including supplements, because they can interact with prescription medicines. Be cautious if you are pregnant, breastfeeding, have a long-term condition or take regular medication. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. And do not stack a supplement on top of a prescription weight-loss medicine without asking first. If you are on insulin or sulfonylureas, doses may need adjusting when you start a GLP-1 type treatment because of low blood sugar risk, which is a conversation for your prescriber, not a reason to self-adjust.
Side effects with licensed injections are monitored. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 You can report any suspected side effect from a medicine or supplement through the MHRA Yellow Card scheme.19 If a new symptom appears, discuss it with your prescriber or pharmacist rather than assuming it is harmless.

There is also the counterfeit risk, which grows when people go looking for an "injection" version of a trending supplement. Be wary of any seller offering to supply an injection without a consultation or prescription, whatever it is called.
In short: berberine is a low-cost supplement with no clinical oversight. That is not the same as unsafe, but it is not the same as assessed either, and the person best placed to tell you whether it is wise for you is a pharmacist or GP who knows your medicines.
08 · Eligibility and the NHS
Who can get a licensed injection, on the NHS or privately?
Eligibility for licensed Mounjaro depends on your BMI and health. It is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity, and a prescriber assesses your BMI, medical history and current medicines.11
On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
This is the useful contrast for anyone who has been looking at berberine because a prescription feels out of reach. A supplement needs no eligibility check, which is exactly why it is easy to reach for, and also why nothing about it is tailored to you. If you meet the licensed criteria, there is a regulated route; if you do not, a prescriber can explain why and what else to consider.

Not everyone will be suitable, and that is by design. The point of a prescriber review is to catch the situations where a medicine is not right: a BMI below the licensed range, a condition or medicine that changes the picture, or a history that calls for a different approach. A supplement will never tell you no.

09 · Dosing and injecting
How does dosing and injecting a licensed pen work?
Everyone starts Mounjaro on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on response and tolerability.1 Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.
Compare that structure with the open-ended nature of supplement use, where dosing is not standardised. A licensed ladder exists so that tolerability is managed step by step.
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose, and remember each KwikPen contains four weekly doses.1 Follow the pen's instructions for the injection itself.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day, and never take two doses within 3 days of each other. You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.1
There is no best time of day: Mounjaro works the same morning or evening. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.
The first weeks vary widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Our injection FAQs cover more of the day-to-day questions.
10 · Storage and travel
How do you store and travel with a licensed injection?
Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
This is another area where a licensed medicine differs from a shelf supplement: it comes with specific storage rules, based on the product's tested window. Outside that window, sterility and potency are no longer assured. That is one reason to buy only from a registered pharmacy that handles cold-chain supply, and why our deliveries use temperature-controlled packaging.
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, so the patient information leaflet and the pharmacy are the right sources.13
Why does this matter for berberine injections? Because an unregulated "injection" of any kind has no tested storage window, no batch traceability and no leaflet. If you cannot tell how a product was made, stored or shipped, you cannot tell whether it is safe to inject. Injecting an unlicensed product is a very different risk from swallowing a capsule.
Practical tips for genuine pens: take the pen out of its box only when needed, keep the cap on between uses, note the date you first use a pen so you can count the 30 days it can be kept not above 30°C, and dispose of needles in the sharps bin supplied with your order. 11 · Spotting fake offers
How do you spot a fake or unregulated injection offer?
The clearest warning sign is anyone offering to supply an injection without a consultation or prescription. A prescription-only medicine such as Mounjaro is supplied only after an online consultation reviewed by a GPhC-registered prescriber.1 If a seller skips that step, they are outside the regulated system, whatever the product is called.
Other red flags are worth knowing. Be cautious of dramatic weight-loss claims, products named after a famous medicine but sold as "natural", sellers that promise specific results, and social-media accounts that sell injectables with no pharmacy details. A legitimate UK online pharmacy is listed on the GPhC register, and you can check ours (registration 9012878) on the regulator's public register.
The MHRA publishes counterfeit medicine alerts and operates the Yellow Card scheme, through which you can report suspected fake products or side effects.719 If you have already used something you now doubt, speak to a pharmacist or your GP and consider reporting it.
It helps to ask three questions of any seller. First, who is the dispensing pharmacy, and are they registered? Second, who reviewed my health before supplying it? 12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
