Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen, the KwikPen, in strengths from 2.5mg up to 15mg.1 The lowest strength is where everyone begins, whatever their starting weight or goals.
The reason is tolerability. Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself.1 A gentle introduction gives the digestive system time to get used to a medicine that changes how quickly the stomach empties. That is why the early weeks should be judged by how you feel and how you tolerate the injection, rather than by a number on the scales.
Understanding the mechanism helps set expectations. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide. It does not mean the lowest dose produces the full effect.
It also helps to know what the medicine is not. 05 · What the trials show
What the trials do and do not tell you
The best-known evidence for tirzepatide in weight management is the SURMOUNT-1 trial. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.
The results are for 5mg, 10mg and 15mg. The starter dose is not listed as a result, because it is the stepping stone people pass through on the way to those doses.
The trial also measured results at 72 weeks, which is a long time. Think in months, not days, and judge progress by trend, not individual weigh-ins.1 Comparing your first few weeks with a figure that took well over a year to reach will almost always feel disappointing, and that disappointment is based on a mismatch of timescales rather than on anything wrong with your treatment.
The trial also reported how many participants reached certain thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Again, these describe people who had moved up to and stayed on those doses, alongside a reduced-calorie diet and increased physical activity.
A fair reading of this evidence has three parts. First, the medicine works, on average, at the doses people reach after titration. Second, an average hides a wide spread, so some people lose more and some less. Third, none of this describes the starter weeks specifically.
Treatment response is also formally reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Note the wording: it is the highest tolerated dose, not the starter dose, that this review refers to. A realistic pace over the first months
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 That single sentence is the most useful thing to hold on to if you are comparing yourself with other people's stories.
By three months, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.1 A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.1
Because each dose is held for at least four weeks, the starter dose covers only the very beginning of that period. By the time you are three months in on a standard titration you will usually have moved through several strengths, so that expectation reflects the whole ladder, not 2.5mg alone. You cannot reliably carve out the share that came from the first pen.
Slower early progress does not predict failure. The dose you respond to may still be ahead of you.1 Some people respond strongly at a middle strength, and many settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1
So how should you track progress in practice? A few habits help:
- Weigh at the same time of day, under the same conditions, and look at the direction over several weeks.
- Notice non-scale signs, such as feeling full sooner, thinking about food less, or clothes fitting differently.
- Keep a simple note of side effects and appetite after each injection, so your monthly review has real information to work with.
- Resist comparing your week-by-week numbers with strangers online, who started from different weights, with different histories and on different schedules.

07 · Early weeks and plateaus
What is normal in the early weeks, and when you plateau
The starter weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss straight away, others feel little until a higher dose, and both are normal.1 If you fall into the second group, you are not doing anything wrong and the medicine is not necessarily failing.
Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 This is why a dramatic drop in the first days can be followed by a flat patch, and why a small rise after a salty meal does not mean the medicine has stopped working. Daily fluctuation is ordinary and says little about fat loss.
Side effects are part of the early picture too. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If symptoms are severe or do not settle, speak to your prescriber, and you can report any suspected side effect through the Yellow Card scheme.7 For severe, persistent stomach pain, swelling of the face, lips, tongue or throat, or difficulty breathing, get medical help straight away. Reading the patient information leaflet that comes with your pen is also worthwhile.13
Plateaus are the next thing people worry about. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Within the licence, the responses are practical ones:
- Review food intake honestly, because portions drift back up.
- Protect protein and activity.
- Check the basics, such as sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products.1
It is also worth remembering what the starter phase is for. If you are still on 2.5mg, a plateau may simply mean the dose you respond to is still ahead of you. In that case, the right conversation is about timing of the next step up, not about pushing harder on willpower.
08 · Beyond the scales
Beyond the number on the scales
Weight is only one of the outcomes that matter, and in the starter weeks it is often the least informative. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they explain why prescribers talk about weight management as health management rather than appearance.
For someone on 2.5mg, this matters because it reframes what success looks like at the start. In the first weeks, a more useful set of questions might be:
- Am I tolerating the injection well, with only mild and settling side effects?
- Do I feel full sooner, and is my appetite between meals quieter?
- Have I settled into a weekly routine I can keep up?
- Am I building the eating and activity habits that the medicine is meant to support?
If the answer to these is yes, you are doing what the starter phase is designed for, even if the scales have barely moved. That groundwork is what lets higher doses work as intended.
It is also why a reduced-calorie diet and increased physical activity remain part of the treatment. Mounjaro is used alongside them, not instead of them, and individual results vary.1 People who treat the medicine as a tool inside a wider plan generally have a clearer picture of their progress than those who watch the scales alone.
A few practical suggestions can make the early weeks more informative:
- Take a few simple measurements, such as waist, at the start and revisit them over months rather than days.
- If you have a blood pressure or blood-sugar monitor at home for another reason, keep using it as your clinician advises, and share the trend at your review.
- Record how you feel after each injection, because patterns in tiredness or appetite help your prescriber decide when a step up makes sense.

09 · Staying on schedule
Injecting, missed doses and storage on the starter pen
Good adherence matters more than people expect, particularly in the starter weeks when you are building the habit. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day, because the medicine works the same morning or evening. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.1
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Each KwikPen contains four weekly doses.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
Storage is straightforward. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements, and for hot climates use an insulated pouch. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

NHS or private, and how ordering works
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
