The reason is the dose ladder. Treatment starts on 2.5mg, which is an adjustment phase rather than a full-strength dose. Most measurable progress builds from 5mg onward, month by month. By the time you reach month three on a standard titration, you have typically spent only part of your treatment at doses that produced the trial averages.
That is why a three-month number can look underwhelming next to headlines about 20% losses. The two figures answer different questions. One describes the first stretch of a long climb. The other describes where large groups of people had got to after 72 weeks of treatment alongside diet and exercise. Comparing them directly is like comparing a runner's first mile with their marathon finish.
It also helps to know what is driving the early weeks. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. 06 · What the trial numbers show
What the SURMOUNT-1 trial numbers show
SURMOUNT-1 is the main trial behind the weight-management licence, and it measured results at 72 weeks. In this trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.
The same trial reports how many people reached certain thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those threshold figures show that most people responded, but they were still measured at 72 weeks.
To make the percentages tangible, take someone starting at 100 kg. An average of 15% on 5mg is about 15 kg, 19.5% on 10mg is about 19.5 kg, and 20.9% on 15mg is about 21 kg. Lifestyle changes alone, at 3.1%, would be about 3 kg. They are not predictions for you, and they say nothing about how many weeks it takes to reach any figure.
What the trial does tell us about month three is limited but useful. Average weight loss built month by month rather than arriving all at once, and the full averages sat at the 72-week mark. 07 · The dose ladder by month three

By month three on a standard titration you are usually partway up the dose ladder, not at the top. Mounjaro comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each pen covers four weeks, and treatment starts at 2.5mg.
At twelve weeks you are therefore still within that stepping-up phase, and the doses linked to the trial averages are mostly still ahead.
Your prescriber decides when to step up, based on how you are tolerating treatment and how you are responding. Stepping up is not a race. Staying at a dose that works and that you tolerate is a legitimate outcome, and the prescribing team reviews this with you through ongoing monthly reviews, dose-titration and follow-up assessments. Repeat orders re-check suitability, and there is no auto-shipping, so every pen is a considered decision rather than a subscription.
If your progress is slower than you hoped, the right conversation is with your prescriber about whether a step-up is appropriate if you are not yet at maximum.
It is also worth remembering the pen itself. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Unopened pens are stored in the fridge until first use and keep until the printed expiry date; the patient information leaflet has the full storage guidance.13 Following the instructions that come with your pen matters more than any shortcut.

08 · The first weeks: what is normal
The first weeks: what is normal
The first weeks on 2.5mg are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal.13 If you are comparing yourself with a friend or an online post, remember that their dose, starting weight, diet and tolerance all differ from yours.
Early loss includes water weight. That means the first scale readings can move more than the underlying fat change would suggest, and they can also bounce back. This is why trend over weeks matters more than any single weigh-in. A weekly weigh-in at the same time of day, written down, gives you something meaningful. Daily checking mostly gives you noise and anxiety.
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.13 If you have severe, persistent stomach pain, pain in the upper right abdomen with fever, yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing, get medical help straight away. For other side effects that are persistent or worrying, contact your prescriber. The most important message for the first three months is patience combined with honesty. Take your doses as prescribed, eat sensibly, stay active and record what happens. If the scales have moved only a little, that is not evidence of failure, and if they have moved a lot, it is not a reason to rush up the dose ladder.
09 · Plateaus and speeding things up
Plateaus, and whether you can speed things up
Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. A stall at week eight or ten does not mean the medicine has stopped working. It usually means you are in the ordinary stop-start pattern that most people see.
If you want to improve your progress, there are four sensible responses within the licence. First, review your food intake honestly, because portions drift back up without anyone noticing. Second, protect protein and keep active, since activity is part of the licensed treatment, not an optional extra. Third, check the basics, including sleep and alcohol. Fourth, discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.
Equally important is what not to do. A plateau is not a reason to double doses or add unlicensed products. Supplements marketed with dramatic weight-loss claims are low-cost and come with no clinical oversight, and dosing is not standardised. Those approaches carry risks and are not part of the licensed use.
People often ask how to speed up weight loss on Mounjaro. The honest answer is that the medicine sets the pace, and your levers are honest food tracking, regular activity, good sleep and a timely dose review. The trials combined tirzepatide with diet and exercise, and the average with lifestyle changes alone was 3.1%.2 That gap shows what the medicine adds, and the lifestyle component still counts.
The same goes for questions about losing a large amount in a single month. Aiming for a rapid result tends to push people toward skipping meals or protein, which usually makes treatment harder to tolerate rather than easier.

10 · Beyond the scales
Beyond the scales: other changes to watch
The scales are only one measure of whether treatment is working. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they explain why prescribers talk about weight management as health management rather than appearance.
At three months, you may not yet have a large number to show, but other signals can appear earlier. Clothes may fit differently before the scale moves, because the body can change shape while weight shifts only a little. Appetite and the pattern of hunger between meals may feel different. Your energy and your confidence with portion sizes may change too. Keeping a short record of these alongside your weight gives you a fuller view of progress, particularly when the scales move slowly.
If you take medicines for blood pressure, cholesterol or diabetes, tell your prescriber about them, because changes in weight and blood sugar can affect how those medicines work for you. For people on insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, and that decision belongs to your prescriber. Never change these medicines yourself.
Contraception is another practical point. Mounjaro can reduce absorption of oral contraceptives, because it slows stomach emptying.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If this applies to you, raise it with your prescriber so that you have a suitable plan.
Why do these other changes matter? Because a focus on one number can hide real progress and can make slow starters feel they are failing. A person whose weight has moved modestly but whose waist, appetite habits and energy have shifted is arguably on a healthy trajectory. And a person who has lost quickly but feels unwell, or is eating very little, may need a conversation about the pace.
It is also a reminder that the licence is for weight management in adults who meet the criteria, alongside a reduced-calorie diet and increased activity. The aim is a healthier weight that you can maintain, and the measures that matter are the ones you and your prescriber agree on at your reviews. Treat the three-month point as one data point in a longer story, not a verdict.
11 · The 16-week review
The 6-month review and what it decides
If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That conversation is about your situation, not an automatic cut-off.
This matters for anyone feeling disappointed at three months. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you. Many people are still stepping up at this point, so a modest result at twelve weeks is often a statement about where you are on the ladder rather than about how well the medicine suits you. Who is eligible in the first place? Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. 12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
