Are GLP-1 Pills as Effective as Injections? (October 2026)

GLP-1 pills can produce meaningful weight loss, but no trial has compared Mounjaro directly with either licensed GLP-1 tablet, so nobody can honestly say they are exactly as effective as injections. Each tablet has its own trial result, and the practical routines differ.

Pills versus injections, honestly

Two GLP-1 weight-loss tablets are now licensed in the UK, so the pill-or-injection question is real. The honest answer depends on which pill, which injection, and what you can stick to.

A quick overview

What to know
before you start.

  • No trial has compared Mounjaro directly with Wegovy tablets or Foundayo, so a like-for-like effectiveness ranking does not exist.
  • Wegovy tablets and Foundayo each have their own trial results, in different people, over different durations and at different doses.
  • The practical differences matter: Wegovy tablets need a strict empty-stomach routine, Foundayo has no food or water rules, Mounjaro is one injection a week.
  • SURMOUNT-5 is the trial that compared tirzepatide directly with injected semaglutide.
  • A GPhC-registered prescriber can check which option suits your health, other medicines and routine.
In this guide

01 · The core answer

Are GLP-1 pills as effective as injections?

GLP-1 pills are licensed treatments, but no trial lets you declare them equal to injections. Mounjaro is a once-weekly injection; Wegovy tablets and Foundayo are once-daily tablets. No trial has compared Mounjaro directly with either tablet, so results from separate trials, with different people, durations and doses, should not be set against each other as if they were head to head.20

The one direct comparison that does exist is SURMOUNT-5, which compared tirzepatide with injected semaglutide. It does not involve any tablet.15

02 · Evidence

What the separate trials show

Each tablet has its own trial result. In the OASIS 4 trial, adults taking semaglutide 25mg tablets for 64 weeks lost on average 13.6% of their body weight, compared with 2.2% on placebo.22 In the ATTAIN-1 trial of 3,127 adults without type 2 diabetes, people taking the highest dose of orforglipron for 72 weeks lost on average 12.4% of their body weight, compared with 0.9% on placebo; the two lower doses gave 7.8% and 9.3%.23

These are averages, and individual results vary. They are also not interchangeable with each other: the trials ran for different lengths of time in different groups of people. Read each number as an answer to "what happened on this tablet in this trial", never as a league table.

04 · Your decision

What should you do next?

Start with what you will realistically keep doing, then ask a prescriber which licensed option fits. A pill you take reliably and tolerate may serve you better than an injection you dread, and the reverse can be true. A prescriber will check which option is suitable for your health, your other medicines and your routine.20

If you are weighing up the options, our weight loss clinic explains how a free consultation works, and the Mounjaro pen page shows live prices for the injection and current availability, which is in stock across the doses.

05 · Wegovy tablets

Wegovy tablets: routine and results

Wegovy tablets contain semaglutide, the same active ingredient as the Wegovy injection, taken as one tablet once a day. They are licensed for weight management in adults with a BMI of 30 or above, or 27 to under 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.20

The routine is the main thing to weigh up. The tablet must be taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and you then wait at least 30 minutes before eating, drinking or taking other tablets.20 That suits some mornings and not others, so be honest about your day before choosing it.

Treatment starts at 1.5mg once daily and steps up to 4mg, 9mg and then the 25mg maintenance dose, with at least one month at each dose; the dose can stay at an earlier step if needed.20 If a dose is missed, skip it and take the next tablet the following day.

In OASIS 4, 76.3% lost at least 5% of their body weight (vs 30.5% on placebo) and 59.8% lost at least 10% (vs 14.1%).22 The most common side effects are digestive: nausea, vomiting, diarrhoea, constipation, indigestion and stomach pain, along with headache and tiredness; gallstones and hair loss are listed as common.22

06 · Foundayo (orforglipron)

Foundayo: a tablet without the food rules

Foundayo contains orforglipron, a once-daily GLP-1 tablet made by Eli Lilly, the maker of Mounjaro. Unlike Wegovy tablets it can be taken at any time of day, with or without food and without water restrictions; the tablet is swallowed whole.21 It is licensed for adults with a BMI of 30 or above, or 27 to under 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.

Treatment starts at 0.8mg once daily, then steps up to 2.5mg and 5.5mg, and then to 9mg, 14.5mg or 17.2mg depending on response and tolerability, with at least 30 days at each dose; 17.2mg once daily is the maximum.21 If a dose is missed, take it as soon as possible, but never more than one tablet a day.

One detail is easy to miss. Foundayo may make oral contraceptive pills less effective: switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase.21

The ATTAIN-1 trial used capsules, and the UK 17.2mg tablet corresponds to the highest trial dose, so the tablet and capsule doses are not comparable milligram for milligram.23 The most common side effects are digestive: nausea, constipation, diarrhoea, vomiting, indigestion and stomach pain. In the trial, 10.3% of people on the highest dose stopped because of side effects, compared with 2.7% on placebo.23 Our page on whether oral GLP-1 is as effective as injections goes deeper on the oral route.

07 · The injection route

The injection route: what you get with Mounjaro

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide.

Day to day, the routine is light. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no "best" time of day: it works the same morning or evening, and what matters is the same day each week. Each KwikPen contains four weekly doses. For people who dislike daily tablets or cannot manage a fasting routine, that is a real advantage. For people who dislike needles, it is not.

For the one head-to-head comparison that exists between tirzepatide and injected semaglutide, see SURMOUNT-5.15 It tells you about two injections, not about tablets.

08 · Switching and pace

Switching between injection and tablet

Moving from injection to tablet is possible for some people, with a defined route. People on the 2.4mg weekly Wegovy injection can move to the 25mg tablet, starting one week after their last injection.20 The prescriber decides when to switch between Mounjaro and either tablet and which dose to start on. Ask your prescriber, who will take your dose, tolerance and other medicines into account.

The pace of results is similar in spirit whichever route you choose: gradual by design. Weight loss on Mounjaro is gradual, the first weeks on 2.5mg are an adjustment phase and losses are usually modest, and most measurable progress builds from 5mg onward, month by month.1 Think in months, not days, and judge progress by trend, not individual weigh-ins.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 A tablet that you take inconsistently will not outperform an injection you take reliably, and the reverse is equally true.

09 · Why summaries mislead

Why quick summaries can mislead

Many quick answers say pills are "nearly as good" as injections and quote a pair of percentage ranges. Treat those with caution. Ranges blend different drugs, different trials, different durations and different populations, and the comparison they imply has not been tested head to head for Mounjaro and the tablets.20

The honest way to compare is to ask three questions. What did this specific product achieve in its own trial? How long did the trial run? And what does the routine demand of me every day or week? Averages also hide the spread: some people lose far more than the average, some far less, and trial averages do not predict your own result.

It is also worth separating tablets from "natural" alternatives. There is no tablet form of Mounjaro, because tirzepatide is injection-only in the UK. Orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses. Supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence.20 Our related guide on whether weight loss pills are as effective as injections covers that wider group.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

11 · Choosing and NHS access

Choosing a route, and NHS access

Choose on suitability and routine first. Wegovy tablets need a strict empty-stomach routine every morning, Foundayo has no food or water rules, and Mounjaro needs one injection a week.20 Eligibility also differs: the tablets, like Mounjaro, are licensed from a BMI of 30, or from 27 with a weight-related condition, while NHS criteria for Mounjaro are narrower.

On the NHS, Mounjaro is available but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Whichever route you take, all of these are prescription-only medicines. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Existing customers also get ongoing monthly reviews. You can compare more in our guide on whether the GLP-1 pill is as effective as the shot, and in how GLP-1 pills work compared with injections.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Is a GLP-1 injection or pill better?
Neither is better for everyone. No trial has compared Mounjaro directly with Wegovy tablets or Foundayo, so averages cannot be ranked against each other. The practical differences decide it: Wegovy tablets need a strict empty-stomach routine, Foundayo has no food or water rules, and Mounjaro is one injection a week. A prescriber can advise which suits you.
Is GLP-1 effective in pill form?
Yes, within the evidence available. In OASIS 4, adults taking semaglutide 25mg tablets for 64 weeks lost on average 13.6% of their body weight, compared with 2.2% on placebo. In ATTAIN-1, the highest dose of orforglipron over 72 weeks gave 12.4% against 0.9% on placebo. Individual results vary.
Are oral GLP-1 as effective as injections?
No trial has compared Mounjaro directly with either tablet, and separate trials involve different people, durations and doses, so their results should not be set against each other. Each tablet has its own trial result, and the practical routines differ.
Which GLP-1 is best for weight loss and is faster?
No licensed GLP-1 medicine is best for everyone, and weight loss on Mounjaro is gradual by design, with the first weeks an adjustment phase and most measurable progress building from 5mg onward, month by month. Think in months, not days. Your prescriber reviews response and can help you decide whether to continue, adjust or stop.
What is the absolute best weight loss pill?
There is no single best pill. Two GLP-1 tablets are licensed in the UK, Wegovy tablets and Foundayo, and each has its own eligibility checks, dosing and trial results. Orlistat is another licensed tablet but produces smaller average losses. A prescriber can check which option suits your health and routine.
What is known as the "poor man's Ozempic"?
The phrase is marketing slang, usually applied to berberine supplements. They are not medicines, are not regulated for this use and have no comparable trial evidence to licensed GLP-1 treatments. Low cost does not mean similar effect. If you are considering weight-loss treatment, a GPhC-registered prescriber can discuss licensed options.
How can I mimic Ozempic naturally?
There is no natural product shown to replicate a prescription GLP-1 medicine. Ozempic is semaglutide licensed for type 2 diabetes; it is not a UK weight-loss medicine. "Natural GLP-1" supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. A reduced-calorie diet and more physical activity remain the foundation, and licensed medicines are used alongside them.
What is a cheaper alternative to Ozempic?
Ozempic is semaglutide licensed for type 2 diabetes, not a UK weight-loss medicine; licensed weight-loss options include Wegovy and Saxenda injections, GLP-1 tablets and orlistat, each with different eligibility and results. Supplements are cheap but are not medicines and lack comparable trial evidence. For Mounjaro, see live prices on our Mounjaro pen page, where the price shown is the price paid.
