Are GLP-1 drugs used for prediabetes?

Not as a licensed treatment for prediabetes itself. GLP-1 drugs are prescribed for the conditions on their licence. For Mounjaro (tirzepatide), those are weight management in adults and type 2 diabetes.1

That does not mean the two never overlap. Many people with prediabetes also live with excess weight, and weight management is a licensed use. So a person with prediabetes may be considered for treatment because of their weight and health picture, with the prediabetes result as part of the context a prescriber reviews.

03 · What GLP-1 drugs are used for

What are GLP-1 drugs used for?

GLP-1 drugs are used mainly for type 2 diabetes and for weight management, depending on the specific product and its licence. Mounjaro, for example, is licensed for weight management in adults and for type 2 diabetes.1

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Other medicines in the wider family have their own licences and dosing. Wegovy, a semaglutide pen, is used for weight management and steps up through 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. The key point: the licence names weight management and type 2 diabetes. Prediabetes sits in between those two conditions on the health spectrum, but it is not itself the named reason for the prescription.

04 · How tirzepatide works

How Mounjaro works in the body

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

The insulin-response effect is why people often wonder about prediabetes. It is a reasonable question, but a mechanism is not the same as a licence or a proven outcome in prediabetes.

05 · What the weight-loss evidence shows

What the weight-loss evidence shows

The main trial for weight management is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2

This is evidence about weight, not a statement about prediabetes outcomes. A worked example: for someone starting at 100 kg, 20.9% is about 21 kg, on average, at the highest dose in that trial.

06 · Prediabetes questions answered

Common prediabetes questions, answered honestly

What causes prediabetes? Prediabetes relates to how the body handles blood sugar and insulin, and a GP can explain the factors that apply to you, including weight, family history and lifestyle.

Can you go back to normal? Many people are told that early action matters, and that is why a prediabetes result is treated as a prompt rather than a verdict. Your GP can retest and tell you where you stand.

Fastest way to reverse it? Sustained changes to eating, activity and weight are the usual starting point. A medicine, if suitable, supports that plan rather than replacing it.

What foods can you eat freely? Ask your GP, practice nurse or an NHS-referred dietitian for guidance tailored to you, because advice on carbohydrates and portions is individual.

What is the worst thing for prediabetes? Ignoring it. A result with no follow-up leaves you without monitoring. Book the follow-up test your GP suggests.

Can you have prediabetes and never get diabetes? Prediabetes is a risk marker, not a certainty. Regular monitoring is how any change gets picked up early.

07 · Getting treatment in the UK

NHS and private access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. A prediabetes result on its own is not what NICE lists in that criterion; weight and weight-related conditions are.

08 · Type 2 diabetes and monitoring

If prediabetes becomes type 2 diabetes

Mounjaro is also licensed for type 2 diabetes, where it improves blood sugar control (HbA1c) and supports weight loss. If you have type 2 diabetes, treatment and monitoring are coordinated differently, so tell your prescriber and your usual diabetes team so care stays joined up.1

This is why a prediabetes result should not sit in a drawer. If your status changes, the route to treatment, the person who monitors you and the paperwork all change with it. Keep your GP informed of any medicine you take, including one bought privately.

If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. Do not change them yourself; speak to your prescriber or diabetes team first.

09 · How the pen is used and side effects

Using a GLP-1 pen day to day

Mounjaro is a once-weekly injection from a pre-filled pen. Needles, a sharps bin and injection guidance come with every order, and you should follow the pen's instructions each time. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Store pens at 2–8°C until first use. Treatment starts at 2.5mg and steps up gradually.

If you stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

If you notice a new symptom, discuss it with your prescriber and report it through the Yellow Card scheme.19 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
