What "all-in-one" GLP-1 products actually are

"All-in-one" is a sales phrase, not a medical category. You will see it attached to capsules, drops, sprays, powders and sometimes injectables, usually alongside dramatic weight-loss claims. The common thread is a promise of many effects from a single product, often with the word GLP-1 borrowed from the licensed medicines to sound credible.

It helps to separate three different things that share the name. The first is a licensed GLP-1 medicine: a prescription-only product with a marketing authorisation, a defined strength and a prescriber involved. The second is a supplement that claims to "support" natural GLP-1. These are available cheaply without assessment, have no clinical oversight and do not carry the evidence of the licensed medicines. The third is an unlicensed or counterfeit injectable sold through social media or messaging apps, often by sellers offering to supply without a consultation or prescription. That last category is the one that causes real harm.

Why the confusion arises is easy to see. The honest answer is that the licensed medicines do act on several related pathways, but they do it as a single, tested molecule at a prescribed dose, not as a blend of extras.

Here are practical red flags to look for:

  • Dramatic weight-loss claims with no named clinical trial behind them.
  • No prescriber, no questionnaire and no follow-up.
  • No patient information leaflet, or one that cannot be found on a public medicines database.
  • Dosing described loosely rather than by a defined strength on the box.
  • Pressure to buy quickly or to pay by routes that cannot be traced.
  • A seller who cannot say where the product was sourced.

None of this means every capsule is dangerous; it means you cannot know what you are getting. The safest rule is simple. If a product is described as a GLP-1 treatment, you should be able to find its licence, its leaflet and the name of the prescriber who assessed you. If you cannot, treat it as unproven. 05 · How GLP-1 medicines work

How licensed GLP-1 medicines work

Licensed GLP-1 medicines work by mimicking natural gut hormones that regulate appetite, so most people feel fuller sooner and less hungry between meals. That in turn supports a reduced-calorie diet, which is the engine of weight loss. The medicine is a tool alongside diet and activity, not a replacement for them.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

So when a product is described as doing "many things in one", the nearest real-world version is a single licensed molecule acting on more than one receptor. That is a very different proposition from a mixed capsule. The mixed capsule has no defined mechanism, no standard dose and no trial; the licensed pen has all three.

Wegovy is a semaglutide product for weight management. Its pens come in 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once-weekly strengths, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 You can read about it on our Wegovy pen page. Liraglutide, the daily injection sold as Saxenda, is supplied at 6mg per millilitre and, unlike Mounjaro and Wegovy, is taken every day.

Is GLP-1 the same as Mounjaro? Not exactly. GLP-1 is the name of a gut hormone and of the class of medicines that mimic it. Mounjaro is a specific brand that also acts on the GIP receptor. Think of GLP-1 as the family and Mounjaro as one member with an extra action.

Beyond appetite, there is a practical effect worth knowing about. Because stomach emptying slows, some people notice nausea or digestive changes, especially after dose increases. No licensed medicine is the strongest for everyone. Different licensed medicines have different trials, different populations and different tolerability. Within Mounjaro, higher is not automatically better, and your prescriber guides the right pace for you.

06 · How much and how fast

How much weight, and how quickly?

Weight loss on a licensed GLP-1 medicine is gradual by design, and the trial headlines describe months of treatment, not days. If you have a specific weight target in mind, the honest framing is a trend over many weeks and months, shaped by your starting weight, your dose and your habits.

The tirzepatide figures come from SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

To translate a percentage into something personal, you can use a worked example. For someone starting at 100 kg, 20.9% is about 21 kg.

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1

Three months in, most people are still on the lower rungs of the dose ladder, so losses at this stage are modest compared with the headline figures. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.

07 · Dose ladder and schedule

The dose ladder and the weekly schedule

Mounjaro is taken once a week, not daily, and everyone starts on 2.5mg. That starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace. This is the opposite of the "more is more" logic that often accompanies blended products.

How much to take for weight loss therefore depends on your response, not on a fixed number. The ladder exists so that appetite effects arrive gradually and side effects stay manageable.

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.13

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember; many people choose a weekend morning so early side effects fall on days off.

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.13

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.

08 · Stopping and keeping weight off

Can you stop after losing weight?

You can stop a GLP-1 medicine, but it should be a planned decision with your prescriber. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

What happens after that varies from person to person. Regular meals with enough protein, consistent activity, good sleep and honest tracking all help you hold on to progress. A reduced-calorie diet and increased physical activity are part of the licensed use, so they are also the foundation for maintenance.

Beyond the scales, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures in trials, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance. They also explain why decisions about stopping are best made with a clinician who knows your history.

09 · NHS and private access

NHS or private: how people get a genuine GLP-1 medicine

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. For Wegovy, NHS access runs through specialist weight-management services; ask your GP.

10 · Storage, travel and daily life

Storage, travel and everyday practicalities

Mounjaro pens belong in the fridge, and storing them correctly protects the medicine. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels. This is another contrast with unregulated products, where storage advice is often vague or missing.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.13

Do you take a GLP-1 every day? Not with Mounjaro or Wegovy, which are once-weekly injections. Liraglutide is a daily injection. Whichever medicine you use, follow the schedule on your prescription rather than improvising, and keep a note of your injection day so it becomes routine.

When is the best time to take it? For Mounjaro, there is no best time of day, only the same day each week. If you take a supplement marketed as GLP-1 support, there is no standardised dosing or timing evidence in the licensed sense, which is another reason not to confuse it with the medicine.

Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
