Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 A smaller body needs less energy, so the intake that produced a deficit at your starting weight may now simply maintain you. That is physiology, not failure.
Tirzepatide works by activating GIP and GLP-1 receptors, which helps you feel fuller sooner and less hungry between meals, supporting a reduced-calorie diet.1 It supports your eating; it does not replace it. When portions creep up, the deficit quietly closes.

What to check first, in order
Start with the basics, because they explain most stalls. The licensed approach to a plateau is to review food intake honestly, protect protein and activity, check sleep and alcohol, and discuss a dose step-up with your prescriber if you are not yet at maximum.1

When a stall needs a prescriber review
A stall that lasts weeks is normal; a stall that sits alongside persistent side effects, or a long flat trend despite a solid routine, merits a conversation. Prescribers review response formally: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Adaptation, pace and realistic expectations
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1
Trials measured results over 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2 In SURMOUNT-1, average weight loss built month by month, with the full averages measured at the end of the trial, not within the first few months.2
Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.1 Trial averages do not predict your own result, and individual results vary.

05 · Diet, protein and movement
Food, protein and movement during a stall
Increased physical activity is part of the licence, not an optional extra.1 Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle.1
Start where you are and build. On injection days with early side effects, lighter movement still counts.1 Exercise also protects against regain if you later stop treatment.1
On the food side, the practical issue is drift. A quieter appetite makes it easy to graze on calorie-dense foods that do not feel like meals. Writing down what you eat for a short stretch is usually more revealing than any change of plan. Put protein at each meal, keep alcohol modest, and protect your sleep, since both can undermine a deficit without you noticing.
Many people also find the scales are the wrong tool during a stall. Waist, how clothes fit and energy levels often move when the number does not. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, though these are trial averages, not promises.2
06 · Dose and tolerance
Dose, tolerance and "it stopped working"
If you are not yet at maximum dose, a step-up may be worth discussing. Dose decisions belong to your prescriber, who weighs your response against side effects and tolerability.1 Some people respond well at a middle strength and stay there; there is no rule that everyone must reach the top dose.
Plateaus are described as the body adapting to a lower weight, not as the medicine itself wearing off. If the medicine seems to have lost its effect, treat that as a reason to talk to your prescriber, not to adjust it yourself. Continued treatment matters for outcomes. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 So a stall on treatment is a very different situation from stopping it.
It is also worth ruling out a practical cause, such as a missed or mistimed dose. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13

What not to do when weight loss stalls
A plateau is not a reason to double doses or add unlicensed products.1 Dramatic weight-loss claims from supplements or sellers offering to supply without a consultation or prescription are a warning sign, not a shortcut. 09 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
