01 · The cause
Why Mounjaro causes diarrhoea
Mounjaro causes diarrhoea because tirzepatide acts directly on the gut-hormone system. It activates two natural receptors at once, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and change how the body responds to insulin.1
Diarrhoea is more likely while the dose is being increased and becomes less frequent over time.1 That is why Mounjaro is started low and stepped up gradually.
How common is it?
The side-effect information lists nausea, diarrhoea, vomiting, constipation and abdominal discomfort as the most common effects, each affecting more than 1 in 10 people.113 The weight-management trials give the dose-by-dose picture:

Two things stand out. Rates rise a little with dose, and a real share of people on placebo also reported loose stools, so not every episode is the medicine. In those trials, 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.12 Most people who have diarrhoea carry on with treatment.
02 · What helps
What to do if you have diarrhoea on Mounjaro
The most useful first steps are practical: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while you adjust, stay well hydrated and avoid lying down straight after eating.1
If symptoms are mild and short-lived, you may simply need to ride out the adjustment period. If they are frequent, disrupting your day or not easing after a dose increase, speak to your clinical team. Prescribers can hold the current dose for longer before stepping up, rather than you pushing through severe symptoms.1 A pharmacist can also advise on what is suitable for you alongside your other medicines.

04 · How long it lasts
How long diarrhoea on Mounjaro lasts
For most people, diarrhoea on Mounjaro eases within days to a few weeks as the body adjusts.1 It tends to appear in the first days after starting or increasing a dose, peak early and then settle.1
If a new dose is hard to handle, speak to your clinical team: prescribers can hold the current dose longer before stepping up.1
Also remember that tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.1 Stopping will not end symptoms overnight, and nothing speeds this up.
05 · Dose increases
Why dose increases make it worse
Diarrhoea is more common while the dose is being raised and becomes less frequent over time.1 This is the reason Mounjaro is supplied in six strengths, 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, and started at the lowest one.
A practical way to think about it: each new strength is a new adjustment for your gut. The first weeks at 2.5mg let your body get used to the medicine, and each step up asks it to adapt again. The trial figures reflect this, with diarrhoea at 18.7% on 5mg rising to 22.5% on 15mg.1
A higher dose does not mean every person will have worse symptoms. Many people step up with barely a ripple.
If a step is hard to tolerate, tell your prescriber before the next dose rather than after. Holding at the current dose for longer is a recognised option, and it keeps you on treatment rather than forcing a stop.1
06 · What to eat
What to eat on Mounjaro to avoid diarrhoea
While your gut is adjusting, favour plain lower-fat foods, eat smaller meals slowly and stop at the first feeling of fullness.1
In practice, that means:
- Smaller portions rather than three large meals, eaten slowly.
- Plainer, lower-fat choices while symptoms are active, rather than rich, heavily fried or very fatty dishes.
- Steady fluids through the day, so you replace what loose stools remove.
- No lying down straight after eating.

07 · Dehydration risk
Dehydration: when diarrhoea becomes a problem
Diarrhoea becomes a problem when you cannot keep up with fluid losses, and dehydration needs prompt attention.1 If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
Staying well hydrated is the single most repeated piece of advice for Mounjaro's digestive effects, and it matters more when stools are loose. Sip water regularly rather than drinking large amounts at once, particularly if you also feel sick.

Contact the prescribing team promptly if you notice that symptoms are not settling, if you are unable to drink, or if you feel unwell in a way that is new. If you take other medicines, mention the diarrhoea, because they can be affected when you are unwell. People using insulin or a sulfonylurea alongside Mounjaro should also watch for low blood sugar (shakiness, sweating, confusion), as their prescriber may need to adjust those medicines.1
08 · Eating too little
What happens if you eat too little on Mounjaro
Mounjaro makes most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet, but eating far too little brings its own problems.1
Fatigue is a recognised common effect, most noticeable in the first weeks and after dose increases. It usually reflects a mix of the body adjusting and simply eating less, so keeping meals balanced, protein especially, staying hydrated and keeping gentle activity going all help.1 Constipation can also follow when people eat and drink less.1
Temporary hair shedding is a recognised effect too, linked to rapid weight loss itself rather than a direct drug effect, and nutrition matters for it. In the trials, hair loss was reported by 4.9% of people on tirzepatide against 1.0% on placebo.1
The takeaway: a small appetite is expected, but if you are struggling to eat enough, are very tired or are losing weight faster than your prescriber intended, tell your clinical team. A pace guided by your prescriber, rather than maximum speed, is the aim.
09 · Pancreatitis signs
Pancreatitis: the signs to know
The key warning sign of possible pancreatitis is severe, persistent stomach pain that may spread to your back, with or without vomiting, and it needs immediate medical help.113 Serious side effects are uncommon, but this is the one to take seriously.
- Early warning signs: severe, persistent abdominal pain that may spread to the back, with or without vomiting — stop using Mounjaro and get medical help straight away. That is a different picture from loose stools, which are usually painless or only mildly crampy.
- How to avoid it: there is no proven way to prevent pancreatitis or gallbladder problems while taking Mounjaro. Before you start, tell your prescriber if you have ever had pancreatitis, gallstones or gallbladder disease, or severe problems with your digestion.
- Whether pancreatitis goes away on its own, and what to eat with it, are questions for treating clinicians. Do not wait to see whether severe pain passes; get medical help straight away and follow the advice of the clinicians treating you.
Do not treat ordinary diarrhoea as a sign of pancreatitis. But if your diarrhoea comes with severe belly pain that will not settle, treat it as an emergency, not a side effect to manage at home. You can also report any suspected side effect through the MHRA Yellow Card scheme.19
10 · Other warning signs
Other signs that need urgent help
Beyond pancreatitis, two other groups of symptoms need urgent attention alongside any digestive upset. Seek immediate medical help for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could indicate gallbladder problems.1 Seek immediate medical help for signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.1
None of these is the same as ordinary diarrhoea, but they can appear during the same weeks when your gut is already unsettled, so it helps to know the difference. A rule of thumb: loose stools that ease with time and fluids are usually the expected adjustment. Pain that is severe, persistent, localised or paired with fever or yellowing is not — get medical help straight away.
