The most likely cause is eating far less than before. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which regulate appetite and slow how quickly the stomach empties, so most people feel fuller sooner.1 Less food means less fuel, and energy can dip while you adjust.

How to ease tiredness on Mounjaro

Small, steady habits work best. Eat smaller meals slowly, stop at the first feeling of fullness, and favour plain lower-fat foods while adjusting.13 Make sure what you do eat counts: protein especially, plus vegetables and whole grains.

  • Drink water regularly through the day and avoid lying down straight after eating.
  • Keep gentle activity going, such as walking, rather than stopping altogether.
  • Do not chase the fastest loss; a pace guided by your prescriber protects lean mass.
  • Keep a note of which days feel worst relative to your injection day.

How long does the fatigue last?

For most people energy returns to normal within a few weeks.1 Expect it to be most noticeable early on, and it may briefly return after each step up the dose ladder. The dose usually rises no faster than after at least 4 weeks on each dose, depending on how you respond, which gives your body time to adapt.1

If tiredness is not settling, your prescriber can review your dose, your eating and your hydration.

When tiredness needs a conversation

Tell your clinical team if tiredness is severe or not settling, or if it comes with persistent nausea or vomiting. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.13 If you take insulin or a sulfonylurea, watch for low blood sugar such as shakiness, sweating or confusion, as your prescriber may need to adjust those medicines.13

Any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17 You can also report suspected side effects through the Yellow Card scheme.19

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
