Is Mounjaro sold over the counter anywhere?
We are not aware of any country where Mounjaro is sold over the counter. We can only speak with authority about the UK, where it is a prescription-only medicine supplied after a consultation. Mounjaro is the brand name for tirzepatide, a once-weekly injectable made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1
Anything described as an off-the-shelf purchase deserves suspicion. The MHRA has seized thousands of fake weight-loss pens in the UK, and some fakes contained insulin rather than tirzepatide.7
What you can do in the UK is simple. A short online consultation replaces the in-person appointment, and a prescriber decides whether the medicine suits you. 02 · What it costs
What Mounjaro costs in the UK
Your first Mounjaro pen at the 2.5mg starting dose costs £129.99 on a first order with code FIRST40; the standard price is £169.99 per month. Each pen is a four-week supply. After the first order, every later order is £25 off the standard price (code DOSE25). The consultation is free, and the price shown is the price paid.
If you are switching from another provider at a higher dose, the same new-customer discount applies at any strength. Switching at 15mg means your first order is £289.99, a first-order price rather than a monthly one. 04 · Why it is prescription-only
Why Mounjaro is prescription-only
Mounjaro is prescription-only because it is a potent injectable medicine whose suitability depends on your health history. It is the brand name for tirzepatide, a once-weekly injection from Eli Lilly, supplied as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 In the UK it is licensed for weight management in adults and for type 2 diabetes.
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).
A medicine that changes appetite and stomach emptying needs a proper assessment. A prescriber looks at your BMI, medical history and current medicines before deciding whether it suits you.1 That judgement cannot be made at a shop counter.
Tirzepatide also stays in the body for a long time. It has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That long tail is another reason a prescriber should plan starting, dose changes and stopping with you, rather than you improvising.
The patient information leaflet sets out who should not use it, what side effects to expect and when to seek help.13 Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Mounjaro should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Reading it before your first dose is sensible whichever route you use. If a seller discourages you from reading it, or ships pens without any leaflet, treat that as a serious warning.

05 · Who is eligible
Who can be prescribed Mounjaro
Adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition, can be considered for Mounjaro for weight management. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.
Those criteria describe the licence, and a prescriber still decides case by case. They will assess your BMI, your medical history and your current medicines before deciding whether it is suitable.1 That is the point of the free consultation: it is a clinical review, not a scoring script. Some people will be told Mounjaro is not right for them, and in that case the order is refunded automatically in full.
Why does a clinical filter matter more than a convenient country? Because the benefits are real but depend on the right person using it properly. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
For a worked example, someone starting at 100 kg would see 20.9% as about 21 kg, but that is the straight arithmetic of a trial average, not a prediction for you. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2
Anyone telling you results come without a diet change, or promising specific results, is selling something other than the licensed medicine. 06 · NHS versus private
NHS access versus private treatment
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.
That phasing is the honest reason many people look elsewhere. If you fall outside the early groups, you may wait a long time, and some people who meet the licensed criteria choose regulated private treatment rather than waiting. The choice is not a trap in either direction. If you qualify for NHS treatment, the NHS route may suit you well, and your GP is the person to ask. If you do not qualify yet, or would rather not wait, a regulated private prescription is the lawful alternative. Both routes involve a clinician. Neither involves buying a pen off a shelf.

07 · Counterfeits and risky sellers
Fake pens and the risk of 'no prescription' sellers
The biggest danger of an over-the-counter route is a fake pen. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Genuine treatment always involves a prescription and a clinical consultation.
Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.12 You can check our listing yourself: we are registered with the GPhC under number 9012878. Checking takes a minute and is the single most useful step you can take.
Warning signs worth treating as a stop sign:
- A seller offering to supply without a consultation or prescription.
- Sales through social media messages, salons, market stalls or marketplace listings.
- Prices far below what the regulated pharmacies show.
- A pen with no batch number or expiry date, or arriving without cold-chain packaging.
- Claims that a particular country or website is a 'loophole'.

Once a pen arrives, look at it. A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.13
If you experience a suspected side effect from any pen, report it through the MHRA Yellow Card scheme.19 Suspected fakes can be reported through the same MHRA channels.
The pull of a cheap, quick solution is understandable, but a pen of unknown content can cost you far more than the price difference. The regulated route costs a few minutes of questionnaire time, and that is what keeps the product and the person safe.
08 · Planning your costs
Planning what a year of Mounjaro costs
If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year of 52 weeks (13 pens) costs £2,711.88 if you settle at 5mg, and up to £3,599.88 if you settle at 15mg. These totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less.
Most people do not stay on the starting dose. Treatment begins at 2.5mg and steps up under prescriber guidance, so your monthly cost changes as you move through the doses.

09 · Foundayo and newer options
Foundayo, Wegovy tablets and the newest weight-loss medicines
Mounjaro is not the only GLP-1-type treatment people ask about, and the same rule applies to every one of them: they are prescription medicines, not shelf products. Foundayo (orforglipron) is a film-coated tablet with a UK product information entry, and like Mounjaro it is supplied on prescription.21 Wegovy tablets (oral semaglutide) also have UK product information and are prescription-only.20
Anything advertised on a marketplace or by a seller offering to supply without a consultation or prescription should be treated as a warning sign, not a bargain. For current availability and pricing of the treatments we supply, see all weight loss treatments.
Tablets and injections differ in practical ways. Mounjaro and Wegovy are once-weekly injections, with Wegovy's pens running 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks to reach the 2.4mg maintenance dose from week 17.18 Liraglutide products such as Saxenda are a daily injection supplied at 6mg per millilitre. A prescriber can explain which format suits your routine and health history.
SURMOUNT-5 is the trial that directly compares tirzepatide with semaglutide.15 Trial numbers from separate studies are not directly comparable, because the participants, designs and durations differ.

10 · Using pens safely
Using and storing your pen safely
Each Mounjaro KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it.1 Some online posts describe drawing out 'extra' or 'golden' doses with syringes. No regulated UK pharmacy or prescriber can endorse it.
The same logic explains why a pen from an unverified source is risky. You cannot confirm what it contains or how it was stored. The pen is a precisely made device, and the contents are only as trustworthy as the supply chain behind them.
Storage matters too. Store unopened pens in the fridge until first use, and they keep until the printed expiry date. For any question about an unopened pen being out of the fridge, check the patient information leaflet and ask the pharmacy.13 Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1 For the injection itself, follow the pen's instructions. Needles, a sharps bin and injection guidance are included with every order.
Before each dose, look at the solution. It should be clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use.13 If anything seems wrong, contact the pharmacy before injecting.
Most side effects are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they are most likely when starting treatment or moving up a dose. Other medicines can interact. The MHRA has guidance on contraception with GLP-1/GIP medicines, and Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mention all your medicines, including contraception, insulin or sulfonylureas, in your consultation. Doses of insulin or sulfonylureas may need adjusting because of the risk of low blood sugar.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
