What "safe" means when you buy an appetite suppressant
A safe purchase has three parts: a verifiable seller, a clinical assessment and a genuine, traceable product. If any one is missing, treat the offer with caution, however convincing the website looks.
First, find the seller on the GPhC register. A real online pharmacy shows its registration number and you can look it up. We are GPhC-registered (9012878).12
Second, expect questions. Genuine treatment always involves a prescription and a clinical consultation.7 A seller offering to supply a prescription-only weight-loss medicine without a consultation or prescription is a warning sign, not a bargain.

03 · How prescription treatment works
How prescription appetite-reducing treatment works
Mounjaro reduces appetite by acting on two natural gut-hormone receptors at the same time. It is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1
Tirzepatide activates GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1
That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 It is why many people now use "appetite suppressant" to describe it, although the licensed description is a weight-management medicine used alongside lifestyle changes.
This matters for safe buying because the mechanism is a pharmacological one. A medicine that changes how your gut hormones signal needs a prescriber to check your medical history and current medicines first. A box from a shelf or a website that skips that step is not offering the same thing.
Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
It stays in the body for some time. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Think of it as a treatment you start with a plan rather than something you pick up and put down casually.

Finally, treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Your prescriber and the monthly reviews we include are there to support that.
04 · What the evidence shows
What the trial evidence shows
In the SURMOUNT-1 trial, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 The trial included 2,539 adults.
These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity.2 Trial averages do not predict your own result.
A worked example makes the percentages concrete. For someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. Your starting weight, dose, adherence and health will all change your own number.
The trial also reported how many people reached particular thresholds at 72 weeks. 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
Beyond the scales, weight loss on tirzepatide in trials came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes rather than promises. They are why prescribers talk about weight management as health management rather than appearance.
Trial evidence sets the bar for buying safely. A licensed medicine arrives with named trials, a published product information leaflet and a regulator watching safety. When a supplement seller makes dramatic weight-loss claims, ask where the named trial is, who ran it and which product it tested.

05 · Eligibility and NHS access
Who can use it, and what about the NHS?
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.1 Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.
A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. That decision belongs to the prescriber, which is part of what makes the route safe. It is not a box you tick yourself.
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
The NHS tirzepatide guide sets out the current picture.5 If you think you may qualify, ask your GP first. If you do not yet qualify, or do not wish to wait, a private prescription from a GPhC-registered pharmacy is the regulated alternative.
Who should not take it? Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems. Always share your full medical history and medicine list.
A supplement seller does not ask about your pancreas, gallbladder or pregnancy plans, and cannot adjust anything if you take other medicines.
If your BMI falls below the licensed range, that is information too. It means a prescription weight-loss injection is not the right route for you at present, and a conversation with your GP or pharmacist about food, activity and other support will serve you better than buying something online. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
06 · Spotting fakes and unsafe sellers
Spotting fakes and unsafe sellers
The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified.7 Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide. That is a strong reason to be careful where you buy.
Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.12

Warning signs worth knowing:
- The seller offers to supply a prescription-only medicine without a consultation or prescription.
- You can only contact them through a social media message or messaging app.
- The seller cannot give a GPhC registration number that matches the name on the website.
- The pen has no batch number or expiry date, or arrives without cold-chain packaging.
- The offer is built on urgency or a price that seems too good to be true.
When a pen arrives, check it. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.13 Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been kept out of the fridge for more than 30 days after first use, or has been above 30°C. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.
Unopened pens belong in the fridge until first use and keep until the printed expiry date. For anything beyond that, read the patient information leaflet and ask the pharmacy.

07 · Over-the-counter and natural options
Over-the-counter and "natural" appetite suppressants
Over-the-counter and natural appetite suppressants are low-cost supplements with no clinical oversight, so there is no prescriber checking whether they suit you. Those are marketing labels, not descriptions of a licensed medicine.
Dosing is not standardised from one supplement to the next. Dramatic weight-loss claims on a label or sales page are not evidence. Ask what trial the claim comes from, and whether that trial tested the exact product in the tub.
- It is not a substitute for assessment. A supplement does not ask about your medicines, conditions or pregnancy plans.
- Natural does not mean harmless. Talk to a pharmacist before combining any supplement with prescribed medicines.
- Check the seller anyway. Buy from a pharmacy or retailer you can identify, not from a social media advert.
- Report problems. If you have a side effect from any product, you can report it through the MHRA Yellow Card scheme.19

The honest summary: a supplement may be easy and cheap to buy, but "easy to buy" is the opposite of what makes a purchase safe. The more a product acts like a medicine, the more it should involve a person who can say no to you.
One further caution. Because supplements are cheap and need no assessment, they can feel risk-free. That feeling is the reason to slow down, check the seller and ask a pharmacist, not a reason to relax.
08 · Side effects and safety
Side effects and safety to know before you start
Serious side effects of Mounjaro are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis.13 Pain in the upper right abdomen, fever or yellowing of the skin or eyes could signal gallbladder problems; get medical help straight away. Swelling of the face or throat and difficulty breathing are signs of a severe allergic reaction; get medical help straight away.
If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, such as shakiness, sweating or confusion. Your prescriber may need to adjust those medicines.13
Thyroid. In rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.1
Kidneys and liver. Mounjaro is not known to damage kidneys or liver directly, and no dose adjustment is needed for mild to moderate impairment. The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys. Keep fluids up and get medical help straight away if you cannot keep fluids down. People with severe impairment need individual prescriber assessment.1
Mood. Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Even so, a big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.
If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.
Contraception. The medicine can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
Report any suspected side effect through the Yellow Card scheme.19
This list is the reason a prescriber review is not a formality. It is also the reason that a product with no assessment, no ongoing review and no named person to contact is a poor choice. Comparing before you buy is sensible, as long as you compare like for like.
Compare a supplement purely on price and it will usually look cheaper. But a low-cost supplement with no clinical oversight is not a cheaper version of a prescription medicine. They are different products with different evidence and different safeguards.

10 · Other prescription options
Other prescription options to ask about
Mounjaro is not the only prescription weight-management medicine, and a prescriber can talk you through alternatives if it is not right for you. The key point is the same for all of them: they are prescribed after a consultation, not bought off a shelf.
Wegovy (semaglutide) is a once-weekly injection. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, and the dose steps up after at least 4 weeks, reaching the 2.4mg maintenance dose from week 17. Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy. For anything beyond that, such as results or dosing, your prescriber can advise.
