A quick overview

What to know
before you start.

  • The licensed maximum is 15mg once weekly; there is no higher Mounjaro strength.
  • Everyone starts on 2.5mg, which is for adjustment rather than weight loss itself.
  • The dose can rise at a minimum of four-week intervals, depending on response and tolerability.
  • Many people settle on 5mg, 10mg or 15mg; higher is not automatically better.
  • Your prescriber decides the pace, and can hold or step down a dose within the licence.
In this guide

01 · The core answer

The maximum Mounjaro dose is 15mg once weekly

The highest licensed Mounjaro dose is 15mg, injected once a week. The pen range runs 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, and nothing sits above that top strength.1 The licence covers adults for weight management and for type 2 diabetes.

You cannot begin at the top. Everyone starts on 2.5mg once a week. That starting dose is about letting your body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg, and it can then step up by 2.5mg at a minimum of four-week intervals, depending on how you respond and tolerate it.111

Because each step takes at least four weeks, reaching 15mg is a gradual process.

02 · Is it for you?

Do you actually need to reach 15mg?

No. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber guides the right pace based on how you respond and how well you tolerate each step.1

The honest way to think about it is that the maximum is a ceiling, not a target. If your appetite is well controlled and your progress is steady on a lower strength, there is usually no clinical reason to climb further just because a higher pen exists. Equally, if progress stalls and you tolerate the medicine well, your prescriber may discuss the next step.

If you are weighing up a dose increase, our guide to upping your Mounjaro dose and the Mounjaro stages page cover the wider journey.

03 · What to do next

What to do next

If you are already on treatment, talk to your prescriber before changing anything; never move up faster than the licensed schedule and never use a pen in a way it was not designed for. If you are researching a start, the sensible question is not "how high can I go?" but "what is the lowest dose that works for me?"

When you are ready, you can see current strengths and prices on the Mounjaro pen page. All six doses are in stock, and your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). The 15mg pen has a standard price of £329.99, and every later order is £25 off the standard price. Every order follows a free consultation reviewed by a GPhC-registered prescriber.

04 · How the dose ladder works

How the Mounjaro dose ladder works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The ladder is deliberately slow. Everyone starts on 2.5mg once a week. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of four-week intervals, to 7.5mg, 10mg and 12.5mg, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111

Notice the words "at least" and "minimum". The four-week gap is the shortest permitted interval, not a timetable you are obliged to follow. Nobody is required to reach the top.

The pen itself reflects this design. Each KwikPen contains four weekly doses, so a single pen covers a four-week step.

Why start so low?

The 2.5mg starting weeks are about adjustment. Experiences differ widely: some people notice an appetite change and early loss in the first week, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Our smallest Mounjaro dose and beginner dose guides cover that opening phase in more detail.

How the medicine works at every strength

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

That is why the ladder matters. Stepping up gradually gives your prescriber time to judge whether the current strength is already doing the job.

05 · Why higher is not better

Why a lower maintenance dose is often the right one

Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1 That single idea answers the most common misunderstanding about the maximum: the top strength is the upper limit of the licence, not the goal of treatment.

Think of the dose as a thermostat rather than an accelerator. The aim is to find the setting at which appetite is controlled, progress is steady and side effects are manageable. For some people that is 5mg. For others it is 10mg. For others still, it is the full 15mg. All three are legitimate, licensed outcomes.

What "settling" looks like in practice

Settling on a dose means you and your prescriber agree that the current strength is working and you stay on it. You continue weekly injections on the same day each week, alongside a reduced-calorie diet and increased physical activity. Your progress is reviewed over time, and the dose is only changed if there is a reason to.

It also means you do not need to chase the next pen. If the lower strength is already doing its job, there may be little reason to move up.

How response is reviewed

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4 Notice the wording: the highest tolerated dose, not the highest available dose. Tolerability is part of the licence logic, which is why a person who feels unwell at a higher strength is not expected to push through it.

Higher strength versus better result

It is natural to assume more medicine means more weight loss. But an average across thousands of people does not tell you what your own body needs. If you want to understand how results are measured, see our guide to maximum weight loss on Mounjaro.

06 · What trials show by dose

What the SURMOUNT-1 trial showed at each dose

The main trial behind the weight-management licence is SURMOUNT-1, and it reported results for the 5mg, 10mg and 15mg doses. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 A trial average does not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Reading these figures honestly

It also helps to remember what the numbers are. They are group averages measured over 72 weeks, in a trial setting, with participants following a lifestyle programme. They are not a promise of what will happen to you, and they should not be used to calculate how quickly you will lose weight.

A worked example, with its limits

To make a percentage tangible: for someone starting at 100 kg, 20.9% is about 21 kg. That is straight arithmetic on the trial average, not a forecast. Your starting weight, your diet, your activity, your tolerance of each dose and your biology all shape what actually happens, and the trial says nothing about how many weeks any particular amount of loss takes.

What the trial does not tell you

SURMOUNT-1 does not tell you the "right" dose for any one person. The decision of how far to climb belongs to you and your prescriber, based on your response and tolerance rather than on the trial table.

Nor should you compare these figures with results from other medicines' trials as if they were head to head; different trials enrol different people and are designed differently. If you are comparing treatments, ask your prescriber to explain the options in your own situation.

07 · Side effects and stepping up

Side effects when you step up a dose

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

That is the reason the ladder moves slowly. Each four-week stretch gives you time to see how a strength sits with you before the next increase. If a step feels difficult, that information matters, and it is a conversation for your prescriber rather than something to push through alone.

Holding a dose or stepping down

Holding is not failure; it is the system working as designed. The licence allows the dose to rise depending on how you respond and tolerate it, which means tolerance is as much a part of the decision as results.

Many people find that a calm, steady approach works best: stay on a strength until it feels settled, and only then consider whether another step is worth it. If you are on a lower strength and doing well, there is no clinical pressure to move.

What not to do

The licensed dose ladder already starts low, at 2.5mg, for tolerability. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

The same logic applies in the other direction: do not try to jump ahead of the ladder or exceed the top strength. If you feel the current dose is not enough, or too much, the safe route is always to speak to your prescriber, who can adjust within the licence.

Reporting new symptoms

If you notice a new symptom after a dose change, tell your prescriber; for severe, persistent stomach pain, chest pain, swelling of the face, lips, tongue or throat, difficulty breathing or yellowing of the skin or eyes, get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19 The patient information leaflet lists the recognised side effects.13

Our team offers ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team, plus side-effect support, so you are not making these decisions on your own.

08 · Injecting at higher doses

Injecting, missed doses and changing day

The way you inject does not change as the dose rises. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.113

Because the schedule is the same at every strength, the habit you build in the starting weeks carries you all the way to maintenance. Follow the pen's instructions each time, and keep your sharps in the bin supplied; needles, a sharps bin and injection guidance are included with every order.

Is there a best time of day?

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember, and many people choose a weekend morning so early side effects fall on days off. That tip becomes more useful at higher strengths, when you may want a quieter day after a step up.1

What if you miss a dose?

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1

This rule is the same at every strength, and you should never take two doses within 3 days of each other. If you are unsure, check with the pharmacy or your prescriber before injecting.

Changing your injection day

You can change your weekly injection day if needed, as long as there are at least 3 days between two doses.1 People often do this around holidays or work patterns. Pick the new day and keep it from then on.

Clearing the medicine

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

09 · Storage and travel

Storing and travelling with higher-strength pens

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13

These rules are the same whichever strength you are on, because every pen is the same device.

Unopened pens

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK leaflet is the reference to trust.13

Flying and holidays

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.1

Planning around a dose change

If you expect to step up during a trip, talk to your prescriber beforehand about timing. Changing strength mid-holiday adds a variable you do not need, and there is usually no reason to rush a step. Keeping the pens cool, labelled and in your hand luggage is the practical priority.

Orders arrive by free next-day tracked delivery in temperature-controlled, discreet packaging, with same-day dispatch when ordered by 3pm. Once the pen is with you, return it to the fridge unless it is your in-use pen.

Discarding pens

When an in-use pen reaches the end of its out-of-fridge window, discard it even if medicine remains.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

11 · NHS and private access

Reaching a high dose: NHS versus private treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

The dose ladder is the same either way. Whether you are treated through the NHS or privately, the licensed maximum is 15mg weekly, the starting dose is 2.5mg and the steps are no faster than after at least four weeks. What differs is the route to getting a prescription and who provides your reviews.

Choosing a regulated private route

If you choose private treatment, the safest approach is a registered UK pharmacy with a prescriber review. EverydayMeds is GPhC-registered, number 9012878, and you can check the register yourself. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber, used alongside a reduced-calorie diet and increased physical activity.12

Treat any seller offering to supply without a consultation or prescription as a red flag. Counterfeit pens are a real risk; our supply comes directly from Eli Lilly's UK distribution through LillyDirect, with genuine KwikPens.

How ordering works

The journey has a few simple steps. First, you complete a free online consultation, a short health questionnaire with no charge and no obligation. Next, a GPhC-registered prescriber reviews it, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

Then our registered pharmacy dispenses and dispatches your order, with same-day dispatch when ordered by 3pm. Delivery is free next-day tracked (DPD), and Saturday delivery is £14.99. Repeat orders re-check suitability, and there is no auto-shipping, so every dose decision stays with a prescriber.

What about cost at higher doses?

A first order is £40 less at any dose with code FIRST40, and every later order is £25 off the standard price (code DOSE25). Our Mounjaro pen page shows every strength with live prices, checked 12 October 2026. Payment options include Visa, Mastercard, Klarna and pay-monthly options, with no subscriptions.

For broader support beyond the injection, our weight loss clinic explains the wider programme, and the free GLPTrackr app helps you record doses and progress.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
What is the maximum dose of Mounjaro?
The maximum licensed dose of Mounjaro is 15mg once weekly, injected under the skin. The pen range includes 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, and there is no strength above 15mg. Everyone begins on 2.5mg and steps up at a minimum of four-week intervals, depending on response and tolerability. Your prescriber decides how far you climb, and many people settle on a lower maintenance dose rather than reaching the top.
Do I have to go up to 15mg?
No. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. The 15mg strength is the ceiling of the licence, not a target everyone must reach. If your appetite is controlled and progress is steady on a lower strength, your prescriber may be happy to keep you there. The right dose is the one that works for you and that you tolerate comfortably.
How quickly can I reach the maximum dose?
The fastest the licence allows is a minimum of four-week intervals between steps. After at least four weeks on 2.5mg the dose goes to 5mg, then rises by 2.5mg at a time up to 15mg, depending on how you respond and tolerate it. That is a minimum, not a schedule you must follow.
Can I start on a higher dose of Mounjaro?
No, everyone starts Mounjaro on 2.5mg once a week. The starting dose is about letting your body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg.
What if I am not losing weight on my current dose?
Progress is gradual by design, so judge it by trend over months rather than single weigh-ins. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. Do not change your dose yourself; speak to your prescriber about the options within the licence.
Can I step back down from a higher dose?
Yes. Stepping down is not failure; the licence ties each increase to how you respond and tolerate it. If a strength feels too much, tell your prescriber rather than pushing on. Never alter doses outside the licence, as that means unknown dosing accuracy and no safety data.
What happens if I miss a dose at a high strength?
The rule is the same at every strength. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other. If unsure, ask your pharmacy or prescriber.
Is the maximum dose the same on the NHS and privately?
Yes, the licensed ladder is the same whichever route you use: start at 2.5mg, step up at a minimum of four-week intervals, up to a maximum of 15mg weekly. Mounjaro is available on the NHS, but access is being phased in over several years, with the highest-need groups first. Many people who meet the licensed criteria choose regulated private treatment, which still requires a prescriber review before any dose is supplied.
