03 · Access

Getting treatment: NHS or private

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). The amount never drops in a straight line, which is why clearance is described over several half-lives rather than a single cut-off moment.

05 · Why weekly dosing

Why the half-life supports once-weekly dosing

Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly, and its long half-life is the reason a weekly schedule works. Because tirzepatide leaves the body slowly, the amount present stays relatively steady from one injection to the next rather than spiking and collapsing.1 That steadiness is practical as well as pharmacological: a single pen routine is easier to keep to than a daily injection.

Contrast this with liraglutide, supplied at a concentration of 6mg per millilitre, which is a daily injection. Mounjaro and Wegovy are both once-weekly. The difference in dosing frequency reflects how long each medicine stays active in the body, which is exactly what half-life measures. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

Pick a day you can keep to, because the weekly rhythm is designed around that long half-life. Some people see a long half-life and wonder whether they could inject less often, or take smaller amounts, and still keep the effect. Altering the licensed schedule is off-label, and the half-life is only one part of why the schedule was set. After a final dose, Mounjaro takes roughly four to five weeks to clear the system: tirzepatide has a half-life of about five days, so there is no way to speed this up, and nothing needs doing while it clears.1

The word "roughly" matters. Clearance depends on dose, individual physiology and how long you have been treated. Four to five weeks is the clearance window, not a deadline by which every effect disappears at the same moment.

Can you speed it up?

No. There is no way to speed this up. Drinking more water does not clear it faster, detox drinks and teas do not either, and exercise does not either. These claims are marketing. Staying well hydrated is generally sensible, especially if you have had stomach upset, but it does not change how quickly tirzepatide leaves your body. If a seller offers a product to "flush" Mounjaro, treat that as a warning sign.

Peak levels and the weekly pattern

Levels build after each injection and then decline until the next dose, and the long half-life means the swings are gentle.

Can you stop "cold turkey"?

You can stop at any time, and because the medicine clears gradually you will not feel an abrupt stop in the way you might with a short-acting medicine. But do not stop without telling your prescriber, particularly if you are taking it for type 2 diabetes or use other glucose-lowering medicines, where doses may need adjusting. 07 · Building up and titration

How the half-life shapes titration and build-up

Because tirzepatide leaves the body slowly, each weekly dose adds to what is already there before the level settles into a pattern. Everyone starts Mounjaro on 2.5mg once a week; this starting dose is about letting your body adjust, not about weight loss itself. The starter pen is 2.5mg, and the strengths run through 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Your prescriber decides when to step up, usually at your monthly review, based on how you are tolerating treatment and how you are responding.1

Why the build-up is gradual

A medicine that clears slowly means any dose change takes time to show its full effect, and also time to wear off if it is not suited to you. This is a general principle of long half-life medicines. It is also why stepping up too quickly is a poor idea: the effect of a new strength is not fully visible straight away, so you cannot judge it from a single injection.

Missed or delayed doses

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day; if more than 4 days have passed, skip the missed dose and take the next one on your normal day — never take two doses within 3 days of each other.13 Do not invent your own catch-up approach. Extra doses or stretching the interval are not safe or licensed; follow the licensed schedule.

08 · Stopping and weight

Stopping Mounjaro: what the half-life does and does not tell you

The half-life tells you how long the medicine takes to leave your body, but it does not predict what happens to your appetite or weight afterwards. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. What happens to your appetite and weight after stopping depends on how you eat, move and live in the following weeks, and on your own biology. Trial results show the benefit of treatment while taking it: in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Appetite after the last injection

As the medicine clears over the four to five weeks, many people notice that the fullness they relied on fades gradually. That is a general expectation, not a prediction. Because the decline is slow, there is time to adjust, which is a good reason to plan with your prescriber rather than stopping suddenly and hoping for the best.

Planning a stop or a break

If you are considering stopping, think about what will replace the support the medicine gave you: regular meals, protein, movement, sleep and a plan for the situations that previously led to overeating. Some people ask whether Mounjaro has to be taken for life. The pharmacology shows only that stopping is not instantly felt, not that treatment must continue forever. What you decide should be driven by your goals, health and prescriber advice.

What the half-life cannot tell you

Safety, interactions and side effects while it clears

Because tirzepatide stays in your body for weeks after the final dose, some safety points carry on for a while after you stop. The key rule is to tell any clinician treating you that you have recently taken Mounjaro, and to give a rough sense of when you last injected. They can then apply their own guidance rather than assuming the medicine has gone.

Oral contraception

Tirzepatide can reduce absorption of oral contraceptives, because it slows how quickly the stomach empties. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Because the medicine takes weeks to clear, that consideration does not end the moment you stop injecting. Speak to your prescriber or pharmacist about what contraceptive arrangements are appropriate, and follow the patient information leaflet and MHRA guidance.713

Diabetes medicines

If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. That is not a prohibition, and it is not a reason to avoid treatment; it is a reason for your prescriber to review your doses. The same applies when you stop, because the effect on blood sugar changes gradually as the medicine clears. Do not change diabetes medicines on your own.1

Side effects don't switch off instantly

Any side effect you experience may ease over the weeks of clearance rather than overnight. Pen storage life is not the same as half-life

The half-life describes what happens to tirzepatide inside your body; storage life describes how long the pen stays suitable for use. People often mix these up, especially when they see "days" in both contexts. They are unrelated measures, and neither can be used to estimate the other.

Unopened pens

Store unopened pens in the fridge, at 2–8°C, until first use. They keep until the printed expiry date. For any question about how long an unopened pen can be out of the fridge, check the patient information leaflet and ask the pharmacy rather than relying on a figure from a forum or a blog.113

Pens in use

Once you start using a KwikPen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. That is the UK licensed in-use window, and it applies to the pen, not to the medicine inside your body. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
