Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals.
That slower emptying explains why meal size and meal type matter more than they used to. A portion that felt normal before treatment can now feel heavy. Food that is rich in fat can feel worse still, which is why people often notice discomfort after a takeaway they previously enjoyed.
The medicine is also a direct cause of loose stools. Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 So diarrhoea can appear even if you eat carefully.

Portion size matters as much as the food itself. A small amount of something rich eaten slowly is often tolerated better than a large plate eaten quickly. Stop at the first feeling of fullness rather than finishing what is in front of you.

Very sugary food is another common source of discomfort for many people on treatment.1 Cakes, sweets, pastries and sugary drinks are the obvious examples. Because appetite is lower, it is also easy to fill a small appetite with sugary items that offer little protein or fibre.
Swapping them out has a second benefit. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit rather than a perfect food list.1 Meals built around protein, vegetables and fibre keep you fuller and are usually gentler on the gut.

A big meal tends to sit heavily because the stomach empties more slowly.1 People often describe fullness, nausea, reflux or burping, and sometimes loose stools afterwards. Reflux and burping are listed as common effects.13
The practical fix is simple. Eat smaller meals slowly, stop at the first feeling of fullness and avoid lying down straight after eating.13 If you overdo it one evening, return to your normal pattern at the next meal rather than skipping meals to compensate.

Can you eat takeaway or McDonald's on Mounjaro?
Yes, because no food is banned on Mounjaro, but fast food is often a poor fit for how you now digest.1 Burgers, fries and similar items tend to be very fatty or fried, which is exactly what many people feel worse after. Large combo portions add to the problem.
If you do eat fast food or takeaway, a few habits help. Choose the smaller portion, pick plainer and lower-fat options where you can, eat slowly and leave what you cannot finish. Add a protein source and drink water rather than a sugary drink.
A better test is how you feel in the hours after eating. If the same meal repeatedly causes diarrhoea, treat it as personal information and adjust.
Takeaway meals can also be harder to portion. Serving yourself a plate from a shared container, rather than eating from it, makes it easier to stop at the first feeling of fullness.
08 · Eating too little
What are the risks of eating too little on Mounjaro?
Eating too little can leave you tired, short of protein and short of fluid. Fatigue is a recognised common side effect, partly because of the body adjusting and partly because of simply eating less.
Keep meals balanced, with protein especially, stay hydrated and keep gentle activity going. For most people energy returns to normal within a few weeks, and you should tell your clinical team if tiredness is severe or not settling.
Muscle and hair are also affected by poor intake. Some of any rapid weight loss comes from lean tissue, so protein at every meal matters more, not less, as the dose rises and appetite falls. Temporary hair shedding is linked to rapid weight loss itself, and overall nutrition matters.
Loose stools can also reduce what you absorb and what you feel like eating, creating a cycle. Small, regular, plain meals are usually the way out.
09 · Hydration and warning signs
Hydration, and when diarrhoea needs help
Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class.
Keep water within reach through the day. Treat inability to keep fluids down as a contact-your-clinical-team issue, not something to push through. If nausea or vomiting is persistent, dehydration needs prompt attention.
Some symptoms need urgent medical help. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Pain in the upper right abdomen, fever or yellowing of the skin or eyes could be a gallbladder problem; get medical help straight away. Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, also need immediate medical help.
If you take insulin or a sulfonylurea, watch for low blood sugar, such as shakiness, sweating or confusion, because your prescriber may need to adjust those medicines. You can report any suspected side effect through the MHRA Yellow Card scheme.19

In the weight-management trials, diarrhoea was reported by 18.7% of people on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.2 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.

A practical eating plan for sensitive days
On a day when your stomach is unsettled, keep to a simple routine. Prioritise protein at each meal, add vegetables and fibre as you tolerate them, and sip fluids regularly.1 Smaller, slower meals work better than large ones.
Plain, lower-fat food is the sensible default while you adjust. Examples might include grilled lean protein, plain rice or potatoes, cooked vegetables and soft fruit. These are practical suggestions rather than a prescribed diet, and you should adapt them to your own tolerance and any medical conditions.
Alcohol deserves a note. There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals.1 If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar.
