1. You are outside the licence. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. 03 · Licence and BMI

Licence criteria and BMI thresholds

The single most common decline reason is a BMI that sits outside the licence. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.111 It must be used alongside a reduced-calorie diet and increased physical activity.

That gives three groups. If your BMI is 30 or above, you meet the weight criterion. If it is between 27 and 30, you also need a weight-related condition. If it is below 27, the licence does not cover you for weight management, however much you would like to lose.

The condition examples in the licence wording are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes.1 People in the 27 to 30 band are often declined because they did not mention a condition they have, or because the condition was never formally diagnosed. If you have a diagnosis, say so clearly and name it in your consultation.

Why do prescribers hold this line? A licence defines the group in which a medicine's benefits and risks have been assessed. Prescribing outside it is a different decision, and a responsible online prescriber will not make it on the strength of a short questionnaire. That protects you as much as it protects the pharmacy.

Some people are surprised by a decline because they feel they carry weight that matters to them, or they have a smaller frame, or they have lost weight recently. BMI is a blunt measure, but it is the licensed one, and the prescriber works from it. If you recently lost weight and your BMI has dropped below the line, you may be declined even though your starting weight qualified.

Honesty matters here. Entering a different height or weight to pass a threshold defeats the purpose of the assessment, and a prescriber who notices inconsistencies may decline for that reason alone. Accurate numbers keep you safe.

04 · Medical history checks

Medical history that leads to a decline

Your medical history is the second big reason for a decline, because a prescriber must check that the medicine is safe for you before approving it. The prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.1

You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, or during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.113 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

The questionnaire asks about your past and present health. The prescriber reads your answers against the product information. If an answer matches a reason not to use the medicine, the request is declined. If an answer is borderline, the prescriber may need more information or may decide the risk is too high to manage remotely.

A decline on safety grounds is not a criticism. Many people find it disappointing, but it means a clinician looked at your situation and chose not to take a risk with it. If you disagree, the right route is your own GP or a specialist who knows your full record, not a different website that asks fewer questions.

That last point matters. Any service offering to supply tirzepatide without a consultation or prescription is not operating safely, and a faster yes from such a service is not a better answer. Check any pharmacy on the GPhC register before you trust it. Ours is registration 9012878.12

To reduce the chance of a decline caused by avoidable confusion, prepare before you start. Write down any diagnoses, past operations and ongoing conditions. Note any hospital treatment you have had. If you see a specialist, have their name and the reason to hand. The more precise you are, the faster a prescriber can reach a clear decision, whichever way it goes.

If your history includes a condition you are unsure about, say so in the free text rather than leaving it out. Prescribers are far more comfortable with an honest "I am not sure" than with a silence that later turns out to matter.

05 · Current medicines

Current medicines and why they matter

Your current medicines are part of the assessment, and an unclear or incomplete medicines list is a frequent cause of delay or decline. The prescriber assesses your current medicines alongside your BMI and medical history before deciding.1

Why do medicines matter for a weight-management injection? Because the medicine acts on how your body handles food, and your other prescriptions may interact with that or with each other. Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose.113

In practice, prescribers decline or pause for medicines in three situations. First, you listed a medicine whose interaction needs specialist input. Second, you listed a medicine but gave no dose or reason, so the picture is unclear. Third, you omitted something that later shows up in other answers. The third is the most avoidable.

Be thorough. Include prescription medicines, anything bought over the counter, vitamins and any supplements, plus anything you take occasionally. If you take the oral contraceptive pill, Mounjaro slows stomach emptying, which can reduce how well it is absorbed. Switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Give names and doses where you know them. If you have a repeat prescription list from your GP surgery or the NHS app, use it.

Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin or sulfonylureas, where doses may need reducing to avoid low blood sugar. Do not change any medicine yourself because of a consultation outcome; speak to your own clinician first.

If you are declined because of a medicine, a sensible next step is a medicines review with your GP or community pharmacist. They can tell you whether a change is possible and appropriate. If circumstances change on their advice, you can start a fresh consultation.

06 · Incomplete consultations

Incomplete or inconsistent consultation answers

A consultation is often declined not because you are unsuitable, but because the prescriber cannot establish that you are suitable. That difference matters, because it is usually fixable.

Typical gaps include a missing height or weight, vague answers such as "some heart thing" with no detail, an empty medicines box, or answers that contradict one another. A prescriber cannot approve a prescription-only medicine on a guess. When the information does not allow a safe decision, the cautious answer is no.

Answer every question. Use measured, recent height and weight, not estimates from years ago. Name your conditions rather than describing them loosely. Where the form offers a free-text box, use it, because that is where borderline details become clear.

A few specific patterns cause trouble more than others:

  • Giving a BMI-qualifying weight that does not match other answers in the form.

  • Listing a medicine without saying what it is for.
  • Rushing, so that a tick-box is wrong.

07 · NHS and private routes

NHS versus private: why the answers can differ

You may be declined online and still be eligible through the NHS, or the reverse, because the two routes use different criteria. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.6

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

That means the groups overlap but are not identical. Someone with a BMI of 32 may meet the licence but not yet the NHS phase. Someone in the highest-need NHS group will meet both. If you are declined privately for a reason related to your health rather than your BMI, the NHS route will usually apply the same safety scrutiny, with your full record in front of the clinician.

Is it hard to get a doctor to prescribe tirzepatide? It depends on which route and whether you fit the criteria for it. On the NHS, access depends on the phased criteria and local service capacity. Privately, the question is whether you meet the licence and pass the safety review. Neither route is a formality, and neither should be.

If you want to talk to your GP, go prepared. Bring your recent weight and height, a list of your conditions and medicines, and your questions about where your local area is in the rollout. Ask what the local pathway is and whether you would be in an early group.

None of this means private is better or worse. It means the criteria are different, and a decline in one place is not a ruling on the other.

08 · Dose and repeat requests

Dose requests and repeat orders that get declined

Not every decline happens at the first order. Prescribers also decline or query repeat orders, and requests for a dose that does not fit the plan. Repeat orders re-check suitability each time, and we do not auto-ship.

Mounjaro comes as a multi-pen KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment is managed by a prescriber, and your dose is a clinical decision, not a menu choice. A request to jump to a much higher strength than your current plan, or to use the medicine in a way the leaflet does not describe, is likely to be queried. Follow the product information and your prescriber's directions.13

If you think your current dose is not right, the answer is a conversation with the prescribing team during your monthly review, not a workaround.

09 · Not responding to treatment

Why some people do not respond to tirzepatide

Some people lose less weight than they hoped, and some prescribers raise this at review. The honest starting point is that trial figures are averages. In SURMOUNT-1, 2,539 adults taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 Individual results vary.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 That means a minority on each dose did not reach that mark. Half of participants on 10mg and 57% on 15mg lost at least 20%, so many did not.2 Trial averages do not predict your own result.

For a worked example, someone starting at 100 kg who matched the 20.9% average on 15mg would lose about 21 kg.

Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 If you are not seeing results, the prescriber will usually look at the plan around the medicine, your dose stage and whether you are still in titration.

Is a lack of response ever a reason for a prescriber to decline a reorder? It can prompt a review conversation. Ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team are part of our aftercare, so a flat result is something to raise, not hide.

10 · Side effects and rejection

"Can your body reject tirzepatide?" and other safety worries

People often ask whether the body can reject tirzepatide, or what the issues with it are. Like all medicines, Mounjaro can cause side effects, and most are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and for most people they ease within days to a few weeks as the body adjusts.113

If you notice a new symptom after starting treatment, tell your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19 Reporting helps regulators monitor the medicine.

The same applies to oddities such as whether you can smell when someone is on an injectable weight-loss medicine: it is not listed as a side effect in the UK patient leaflet.

Is tirzepatide stronger than Ozempic? Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which is what distinguishes it from single-action medicines such as semaglutide (Wegovy and Ozempic).1 Different mechanism is not the same as stronger for you. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply it for weight loss.

Why might someone avoid it? Because the licence does not cover them, because a safety check shows a reason, or because they cannot commit to the diet and activity changes that accompany it. Those are the reasons a prescriber will discuss.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. If a side effect worries you, stopping does not give instant relief, so speak to a clinician rather than waiting.

If you ever feel seriously unwell after an injection, get medical help straight away rather than waiting for an online reply. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

What to do after a decline, including "poor man's Ozempic"

If you are declined, the next step is to work out why and choose a route that fits. Start with the reason given, then talk to your own GP, who has your whole record.

Here are practical options:

  • Ask your GP for a second opinion. They can check whether a condition qualifies you, review your medicines and explain NHS access in your area.
  • Recheck later. If your BMI or health picture changes, a new consultation may reach a different outcome.

  • Use structured lifestyle support. Diet and activity changes matter whatever medicine you take.

You may see the phrase "poor man's Ozempic" used for low-cost supplements such as berberine. These are available cheaply without assessment, with no clinical oversight, and their dosing is not standardised. They are not a licensed replacement for a prescribed medicine, and dramatic weight-loss claims made for them should be treated with caution.

Be especially careful of sellers offering to supply prescription tirzepatide without a consultation or prescription. A decline from a regulated prescriber is safer than an approval from a seller with no clinical review. Why do some people not respond to tirzepatide?

Can your body reject tirzepatide?
Side effects and warnings are listed in the patient information leaflet. Seek immediate medical help for severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of skin or eyes, or signs of a severe allergic reaction. If you notice a new or worrying symptom, tell your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme. Because the medicine takes roughly 4 to 5 weeks to clear after a final dose, speak to a clinician rather than waiting.
What are the issues with tirzepatide?
Like all medicines, Mounjaro can cause side effects, and most are digestive: the most common are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they are most likely when starting or moving up a dose. It also only suits people who meet the licence and pass a safety review. It must be used with a reduced-calorie diet and increased activity. Results vary between individuals, so a prescriber will review your progress during ongoing monthly reviews.
How does tirzepatide compare with Ozempic?
Ozempic is semaglutide licensed for type 2 diabetes; it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply it for weight loss. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, while semaglutide is a single-action medicine. A different mechanism does not tell you which will suit you better. A prescriber can discuss which licensed option fits your health and goals.
What is known as the "poor man's Ozempic"?
The phrase is used for low-cost supplements such as berberine. These are available cheaply without assessment, have no clinical oversight, and their dosing is not standardised. They are not licensed weight-loss medicines, and dramatic weight-loss claims made for them should be treated with caution. If you want a medicine for weight management, a prescriber can assess whether a licensed option suits you.
How much weight can you lose on tirzepatide in 3 months?
Three-month losses are usually modest, because most people are still on the lower rungs of the dose ladder. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12; a sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Individual results vary, and trial averages do not predict your own result.
Can you smell when someone is on Ozempic?
A noticeable smell is not a listed side effect in the UK patient leaflet. If you notice any new symptom after starting treatment, discuss it with your prescriber and consider reporting it through the MHRA Yellow Card scheme, which helps regulators monitor medicines.
What should I do if my tirzepatide request is declined?
Read the reason given, then speak to your own GP, who can check your conditions and medicines and explain NHS access. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
