How much weight can you lose in three months?

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Most of the change happens gradually as the dose is increased, so a short window will not tell you how treatment is going. 06 · Why people feel good

Why do some people feel so good on GLP-1?

Most people feel fuller sooner and less hungry between meals.1 For someone who has struggled with constant hunger, that change can feel like relief.

Other reasons are plausible but are not medicine effects: moving more as weight comes down, eating more regularly, or feeling in control of a plan. If you feel unwell, low or unlike yourself at any point, tell your prescriber rather than assuming it will pass. The same applies if you notice mood changes that worry you.

A good feeling is not a reason to change your dose or skip your reviews. Dose steps are decided with your prescriber, with at least 4 weeks per dose step depending on how you respond.

07 · Who should not take it

Who should not take GLP-1 medicines?

GLP-1 medicines are not for everyone, and they are not for adults at a healthy weight who simply want to lose a little more. Mounjaro is licensed for weight management in adults and for type 2 diabetes, and it is supplied only after an online consultation reviewed by a GPhC-registered prescriber.1 Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.13 Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.13 Never stop or change diabetes medicines on your own.

08 · The downsides

What is the downside to GLP-1?

The main downside is side effects, and most are digestive. The most common are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Others include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.13 Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13

Serious side effects: when to seek help

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). Seek immediate medical help also for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

If you notice any side effect, you can report it through the Yellow Card scheme.19 Treatment also takes time to leave the body: tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1

09 · Poor man's Ozempic

What is the "poor man's Ozempic"?

The label is usually attached to berberine and similar over-the-counter supplements. They are low-cost supplements with no clinical oversight, and they are not licensed medicines. Their dosing is not standardised, and a supplement is not a substitute for a regulated GLP-1 treatment.

Dramatic weight-loss claims on supplement packaging should be treated with caution. If you are considering any supplement alongside prescribed treatment, check with a pharmacist first, because it may interact with other medicines. Anyone offering to supply a prescription medicine without a consultation or prescription is not operating safely.

10 · Contraception and the pill

Contraception: a practical point

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only — implants, coils and injections are unaffected.71

NHS and private access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
