Mounjaro activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals. That supports a reduced-calorie diet, but it also makes eating enough protein harder.

That is where a shake earns its place. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The levers are adequate protein, with a protein source at every meal, and resistance exercise two or more times a week.1 A shake is one easy way to reach the first of those when a plate of chicken and vegetables feels like too much.

How much protein a day on Mounjaro?

The right amount depends on your size, activity and health. The practical advice is to include a protein source at every meal, which is harder when appetite is low.1 That rule is easier to follow than counting grams.

If you want a personal target, ask your prescriber, a pharmacist or a registered dietitian, especially if you have kidney problems or other conditions. Do not copy a figure from social media or a supplement label.

In practice, protein-rich choices include eggs, fish, poultry, lean meat, Greek-style yoghurt, cottage cheese, tofu, beans and lentils. A shake sits alongside these. It can make up the shortfall on a day when you managed only a small lunch.

The need matters more, not less, as the dose rises and appetite falls.1 So revisit your routine after each dose step rather than assuming what worked at the starting dose will still be enough.

05 · Muscle and exercise

Protecting muscle: protein is only half the job

Protein on its own does not protect muscle; it works alongside resistance exercise. Both levers work together, and that is how to think about shakes.1 Drinking shakes while sitting still is a weaker plan than eating sensibly and lifting, pushing or pulling something regularly.

Increased physical activity is part of the licence, not an optional extra. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build. On injection days with early side effects, lighter movement still counts.1

Resistance work does not need a gym. Bodyweight exercises, resistance bands, or light weights at home all count. Exercise also protects against regain if you later stop treatment.

For context on results: in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 For someone starting at 100 kg, 20.9% is about 21 kg. That is a large change, which is exactly why looking after muscle matters.

A sensible shake is usually the plainest one that you will actually drink: high in protein, modest in calories, and low in added sugar. There is no single best brand, and we do not recommend one. Read the label rather than the front of the tub.

Things to check on any label: how much protein per serving relative to calories, the amount of added sugar, and whether it is designed as a snack or a full meal. Smaller servings sipped slowly are usually gentler when the stomach is emptying slowly.

Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily, so smaller, slower meals work better.1 The same logic applies to a rich, thick shake. 07 · Shakes and weight loss

Will shakes help you lose weight or belly fat?

A protein shake does not burn fat, and you cannot choose where on your body weight comes off. Weight loss on Mounjaro comes from eating fewer calories, supported by reduced appetite. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.1

That answers the question about drinking several shakes a day. If those shakes replace higher-calorie food, the deficit may widen. If they are added on top of normal meals, they add calories and can work against you. The result depends entirely on what else you eat.

Is a shake every day fine? A daily shake that fits within a balanced diet is a reasonable choice for many people. If you have kidney disease or another medical condition, ask your prescriber or GP before making protein a big part of your day.

08 · Only shakes?

What happens if you only drink protein shakes?

Living only on shakes, even for a short period, is not a good idea on Mounjaro. You would miss fibre, vegetables and variety, which are the foods to prioritise alongside protein.1 You would also be taking in far fewer nutrients than a normal diet provides, and eating much less can leave gaps.

Skipping solid food also does nothing to build the habits you will need later. When you stop treatment, appetite returns.

No supplement is required with Mounjaro, but a standard daily multivitamin is a reasonable safety net while losing weight; discuss anything beyond that with a pharmacist.1 Supplements sold as natural GLP-1 boosters are not equivalents and are not substitutes for prescribed treatment.

Very low intake is also a reason to talk to your team. If you cannot manage meals, or you feel weak, dizzy or unwell, contact your clinical team instead of trying to push through with shakes alone. If you faint, get medical help straight away. You can also report suspected side effects through the Yellow Card scheme.19

09 · Timing and tolerability

Best time to drink a protein shake

There is no clinically established best time. Choose the point in the day when you struggle most to eat protein. For many people that is breakfast, or the afternoon when a snack is needed but nothing appeals.

A few practical points follow from how the medicine works. Because the stomach empties more slowly, large volumes can sit heavily. Sip, do not gulp, and keep servings small. On injection days when early side effects are strongest, a gentle shake may be easier than a full meal, but listen to your body.

Shakes can sit alongside the rest of your fluids. Drinking enough matters more on Mounjaro than usual: reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class. Keep water within reach through the day, and treat inability to keep fluids down as a contact-your-clinical-team issue, not something to push through.1 A shake is not a substitute for water.

Oral products, like oral supplements, may be absorbed more slowly when you start treatment.13 That is one more reason to introduce new products one at a time, so you know which one is responsible if your stomach objects.

If you also drink coffee in your shake or alongside it, there is no interaction between Mounjaro and caffeine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people, and high-calorie coffee drinks are an easy place for the calorie deficit to leak.1

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
