03 · Why a restart differs

Why restarting is not the same as continuing

Tirzepatide has a half-life of about five days, so about 4–5 weeks after a final dose it has cleared, and nothing can speed this up.1 After three months, any appetite effect from your last course has gone. Your body is starting from scratch with the medicine, which is why your prescriber, not you, should choose where on the dose ladder you begin. 04 · Does it work again?

Does Mounjaro work the second time?

In the SURMOUNT-1 trial, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2

Expect the early weeks to feel modest again. Three months in, a sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide variation, and slower early progress does not predict failure. See weight loss after 3 months for more.

05 · If it seems stalled

If Mounjaro seems to stop working after a restart

Plateaus of a few weeks are normal as the body adapts to a lower weight. Within the licence, the responses are: review your food intake honestly, since portions drift back up; protect protein and activity; check sleep and alcohol; and ask your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.

06 · Early effects again

Expect the early weeks to feel familiar

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Early loss includes water weight, so trend matters more than any single weigh-in.
