Why stomach emptying matters for tablets

Tirzepatide activates two gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 That slower emptying is why most people feel fuller sooner.

A tablet has to leave the stomach and dissolve before it can be absorbed. If emptying slows, the timing can change. The product information applies this specifically to oral contraceptives, whose absorption can be reduced.7

How much any of this matters for a given tablet is a clinical judgement, which is why a clinician who knows your history should make it.

04 · Delaying a period

Using norethisterone to delay a period

If you take norethisterone to delay a period, the practical worry is breakthrough bleeding at the wrong moment. Because Mounjaro can affect how oral tablets are absorbed, you should not assume your usual routine behaves the same.7

Ask the prescriber of your norethisterone how far ahead to plan and whether any back-up arrangement is sensible. Do not add extra tablets or change the timing yourself.

Tell your clinician if bleeding starts unexpectedly while you are on Mounjaro. Unexplained or ongoing bleeding needs a proper assessment, so tell your clinician.

05 · HRT and the womb lining

HRT, the coil and non-oral options

Some women take a progestogen tablet as part of HRT to protect the womb lining. If absorption were reduced, that protection could be affected, which is why this is worth raising rather than assuming.

Non-oral routes such as patches, gels or a coil are sometimes discussed because they do not rely on the stomach. Whether one suits you, including a Mirena coil, is a decision for your GP or menopause or gynaecology clinician.

Ask the clinician who fits or manages your coil.

06 · Contraception and pregnancy

Contraception and planning a pregnancy

If you rely on an oral contraceptive, Mounjaro slows stomach emptying, which can reduce how well the pill is absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only — implants, coils and injections are unaffected.713 Talk to your prescriber about which method is reliable for you while you are on treatment. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

If you want to conceive, clearance time matters. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Speak to your prescriber and GP before stopping contraception or Mounjaro, and plan the timing together.

07 · Endometriosis and other uses

Does Mounjaro help with endometriosis or periods?

No. Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes.1 It is not licensed to treat endometriosis, heavy periods or menopause symptoms.

Some people notice changes in their cycle while their weight changes. If you notice new bleeding or cycle changes, discuss it with your prescriber and report suspected side effects through the Yellow Card scheme.19
