Non Surgical Medical Weight Loss: UK Options Explained (October 2026)

Non surgical medical weight loss means prescribed treatment, such as a licensed weight-loss injection or tablet, used alongside a reduced-calorie diet and more physical activity, with no operation involved. In the UK the licensed injectables are Mounjaro (tirzepatide) and Wegovy (semaglutide), and a prescriber decides what suits you. Our medical weight loss guide covers the wider picture.

A quick overview

What to know
before you start.

  • Non surgical medical weight loss means supervised, prescribed treatment plus diet and activity changes, with no operation.
  • Licensed UK injectables for weight management are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).
  • Two weight-loss tablets are now licensed in the UK, Wegovy tablets and Foundayo, but there is no tablet form of Mounjaro.
  • Supplements marketed as natural alternatives are not medicines and have no comparable trial evidence.
  • NHS access is being phased in over several years, so many eligible people consider regulated private treatment.
In this guide

01 · The core answer

What non surgical medical weight loss actually means

It means a clinician-led plan that uses licensed medicine, not surgery, to reduce weight. The medicine supports your appetite control; you still change how you eat and move. Mounjaro and Wegovy are both licensed for weight management and are used with a reduced-calorie diet and increased physical activity.1

This is different from a casual "non medical" approach, such as dieting alone, over-the-counter products or supplements. Those have no prescriber oversight and no comparable trial evidence.

02 · Your decision

Which route is relevant to you?

The right route depends on your health, your goals and your history, so start with a prescriber conversation rather than a product. Three questions usually decide it:

  • Do you meet the licensed criteria, such as BMI thresholds of 27, 30, 35 or 40 kg/m² with or without weight-related conditions?
  • Do you prefer a weekly injection, a daily injection or a tablet?
  • Have you had bariatric surgery before, or are you considering it?

If you are ready to look at the medicine route, our Mounjaro pen and consultation page shows the current price for the starting dose, £169.99 per month at standard price, with a free consultation reviewed by a GPhC-registered prescriber. If you want to browse first, see our weight loss clinic overview.

03 · Comparison

Medical weight loss vs surgical weight loss

Medical treatment works through appetite and digestion and can be stopped or adjusted; surgery permanently changes the digestive system.

FactorMedical (non surgical)Surgical
What it involvesPrescribed injection or tablet, plus diet and activityOperation such as sleeve or bypass
AnaestheticNoneGeneral anaesthetic
ReversibilityCan be paused or stoppedChanges gut anatomy long term
Ongoing reviewRegular prescriber reviews and dose adjustmentsLifelong nutrition follow-up

Neither route is a shortcut. Both need sustained changes to eating and activity to hold results.

04 · Licensed medicines

The licensed non surgical medical options

The licensed weight-management injectables in the UK are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).1 Mounjaro is a once-weekly pre-filled KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Liraglutide (Saxenda) is a daily injection supplied at 6mg/ml, unlike the weekly Mounjaro and Wegovy pens. Orlistat is a tablet that blocks some dietary fat absorption and produces smaller average losses.1

05 · What trials show

What the trial evidence says

In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.15

In the STEP 1 trial, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 Individual results vary, and trial averages do not predict your own result.

For someone starting at 100 kg, 20.9% (SURMOUNT-1, 15mg over 72 weeks) is about 21 kg. Treat that as arithmetic, not a promise.

06 · Tablets and alternatives

Tablets, supplements and "natural" alternatives

There is no tablet form of Mounjaro; tirzepatide is injection-only in the UK. Two weight-loss tablets are now licensed: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026.2021 They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.

"Natural GLP-1" supplements, such as berberine, are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost, with no clinical oversight, and dosing is not standardised.

Anything that promises faster results outside a licensed pathway is a warning sign. Mounjaro must not be combined with other GLP-1 medicines, unlicensed research peptides (such as retatrutide, which has no UK licence) or compounded copies, because these combinations have no safety data and no regulated supply chain.

07 · Procedures and surgery

Non surgical procedures and bariatric history

Some specialist clinics offer endoscopic procedures, such as gastric balloons, which are not operations in the traditional sense. A specialist clinic is the right place to ask about their suitability and aftercare.

If you have already had a gastric sleeve or bypass, medicine may still be considered. Some people regain weight years after surgery and ask about Mounjaro. It can be clinically appropriate, but this sits with an individual prescriber decision, because surgery changes gut anatomy and nutrition needs, and trial data in post-surgical patients is limited. Declare any bariatric history in your consultation.

If you are having an operation or procedure under sedation or general anaesthetic, tell the surgical team you take Mounjaro. Because it slows stomach emptying, they may give specific fasting or medication-pause instructions before the procedure.

08 · NHS or private

NHS or private access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Our medical weight loss on the NHS page explains the pathway.

If you go private, choose a registered pharmacy. Ours is GPhC-registered (9012878), supplies genuine KwikPens through official LillyDirect supply, and every order follows a free consultation reviewed by a GPhC-registered prescriber. Ongoing monthly reviews are included, and delivery is free, next-day and tracked in temperature-controlled packaging. New customers save £40 on their first order with code FIRST40, and every later order is £25 off the standard price.

09 · Non-medical causes

Non-medical causes of weight loss: a different question

Unexplained weight loss deserves a conversation with your GP rather than a self-diagnosis. Do not start or stop any prescription medicine because of it without advice.

10 · Starting safely

How to start safely

Begin with a consultation, not a purchase. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Needles, a sharps bin and injection guidance come with every order.

Mounjaro is a prescription-only medicine used alongside a reduced-calorie diet and increased physical activity.1 Report any side effect to your prescriber and through the Yellow Card scheme.19 Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

Switching between medicines is also a prescriber decision. There is no licensed direct conversion between semaglutide and tirzepatide; typically the previous medicine is stopped, a washout interval observed, and Mounjaro started at a dose your prescriber judges appropriate. Never run two GLP-1 medicines together. If you are comparing providers, see how online medical weight loss services differ.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
What is non surgical medical weight loss?
It is weight loss supported by prescribed treatment rather than an operation. In the UK that usually means a licensed injection or tablet reviewed by a prescriber, used alongside a reduced-calorie diet and more physical activity. Results vary between individuals, and the prescriber decides what suits your health and history.
What are the main non-surgical medical weight loss options?
Licensed injectables are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg), with Saxenda as a daily injection. Wegovy tablets and Foundayo are licensed tablets, and orlistat is an older tablet with smaller average losses. Specialist clinics also offer procedures such as gastric balloons. A prescriber can advise which fits you.
What is the difference between medical and surgical weight loss?
Medical weight loss uses prescribed medicine that you can adjust or stop, with regular reviews. Surgical weight loss is an operation that changes the digestive system permanently and needs lifelong nutrition follow-up. Both need lasting changes to eating and activity, and neither is a shortcut.
Is there a tablet version of Mounjaro?
No. Tirzepatide is injection-only in the UK. Two different GLP-1 weight-loss tablets are now licensed, Wegovy tablets and Foundayo, each with its own dosing, eligibility checks and trial results. Both are prescription-only, so see a prescriber rather than buying anything marketed as a Mounjaro tablet.
Are supplements a non medical weight loss alternative?
Supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost with no clinical oversight, and dosing is not standardised. They should not be treated as equivalent to licensed prescription treatment.
Can I have medical weight loss treatment after bariatric surgery?
Sometimes. Some people regain weight years after a gastric sleeve or bypass and ask about Mounjaro. It can be clinically appropriate, but it is an individual prescriber decision because surgery changes gut anatomy and nutrition needs, and trial data is limited. Declare your surgical history in the consultation.
Is Mounjaro available on the NHS?
Yes, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first. Many eligible people choose regulated private treatment rather than waiting.
How do I start non surgical medical weight loss with you?
Complete a free online consultation, which a GPhC-registered prescriber reviews. If approved, the pharmacy dispenses your order, with same-day dispatch when ordered by 3pm and free next-day tracked delivery. Ongoing monthly reviews are included. See the Mounjaro pen page.
