03 · Suitability

Is the 12.5mg pen right for you?

It is right only if your prescriber has taken you up the ladder to it. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.1 Your prescriber guides the pace. 12.5mg suits someone who has tolerated 10mg and for whom the prescriber judges a further step worthwhile. 05 · Is 12.5mg strong?

Is 12.5mg of Mounjaro strong?

12.5mg is the second-highest of six strengths, just below the 15mg maximum weekly dose.1 The pen comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.

Whether 12.5mg is strong for you depends on the steps before it. Because the ladder moves up by 2.5mg at a minimum of 4-week intervals, your body has had time to adjust at each level.1 People who reach 12.5mg have already spent at least four weeks on each lower strength. Higher is not automatically better. Your prescriber balances response against how well you tolerate each step. Many people find a lower dose does everything they need, so a bigger number on the pen is not a goal in itself.

06 · Trial results and pace

How long does 12.5mg take to work?

Weight loss on Mounjaro is gradual by design, so think in months rather than days. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 By the time you reach 12.5mg you have typically been on treatment for several months.

The main trial, SURMOUNT-1, enrolled 2,539 adults. Alongside diet and exercise, people taking tirzepatide lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, against 3.1% with lifestyle changes alone. At 72 weeks, 85% on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2

Those are trial averages, and individual results vary. As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Your response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1

07 · What to expect

What to expect on Mounjaro 12.5mg

Expect a steady, treatment-supported reduction in appetite, with the same early side effects you may have met on lower doses. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together they help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals.1

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.1 A step up in dose can bring some of these back for a short time. Read the full list of side effects in the patient information leaflet.13

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 If a symptom worries you, or is new and not described in the leaflet, speak to your prescriber. You can also report suspected side effects through the Yellow Card scheme.19

08 · Staying on 12.5mg

How long can you stay on 12.5mg Mounjaro?

Your prescriber decides how long you stay on 12.5mg, based on your response and tolerance. The licence sets the ladder: after at least 4 weeks on a dose it can step up by 2.5mg, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

In practice there are three outcomes. You may stay on 12.5mg as a maintenance dose, move up to 15mg, or step back to a lower strength if side effects are a concern. Your prescriber can hold a dose for longer or step down within the licence.1 Our monthly clinical reviews exist for these decisions.

If you stop altogether, tirzepatide has a half-life of about five days, so it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

09 · Using the weekly pen

How to use the weekly pen

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses.1 Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13

You set the dose by turning the knob until the dose indicator shows the correct number in the window. Follow the pen's instructions for the injection itself.

When is the best time of day to take Mounjaro?

There is no best time of day. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1

Missed doses and changing day

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on as usual. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.1 You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.1

10 · Food, alcohol and exercise

What should I avoid while taking Mounjaro?

No food is banned on Mounjaro, including bananas, but some choices make treatment easier. Many people feel worse after very fatty, fried or very sugary food. Large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better.1

Alcohol has no direct interaction with Mounjaro, but drinking with a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Moderation is the sensible rule.1

What snacks are good on Mounjaro?

Choose snacks that help you meet protein and fibre goals. Prioritise protein at each meal, plus vegetables, fibre and regular fluids. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.1

Protein matters more as the dose rises and appetite falls. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your strength and metabolism. The two levers are a protein source at every meal and resistance exercise two or more times a week.1 Regular moderate activity such as brisk walking counts too, and on injection days with early side effects, lighter movement still counts.

11 · Medicines to check

What drugs should you not take with Mounjaro?

Tell your prescriber about every medicine you take, because two groups need particular attention. First, Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

Second, if you take insulin or a sulfonylurea, the doses may need adjusting because of the risk of low blood sugar. That is a conversation for your prescriber, and these medicines are not automatically ruled out.1 A prescriber assesses your BMI, medical history and current medicines before deciding whether Mounjaro is suitable.1

Do not combine Mounjaro with another weight-loss injection to try to boost results. Ask your prescriber before using any other product alongside it. You can also compare the weekly Wegovy pen.

12 · Storage and genuine supply

Storing your pen and buying safely

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C. Keep the cap on to protect it from light, and never use a pen that has been frozen.1

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Unopened pens keep in the fridge until the expiry date on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it.1

Counterfeit weight-loss pens are the main risk when buying online. 13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

14 · NHS or private?

NHS or private supply?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
