The distinction matters because the clinical evidence for tirzepatide belongs to the injection. A bottle of liquid drops cannot borrow those results just by borrowing the name.

The idea of Mounjaro drops comes from marketing, not from medicine. Tirzepatide is a once-weekly injectable prescription medicine, and the licensed product is a pre-filled pen.1

Typically, these products are sold as a small bottle with a dropper, taken under the tongue or added to water. They are often described with phrases like "natural GLP-1", "tirzepatide alternative" or "Mounjaro in a bottle". Those words are chosen to sound like the real thing without legally being the real thing. In many cases the product is a dietary supplement or a herbal blend. In the worst cases it is an unregulated product with unknown contents.

It helps to separate three categories. The first is the licensed medicine: genuine Mounjaro, supplied on prescription after a consultation reviewed by a prescriber. The second is over-the-counter supplements, which are sold without clinical oversight, whose dosing is not standardised, and which are not licensed treatments for weight management in the way Mounjaro is. The third is counterfeit or illegally supplied injectables and liquids that borrow the name of a licensed medicine. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7

A useful test is the supply route. A licensed prescription medicine is supplied by a registered pharmacy after a consultation. If a product claims to be a prescription medicine but is offered to you without a consultation or prescription, that is a warning sign in itself. Genuine Mounjaro works by activating two natural gut-hormone receptors at once. Tirzepatide is the first treatment of its kind to activate both GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, sold as Wegovy and Ozempic. It is a property of the specific molecule in the specific licensed pen. It is not a feature you can reproduce with a herbal extract or an unidentified liquid, and it is certainly not something a dropper bottle can promise.

The medicine is made by Eli Lilly, and in the UK it comes in a pre-filled multi-dose pen. The pen is built to deliver a measured dose each week.

The licensed strengths are 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Each step is usually after at least four weeks. Because the medicine works with your appetite signals rather than replacing your effort, it is used alongside a reduced-calorie diet and increased physical activity. The trial results that follow assumed exactly that combination.

As for the pen itself, storage is simple. Before first use, keep unopened pens in the fridge at 2–8°C; they keep until the printed expiry date. Once you start using a pen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. For anything beyond that, the patient information leaflet and your pharmacy are the right places to check.13 None of this applies to drops, because a product that is not the licensed pen comes with no such tested instructions.

06 · What the trials show

What the trial evidence actually shows

The weight-loss evidence for tirzepatide comes from trials of the weekly injection, not from drops. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result. Some people lose more than the average and some lose less, and a prescriber will review how you are responding rather than hold you to a headline figure.

The same trial reported how many participants reached certain thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To make the percentage easier to picture, here is a worked example. For someone starting at 100 kg, 20.9% is about 21 kg. That is a straight piece of arithmetic on a trial average, not a forecast of what you will lose or how fast.

Why does this matter for drops? Because many drop sellers quote figures like these next to their own product. That is misleading. The trial population took the licensed injection, in the doses stated, under medical supervision. Any seller who places those percentages beside a bottle of liquid is borrowing evidence that belongs to something else.

It is also worth stating plainly that these results come from people who were in a clinical programme with a diet and activity component. They were not a passive group. If you are comparing options, the fair comparison is between the licensed medicines and lifestyle change alone, which is exactly what SURMOUNT-1 did, rather than between a licensed medicine and an unlicensed liquid that has never been tested in this way.

07 · Spotting fake drops and scams

How to spot fake Mounjaro drops and scams

The quickest way to spot a fake is to check what the seller is claiming against what the licensed product actually is. Genuine Mounjaro is a once-weekly prescription injection in a pre-filled pen. Any listing that describes drops, a spray, a patch or a powder as "Mounjaro" or "tirzepatide" is not describing the licensed medicine.1

Several other warning signs tend to appear together. Look out for:

  • Sellers offering to supply without a consultation or prescription, when Mounjaro is a prescription-only medicine.
  • Dramatic weight-loss claims, especially ones that promise specific results or a specific speed of loss.
  • Words such as "natural", "herbal" or "alternative" attached to a famous prescription medicine name.
  • No pharmacy registration that you can look up on the GPhC register.
  • Prices that look far too good compared with other providers, or pressure to pay quickly by transfer.
  • Marketplace listings or social media accounts with no clear contact details or pharmacist information.

The MHRA runs counterfeit medicine alerts and the Yellow Card scheme, and it is the right body to tell if you suspect a fake.7 You can also use the Yellow Card scheme to report a suspected side effect from any medicine or product, including one you bought online and are unsure about.19

Why do these products matter for safety, and not just for your wallet? Because an unregulated liquid has no assured contents. It might contain nothing useful, an ingredient that is not listed, or something that interacts with a medicine you already take. Without a prescriber's review of your health history, nobody has checked whether the product is suitable for you.

If you have already taken something you now doubt, do not simply carry on. Speak to a pharmacist or your GP, bring the packaging with you, and tell them what you took and when. If you feel unwell, get medical help straight away. You can then report the product through Yellow Card.

First, Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Second, if you take insulin or a sulfonylurea, the doses may need adjusting because of the risk of low blood sugar. That is a conversation for your prescriber and diabetes team, not a reason to stop or change medicines yourself.

What should you avoid while taking Mounjaro? The sensible answer is to avoid doing anything on your own that your prescriber has not agreed. Do not combine the pen with unlicensed products sold as GLP-1 alternatives, do not adjust your dose without advice, and do not take extra doses. NHS and private access to real Mounjaro

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

This matters for the drops question, because the gap between demand and NHS availability is part of what makes unregulated products attractive. When treatment feels out of reach, a cheap bottle with a familiar name can look like a shortcut.
