02 · Is it optional?

Does Mounjaro have to be titrated?

Yes. In the UK licence, every patient starts at 2.5mg and moves up through the ladder; nobody begins at a higher dose, including people who have lost a lot of weight on other treatments.1 The reason is tolerability.

What is flexible is where you stop. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 Treating 15mg as the goal misreads the ladder. The right dose is the lowest one that works well for you and that you tolerate comfortably.

03 · Planning the cost

What does titration cost?

Titration costs more than one pen because you move through several strengths. At standard prices, if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96. 04 · What you actually decide

The decisions you are really facing

Most readers face two practical choices. The first is whether you are ready to start the ladder at all. Treatment is for adults being treated for weight management, alongside a reduced-calorie diet and increased physical activity, and it follows a consultation reviewed by a GPhC-registered prescriber.1 Our weight loss clinic explains how that works.

The second is pace. If side effects are heavy at a given step, the sensible question is not how to push through but whether to stay at that dose longer. That is a prescriber conversation, and it sits entirely within the licence.

05 · The dose ladder

The Mounjaro dose ladder, step by step

The ladder has six strengths, supplied as a pre-filled multi-dose pen (KwikPen): 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each pen holds your weekly doses at a single strength. You do not mix strengths within a pen, and you move to a new pen when the dose changes.

Where people end up

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 Some stop at 7.5mg or 12.5mg; that is also within the licence. The maintenance dose is simply the dose at which you are making steady progress with side effects you can live with.

What each step is for

Think of the ladder in phases. The 2.5mg phase is adjustment. The 5mg step is the first maintenance-level dose, and for some people it is where they stay. Higher steps are for people who tolerate treatment well and whose prescriber judges that more would help. At every step the same test applies: how are you responding, and how are you tolerating it?

Because pens are single-strength, a dose change always means a new pen. Plan reorders so that you have the next strength in hand before you need it, and remember that repeat orders re-check suitability rather than shipping automatically.

06 · Why we titrate

Why Mounjaro is titrated at all

Titration exists to give your body time to adapt. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Starting low and rising gradually gives your body a chance to adjust, which is why the first weeks are about getting used to the medicine rather than chasing results.

The same logic across GLP-1 medicines

This is a general principle across the class, and our overview of the GLP-1 dosing schedule compares how different medicines climb their ladders. Wegovy, for example, uses the pens 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 Liraglutide (Saxenda) is different again: it is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. The shared idea is a slow, planned climb, not the particular numbers.

What the first weeks feel like

The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

Side effects are most likely when starting treatment or moving up a dose. That is one reason the interval between steps is a minimum rather than a target. If a step feels rough, the interval can simply be longer.

Tolerability, not speed

It is tempting to treat titration as a race to the effective dose. The licence frames it the other way round: the ladder is built so that you reach a dose you can tolerate. A comfortable dose you can keep taking will do more for you over months than a higher dose you struggle with. Any new or worrying symptom should go to your prescriber, and you can report suspected side effects through the Yellow Card scheme.19

07 · Pacing and holding

Staying, stepping up or stepping down

The pace of titration is a clinical judgement made between you and your prescriber. The minimum interval between increases is after at least four weeks, but nothing requires you to move at that speed. If you are tolerating a dose well and progress is steady, there may be no reason to move up at all.

When it makes sense to hold

Holding is not failure; it is the ladder working as designed. Reasons to stay put might include lingering nausea after a recent step, a busy or stressful period, or simply good progress at the current strength.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 If you are doing well at 5mg, going to 7.5mg for the sake of it adds nothing but potential side effects.

When to step up

A step up is reasonable when you have been on the current dose for at least the minimum interval, side effects have settled, and appetite control or progress has plateaued. The decision belongs at your monthly review. Why you should not improvise

The licensed dose ladder already starts low (2.5mg) for tolerability.1 Altering doses outside the licence means unknown dosing accuracy and no safety data.1

08 · Missed doses

Missed doses, late doses and changing day

A missed dose has a clear rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

The rule matters during titration because it keeps your dosing predictable. Do not try to "catch up" by doubling; the spacing between doses is a safety limit, not a suggestion.

What if you miss more than a single dose?

Speak to your prescriber before your next injection: they will advise whether to resume at your previous strength or to go back down the ladder. This is especially relevant if you have had a gap because of illness, holiday or running out of pens.

Plan ahead to avoid gaps. Reorders re-check suitability and are not auto-shipped, so build in time for the review, dispensing and delivery. Orders placed by 3pm are dispatched the same day, with free next-day tracked delivery.

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Move the day forward or back, then keep the new day consistently. Do not shorten the gap below the minimum to fit a trip or a social plan.

Choosing a day and time

There is no "best" time of day — Mounjaro works the same morning or evening, with or without food.1 What matters is the same day each week.1 Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.1 The patient leaflet repeats these instructions in plain language and is worth keeping with your pen.13

09 · Injecting and the pen

Injecting and using the KwikPen correctly

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1

Where to inject

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.1 Rotate the site with each dose.1 Rotating helps avoid soreness building up in one spot. Needles, a sharps bin and injection guidance are included with every order.

Setting the dose and using the pen

Do not combine the remainder of one pen with a fresh one to make up a dose. Contact your pharmacy instead. Follow the pen's instructions for how to deliver the injection, and ask us if anything in the leaflet is unclear.

Same strength, new pen

Each pen holds a single strength. When you step up, you start a new pen at the higher dose; when you stay on the same dose, the next pen is the same strength. Check the strength printed on the label every time you start a pen. A quick glance prevents the most avoidable titration mistake: injecting the wrong strength.

Why "every other week" is not a technique

If you feel that your current dose is more than you need, the licensed route is to discuss a lower dose with your prescriber, not to stretch the interval between injections.

10 · Storage and pen life

Storage, travel and how long a pen lasts

How you store the pen affects every dose on the ladder. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Can a pen be used for longer than its four doses?

No. An in-use pen should be discarded after 30 days (not above 30°C) out of the fridge. People sometimes ask whether they can use one pen for two months or eight weeks by dosing less often. That would mean altering the schedule outside the licence, with unknown dosing accuracy and no safety data.1

Unopened pens

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.13 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13 We deliver in temperature-controlled packaging, so the pen arrives properly protected, and you should move it to the fridge promptly.

Travelling during titration

You can fly with Mounjaro.13 Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription.13 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.13 Check destination rules for travelling with prescription injectables before you fly.13

Time your step-ups with travel in mind. If you are about to start a new strength, it may be sensible to avoid doing so in the middle of a trip, when side effects would be harder to manage. Your prescriber can help you plan around it.

11 · Results and review

What titration delivers, and how it is reviewed

Titration is a means to an end, and the end is gradual weight loss alongside diet and activity. Weight loss on Mounjaro is gradual by design.1 The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.2

What the main trial found

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. That is arithmetic on a trial average, not a forecast for you, and trial averages do not predict your own result.

Higher dose, not always higher benefit for you

The trial shows larger average losses at higher doses, which is why the ladder goes up to 15mg. But it also reflects the fact that many people do well at 5mg: at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Whether the extra step is worth it for you is a conversation about your goals and tolerance, not an automatic upgrade.

The 6-month review

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Note the wording: the highest tolerated dose. It does not mean the maximum dose is mandatory. It means the dose you can actually tolerate.

Judge your own progress by trend. Early loss includes water weight, and a stalled weigh-in during a dose change does not mean the treatment has stopped working. Keep a record in GLPTrackr and bring it to your review.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

13 · Stopping and NHS access

Titrating off, stopping and NHS access

There is no licensed step-down schedule, so be wary of anyone offering a precise taper. If you want to stop or reduce, speak to your prescriber, who can decide whether stepping down within the licence suits you. Do not improvise by spacing injections out; that is not a supported route.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

What happens after the last dose

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 That means the effects, and any side effects, fade gradually rather than switching off at once.

Appetite may feel different as the medicine clears. Plan for that with your diet and activity routine before you stop, and tell your prescriber so that follow-up is appropriate. If you restart after a break, do not assume you can resume at your old strength; ask first.

NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

The titration ladder is the same whichever route you use. If you move from an NHS service to a private one, or the reverse, tell the new prescriber your current dose and how long you have been on it so they can pick up in the right place.
