NHS or private: which route suits you?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment.

Someone who has taken a single pen is describing a very different experience from someone several months into treatment.

06 · What the trials found

What the clinical trial data shows

SURMOUNT-1 is the main trial of Mounjaro for weight management. It enrolled 2,539 adults and ran for 72 weeks. Everyone received lifestyle support, and the comparison group received lifestyle changes alone.2

On average, people taking tirzepatide lost 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg. The lifestyle-only group lost 3.1%. These are averages, so some people lost more and some lost less. Individual results vary.

The trial also reports how many people crossed particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.

Percentages are easier to understand with a starting weight attached. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. Starting at 120 kg, the same averages would be about 18 kg, 23.4 kg and 25 kg. These are straight arithmetic on the trial averages, not predictions for you.

A person with a lower starting weight can lose the same percentage but a smaller number of kilograms. Your prescriber can discuss what a sensible goal looks like for your own body weight.

Three cautions apply when reading any trial result:

  • Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
  • Trial averages do not predict your own result.
  • Results from one trial should not be set against another trial's figures as if they were head to head.

07 · How it works

How Mounjaro works and what you might notice

Mounjaro contains tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together they help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.

In practice, most people feel fuller sooner and less hungry between meals. That makes it easier to follow a reduced-calorie diet, which is how the medicine is meant to be used. It does not replace changes to eating habits and activity.

This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). Wegovy is also licensed and effective, and which medicine suits you is a decision for you and your prescriber.

Because stomach emptying slows, the digestive side of treatment is where most people notice changes. You may feel full after smaller portions, and some people find that rich or high calorie foods sit heavily. Eating smaller portions and paying attention to fullness are practical ways to work with the medicine rather than against it, and your prescriber can advise on food choices.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.

Mounjaro comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. It is licensed in the UK for weight management in adults and for type 2 diabetes.1 Needles, a sharps bin and injection guidance are included with every order, and you should follow the pen's instructions each time you inject.

08 · First weeks and side effects

The first few weeks: what to expect and side effects

The first few weeks on Mounjaro are when side effects are most likely, and most of them are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects, up to 1 in 10 people, include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.13

Side effects are most likely when starting treatment or moving up a dose. For most people they ease within days to a few weeks as the body adjusts. Feeling fuller sooner is expected, because the medicine slows stomach emptying and helps regulate appetite. How you feel after your first injection varies from person to person, and you should not judge the treatment on a single week.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for:

  • severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis);
  • pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems);
  • signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, such as shakiness, sweating or confusion. Your prescriber may need to adjust those medicines.1

You can report any suspected side effect through the MHRA Yellow Card scheme.19 Any symptom that is not listed in the patient leaflet should be discussed with your prescriber or pharmacist if it continues.

09 · Who it is licensed for

Who Mounjaro is licensed for

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1

A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable. That assessment matters more than any review you read, because it takes your own circumstances into account. It is also why the consultation is reviewed by a GPhC-registered prescriber rather than by a scoring script.

Not everyone who wants the medicine will be suitable. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

The NHS route has stricter, phased criteria. Mounjaro is available on the NHS, but access is being phased in over several years. 10 · Dose steps and cost

Dose steps and what a year of Mounjaro costs

Treatment starts at 2.5mg and moves up through 5mg, 7.5mg, 10mg and 12.5mg to 15mg. You stay at least 4 weeks on each dose, depending on how you respond, and your prescriber decides when to step up and where to settle. You can step up to a maximum of 15mg weekly, and not everyone needs the highest dose.1

Planning costs in advance avoids surprises. These figures are standard prices before any discount.

11 · Safety practicalities

Contraception, storage and other practical safety points

Several practical points are worth knowing before you inject. The first concerns contraception.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only. Implants, coils and injections are unaffected.17 Tell your prescriber which method you use so they can advise you.

The second point is storage. Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

For unopened pens, store them in the fridge until first use. If you are unsure about a pen that has been left out, check the patient information leaflet and ask the pharmacy before using it.

The third point is other medicines. If you take insulin or a sulfonylurea, your prescriber may need to adjust those doses because of the risk of low blood sugar. Tell the prescriber about every medicine you take, including those bought over the counter, and check with a pharmacist before adding any supplement.

The fourth point is where you buy. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. Be wary of anyone offering to supply without a consultation or prescription, as counterfeit medicines are a real risk. Experiences vary, and many people notice little straight away. You may feel full sooner, and some people have nausea, indigestion or constipation, which usually ease as the body adjusts. If you have severe stomach pain, a swollen face or throat or difficulty breathing, get medical help straight away.
What do I wish I knew before starting Mounjaro?
Three points matter most. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Side effects are most likely when starting or moving up a dose, so expect an adjustment period. And if you use the contraceptive pill, switch to a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase. Tell your prescriber about all your medicines.
Can Mounjaro users lose a stone in 12 weeks?
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. In SURMOUNT-1, average loss was 15% to 20.9% across the 5mg to 15mg doses over 72 weeks, and individual results vary. Treatment begins at 2.5mg and you stay at least 4 weeks on each dose, depending on how you respond. Your prescriber can set realistic goals with you.
Are online Mounjaro reviews reliable?
Online reviews are personal accounts, and they cannot show the dose, diet, medical history or supervision behind someone's experience. They also tend to come from people at either extreme. Clinical trial data is a more dependable guide to averages, though it does not predict your own result. Is Mounjaro available on the NHS in the UK?
Yes, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years, so many people who meet the licensed criteria choose regulated private treatment. Your GP can explain local access.
How much does Mounjaro cost in the UK?
At our standard prices, a 2.5mg pen costs £169.99 per month and a 15mg pen costs £329.99, with all doses in stock. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. What happens if I stop taking Mounjaro?
Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. If you are thinking about stopping, or have side effects that worry you, speak to your prescriber first. You can also report suspected side effects through the MHRA Yellow Card scheme.
