Which kind of overheating are you worried about?
The first step is to separate a symptom in you from a storage problem with the pen, because the actions are completely different. If you feel hot, flushed or sweaty, that is a symptom to discuss with a clinician. If the pen sat in a hot car, a sunny windowsill or a warm bag, that is a storage question for your pharmacy.

03 · Feeling hot on Mounjaro
Is feeling hot a side effect of Mounjaro?
Feeling overheated is not listed as a side effect in the UK patient leaflet.
That does not mean your experience is imaginary. It means the cause is more likely to be something else, or at least needs to be checked, and a clinician is the right person to do that. Feeling hot can have many ordinary causes that have nothing to do with a prescription: warm weather, a warm bedroom, illness, other medicines, or simply changes in how much you are eating and drinking.
Changing your eating pattern is a big shift in its own right. On treatment, many people eat far less than before. That alone can change how your body feels day to day, and it is one reason regular fluids matter so much.
- Tell your prescriber about any new symptom, even if you think it is minor.
- Record the timing in relation to your weekly injection day and your meals.
- Report it via the Yellow Card scheme, which is how suspected side effects are collected in the UK.19
- Read the patient information leaflet's list of side effects so you know what is formally recognised.13
If it started after you began treatment or changed dose, that timing is useful information for the person reviewing you, and we would want you to share it.
04 · Heat, hydration and summer
Hot weather and hydration on treatment
Drinking enough matters more on Mounjaro than usual, and hot weather makes that more important rather than less. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. Dehydration is the main route to the rare kidney problems reported with this class of medicine.13
So a hot spell is the moment to be deliberate. Keep water within reach through the day rather than waiting until you feel thirsty, particularly if your appetite is low and you are barely eating anything that contains water. Carry a bottle when you go out. Put a glass by the bed. Make it a habit attached to something you already do, such as every time you sit down at your desk.
Treat an inability to keep fluids down as a contact-your-clinical-team issue, not something to push through.13 That applies in summer, on holiday, and after any bout of sickness or diarrhoea.
Prioritise protein at each meal, vegetables and fibre, and regular fluids. And many people feel worse after very fatty, fried or very sugary food, while large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better.13 In heat, heavy meals tend to feel worse, so lighter food can be a comfortable choice.
Gentle activity still counts. On injection days with early side effects, lighter movement is fine, and in very hot conditions it is sensible to choose the cooler part of the day. Increased physical activity is part of the licence, but it should never come at the cost of becoming dehydrated.
05 · Keeping the pen cool
What happens if a Mounjaro pen gets too warm?
The safe approach to a pen that may have overheated is to stop, not inject, and ask the pharmacy that supplied it. We would rather you waste a conversation than inject a medicine whose condition you cannot verify.
The storage rules themselves are short. Unopened pens are stored in the fridge until first use, and they keep until the printed expiry date. The fridge range is 2–8°C.1 Once you have started a pen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1 Do not use a pen that has been above 30°C or has been frozen. That 30 days is the figure for the UK KwikPen, so ignore other numbers quoted from American sources.
If an unopened pen has been out of the fridge, the leaflet and your pharmacy are the right places to look.
Why appearance is not a test
A pen that has been through excess heat can look exactly like a good one. The liquid may stay clear. So the usual instinct, holding it up to the light and deciding it looks fine, is not a safe check. If you know or suspect a pen has been somewhere much too warm, ask before you use it.
Sensible habits
- Put the pen back in the fridge after use, unless it is the pen you are currently carrying with you within the in-use window.
- Never leave a pen in a parked car, on a sunny windowsill, or in luggage left in the sun.
- When travelling, use an insulated cool bag. It is a small accessory purchase and far less costly than a wasted pen.
- Check the printed expiry date before each injection.
- Follow the instructions in the pen's leaflet for use and disposal.
06 · If the pen seems weaker
Could heat make my injection stop working?
You should not try to diagnose a heat-damaged pen from how you feel afterwards. Appetite changes from week to week for many reasons, and a quiet week does not prove anything about the pen. Equally, if you have good reason to think a pen was exposed to excessive warmth, do not use your results as the test.
For context on what the medicine does when it is working as intended, the SURMOUNT-1 trial of 2,539 adults found that people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages; individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
That is a reminder that outcomes depend on the dose, the diet, activity and time, so a change in appetite alone is a weak signal. If you are worried your pen is not performing, talk to your prescriber, who can review your dose and progress properly. 07 · Cold, itching and other effects
Feeling cold, itchy skin and other odd symptoms
Feeling cold is not listed as a side effect in the UK patient leaflet, but itching and rash are listed as common allergic reactions, and injection-site reactions are also common. If you are affected, tell your prescriber, check the side-effect list in the patient information leaflet, and report it through Yellow Card.19
Itching at the injection site or a rash that spreads is different from a vague sensation of temperature. Swelling of the face, lips, tongue or throat needs medical help straight away. Any sign of a serious allergic reaction needs urgent medical attention rather than a wait-and-see approach.
With symptoms nobody has firmly linked to the medicine, the safest route is a record, a conversation and a report. That helps you, and it helps regulators build a picture across many people. Unusual symptoms that cluster at the same point after an injection are especially worth describing carefully.

08 · Is tirzepatide safe?
Is tirzepatide safe, and what should you avoid?
Tirzepatide is a licensed prescription-only medicine, supplied only after a consultation reviewed by a GPhC-registered prescriber, and used alongside a reduced-calorie diet and increased physical activity. Like all medicines, Mounjaro can cause side effects, and most are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13
Food and drink
No food is banned on Mounjaro, including bananas. Some choices make treatment easier, though. Very fatty, fried or very sugary food leaves many people feeling worse, and large portions sit heavily because the stomach empties more slowly.
There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and an emptier stomach can hit harder than usual. In hot weather, alcohol and dehydration are an unhelpful pairing. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Moderation is the sensible rule.
Coffee and tea are fine, as there is no interaction with caffeine. Strong coffee on an empty stomach can aggravate nausea or reflux for some people. A standard daily multivitamin is a reasonable safety net while losing weight, but supplements sold as "natural GLP-1 boosters" are not equivalents and not substitutes for prescribed treatment.
Other medicines
Tell your prescriber and pharmacist about everything you take. Insulin and sulfonylurea doses may need adjusting because of the risk of low blood sugar. 09 · Holidays and travel
Travelling with your pen in hot weather
Travel is where overheating worries are most real, because heat, delays and luggage all work against a medicine that likes the fridge. Plan the journey before you pack.
Carry your pen in hand luggage in an insulated cool bag, not in the hold and never loose in the sun. Check the pen's in-use position first: a pen that you have already started is licensed for up to 30 days (not above 30°C) out of the fridge, while a pen that you have not started should stay in the fridge until first use.1 If your accommodation has a fridge, use it. Needles, a sharps bin and injection guidance are included with every order, so think about how you will dispose of used needles safely while away.
Can I pause treatment for a holiday?
Do not decide this alone. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That means skipping a dose does not switch the medicine off immediately, and it also means restarting is a conversation about dose, not a casual resumption.
Speak to your prescriber before you go, particularly if you are on a higher dose or if you would be away at your usual injection time. They can tell you what is sensible for your own dose and schedule. Avoid improvising with extra or delayed injections.

10 · NHS and private routes
Where can you get advice or treatment?
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
