01 · The short answer

What is the O method on Mounjaro?

The O method is an informal name used in social media videos and posts about Mounjaro and similar injections. It is not a term used by Eli Lilly, the MHRA, NICE or the NHS, and it is not described in the product information. Because it has no official definition, different creators attach different meanings to it.

That matters. When a technique has no agreed definition, you cannot check it against a reliable source. If a video makes a specific claim, such as that a certain pattern or movement improves absorption or boosts weight loss, treat it as unproven.

Mounjaro (tirzepatide) is a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The pen is designed to be used the way its instructions describe.

What should you do instead of trying a trend?

Use the instructions for use in the patient information leaflet, and ask your prescriber if anything is unclear.13 The leaflet covers preparing the pen, choosing and rotating an injection site, how to tell the dose has been delivered, and what to do with the used pen. It is written and approved for exactly this purpose.

If you already have a technique you saw online and are unsure whether it is safe, you have two simple options. Compare it with the leaflet, and if it differs, follow the leaflet. 03 · How Mounjaro works

How Mounjaro works

Mounjaro works by activating two natural gut-hormone receptors at once. Tirzepatide is the first treatment of its kind to activate GIP and GLP-1 receptors together. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.2

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. Notice what this mechanism does and does not involve. The medicine acts through hormone receptors after it has been absorbed from the tissue under the skin. Nothing in how it works depends on a particular pattern, shape or hand movement when you inject. A claim that a special method changes the way tirzepatide works is not supported by the product information.

04 · Injecting by the leaflet

The only injection instructions you need are the ones supplied with your pen. Many of the questions people ask online, such as how many clicks the pen makes, whether you must pinch the skin, and where to inject, are answered in the instructions for use in the patient information leaflet.13

  • Do you have to pinch the skin? Whether to pinch is a technique detail covered by the pen instructions. Follow what they say for the KwikPen, and ask your prescriber if you have very little or very much tissue at the site.
  • Where should you inject? Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.
  • Is there a wrong way to inject? Yes. Using the pen differently from its instructions can mean an incomplete dose or an avoidable injection-site problem. That is why improvised methods are a poor idea.
  • What is the best time of day? There is no "best" time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day; never take two doses within 3 days of each other.

05 · What to avoid on Mounjaro

What to avoid on Mounjaro and after injecting

The most important thing to avoid is improvising with a prescription medicine. Instructions for what to do and not do after an injection are in the patient leaflet, and that is where you should look for them.13

  • Do not follow unverified techniques. If a method, including anything called the O method, is not in the leaflet, do not assume it is safe or that it improves results.
  • Do not change your dose or schedule on the basis of social media. Dose changes belong with your prescriber. Our prescribing team handles dose titration and follow-up assessments.
  • Do not buy from sellers who offer to supply without a consultation or prescription. Mounjaro is prescription-only. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting problems.7
  • Do not ignore the contraception warning. The medicine can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

If you experience a new symptom, tell your prescriber. Severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing: get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19

06 · Storing your pen

Storing and handling the pen

Store unopened pens in the fridge between 2–8°C until first use; they keep until the printed expiry date.1 Once you start using a pen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.

Handling steps matter more than any named method. Check the label and strength before each use, follow the pen's instructions, and put used needles in a sharps bin. If a pen looks damaged or the instructions do not seem to match what you see, stop and ask the pharmacy before injecting.

07 · What results to expect

What results to expect, and what a method cannot change

Results come from the medicine, the dose and your lifestyle, not from an injection ritual. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2

None of those trial results involved an O method or any special technique. They came from using the pen as directed, at the doses in the trial.

08 · Spotting weak advice online

How to judge injection advice you find online

Treat any advice as unproven unless it matches the leaflet or comes from your prescriber. Viral content is built to be shared, not checked, and a catchy name such as the O method makes a tip feel official when it is not.

A few questions help you decide quickly.

  • Does it cite the SmPC, the leaflet, NICE or the NHS? If it cites nothing, you cannot verify it.
  • Does it promise specific results? Credible sources describe trial averages and say that results vary. Anyone promising a particular outcome from a technique is overselling.
  • Is it selling something? Be wary of paid courses, supplements or products attached to a technique. Low-cost supplements sold as alternatives carry no clinical oversight and are not equivalent to a licensed medicine.
  • Does it tell you to deviate from the pen's instructions? If so, do not follow it.

If you are worried that a seller or an account is offering Mounjaro without a consultation or prescription, that is a red flag for counterfeit or unsafe product. The MHRA runs counterfeit medicine alerts and the Yellow Card scheme, which you can use to report concerns.719

09 · Getting Mounjaro safely

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Trials measured results at 72 weeks, so think in months, not days; progress at 3 months is usually modest, and your prescriber can set realistic expectations. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses are modest: in SURMOUNT-1, the full 15–21% averages were measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Individual results vary, and treatment works alongside diet and activity.
Is there a wrong way to give a subcutaneous injection?
Yes. Using the pen differently from its instructions for use can lead to an incomplete dose or avoidable injection-site problems.
