A quick overview

What to know
before you start.

  • Mounjaro is licensed alongside a reduced-calorie diet and increased activity. It is not a reason to stop eating altogether.
  • Very low appetite is common, but small protein-first meals still matter, especially as the dose rises and appetite falls.
  • Fluids matter even more than food. Being unable to keep fluids down is a contact-your-clinical-team issue.
  • Eating almost nothing does not make the treatment work better and can undermine muscle protection.
  • If you have no appetite at all for several days, speak to your prescriber before your next dose. If you cannot keep fluids down, get medical help straight away.

01 · Core answer

What to do when you are not eating at all on Mounjaro

The safest first step is to treat this as a signal to adjust, not a sign the medicine is working brilliantly. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals. That supports a reduced-calorie diet. It does not replace eating.

Here is a practical order of play when food feels impossible:

  • Keep fluids going. Sip water through the day and keep it within reach.
  • Eat small and slow. Smaller, slower meals work better than large portions, because the stomach empties more slowly.
  • Lead with protein. Include a protein source at every meal, even a tiny one.
  • Choose gentle foods. Many people feel worse after very fatty, fried or very sugary food.
  • Tell your prescriber. If this goes on for days, ask whether your dose or timing needs reviewing.

If you are also seeing little change on the scales, our guide to not eating but not losing weight explains why the two can happen together.

02 · Your next step

Is this a problem, or just a quiet appetite?

A quiet appetite is a normal effect of treatment. Having no appetite at all, for days, with weakness, dizziness or an inability to keep fluids down, is a reason to get advice.

03 · Why eating nothing backfires

Why eating nothing is not the goal

Mounjaro is licensed for use alongside a reduced-calorie diet and increased physical activity. The aim is a sustainable deficit, not a perfect food list and not an empty plate.1 Trial results reflect that combination. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

Those are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The practical problem is that your body still needs fuel, protein and fluid. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance.1 Skipping food altogether makes that harder to do.

04 · Protecting muscle and strength

Protecting muscle when appetite is low

The two levers for protecting muscle are adequate protein and resistance exercise. Aim to include a protein source at every meal, which is harder when appetite is low. Resistance exercise two or more times a week matters too. This matters more, not less, as the dose rises and appetite falls.1

When you cannot face a plate of food, think in small units. Some people find soft, easy-to-swallow protein sources more manageable than a full meal. Eat slowly and stop when you feel full.

Movement still counts too. The evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build. On injection days with early side effects, lighter movement still counts.1

05 · Fluids come first

Hydration when you are not eating

Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. That is the main route to the rare kidney problems reported with this class of medicine.1

Keep water within reach through the day. Treat an inability to keep fluids down as a contact-your-clinical-team issue, not something to push through.13

Coffee and tea are fine, as there is no interaction with caffeine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people, and high-calorie coffee drinks are an easy place for the calorie deficit to leak.1 Alcohol is another point to watch. Drinking on a reduced appetite and emptier stomach can hit harder than usual.1

06 · What to eat when nothing appeals

What to eat when nothing appeals

No food is banned on Mounjaro, but some choices make treatment easier. Prioritise protein at each meal, vegetables and fibre, and regular fluids.1 Large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better.

  • Choose small, frequent portions rather than a big plate.
  • Put the protein on the fork first.
  • Avoid very fatty, fried or very sugary food if it leaves you feeling worse.
  • Eat at set times rather than waiting for hunger, which may not come.

Eating much less can leave gaps in your nutrition. No supplement is required, but a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist. Supplements sold as "natural GLP-1 boosters" are not equivalents and are not substitutes for prescribed treatment.1

07 · Why the scales may not move

Not eating, but not losing weight?

Many people assume that eating almost nothing must mean rapid loss, and are surprised when it does not. Results are trial averages over 72 weeks, and individual results vary.2

Weight can also fluctuate on a day-to-day basis, so a single reading tells you little.

If you feel that treatment is not working, read our guides on not eating on Mounjaro but not losing weight and the week-by-week guide to not eating enough. Both go through the common reasons in more detail.

08 · When to get advice

When to contact your prescriber

Get medical help straight away if you cannot keep fluids down or feel unwell in a way that worries you. If you have gone days without eating, speak to your prescriber before your next dose. They can review your dose and your symptoms, and tell you whether to continue.13

For new or unexpected symptoms, discuss them with the prescriber and report suspected side effects through the Yellow Card scheme.19 Our side effects guide to not eating enough and the full list of Mounjaro side effects explain what is expected and what is not.

If you are in the very early weeks, a very low appetite can feel overwhelming. Our pages on being not hungry at all and having no appetite at all cover that experience in more depth.

09 · If you stop treatment

If you decide to stop

If appetite loss is more than you can manage, you may wonder about stopping. Speak to your prescriber first. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

That means appetite effects can linger for a while after the last injection. Keep eating small, regular, protein-led meals and keep up your fluids during that period.

Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber. This page is information, not a substitute for medical advice.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
What to do when you have no appetite on Mounjaro?
Eat small, slow, protein-led meals at set times rather than waiting for hunger, and keep sipping fluids through the day. Avoid very fatty, fried or very sugary food if it leaves you feeling worse. If you go days without eating, speak to your prescriber before your next dose; if you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.
Is it okay to eat only a single meal a day on Mounjaro?
Treatment is licensed alongside a reduced-calorie diet, and the aim is a sustainable deficit. A single meal can make it hard to include protein at every meal, which protects muscle. If your appetite is so low that this is all you manage, tell your prescriber so they can review your dose.
Why am I not losing weight on Mounjaro when I'm not eating?
Trial results are averages over 72 weeks and individual results vary. Raise it with your prescriber, who can look at your dose and pattern.
Why does it feel like my Mounjaro is not working?
Results are trial averages and individual results vary. Treatment works alongside diet and activity. Speak to your prescriber if you are concerned, rather than changing anything yourself.
How can I get my Mounjaro to work again?
Do not change your dose or injection schedule yourself. Check the basics: regular protein-led meals, fluids, activity including resistance work, and following the pen's instructions. Then ask your prescriber to review your progress and dose. They can advise whether any change is appropriate for you.
How do I know if fat is leaving my body?
The scales are only one measure. Clothes fitting differently, changes in your shape and your strength and energy can all tell you something. Ask your prescriber at your monthly review how best to track your progress safely.
Should I take a vitamin if I am barely eating?
No supplement is required with Mounjaro, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist. Supplements marketed as natural GLP-1 boosters are not equivalents and do not replace prescribed treatment.
