03 · Why faces change

Why your face changes when you lose weight

Your face changes because fat comes off the whole body, and the face is one of the places people look first. You cannot instruct the body to lose fat from the stomach and keep it in the cheeks. Where fat goes first is largely down to your own biology.

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Eating less is what produces the weight loss, and the weight loss is what changes your face.

The size of the effect scales with how much weight comes off. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The counter-measures that matter are the ones that protect lean mass: adequate protein, resistance exercise, and a loss pace guided by your prescriber rather than maximum speed.1 None of them is exotic, and all of them are within your control.

Protein. Eating less overall makes it easy to under-eat protein without noticing. Build each meal around a protein source, and keep meals balanced. Smaller portions eaten slowly suit most people, and you should stop at the first feeling of fullness.1

Resistance exercise. Strength training signals your body to hold on to muscle while you lose fat. It does not target the face directly, but it supports lean mass. Gentle activity is a sensible starting point if you are tired in the early weeks.

05 · Skin and loose skin

Loose skin and tightening after weight loss

Skin commonly adapts gradually after weight stabilises, so the best time to judge loose skin is after your loss has slowed, not during it.1 Skin cannot shrink as fast as fat disappears, which is why the jawline and neck can look slack in the middle of a fast loss.

If you want to go further than that, a pharmacist or a regulated aesthetic practitioner can discuss cosmetic options with you in person. Be wary of anyone promising specific results or selling a quick fix. Anything marketed as a miracle skin tightener deserves scepticism.

06 · Plumping the face naturally

Can you plump your face naturally after weight loss?

Be honest with yourself about this, because many online claims overpromise.

What you can do is look after the things that influence how your face looks. Keep meals balanced with enough protein, keep strength work going, stay well hydrated and avoid crash dieting on top of treatment. Stable weight also matters, because a face that is still changing will keep looking different.

Hydration is worth a word. Staying well hydrated is part of managing digestive side effects and looking after yourself on treatment.

Some people find that their face looks fuller again once weight loss slows and eating becomes more regular. That is a common experience of weight stabilising, but results vary and cannot be predicted.

07 · Hair and other effects

Hair shedding and other effects that affect how you look

Hair shedding is the other change that affects appearance, and it is a recognised side effect. Temporary hair shedding is reported as common in trials and is linked to rapid weight loss itself rather than a direct drug effect. For most people it settles as weight stabilises.1 Protein intake and overall nutrition matter, which is another reason the reduced-calorie diet should stay balanced.

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common — affecting more than 1 in 10 people — are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1 Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.

08 · When to get medical help

When a change in your face is not cosmetic

Swelling of the face is not a Mounjaro face change, and it needs a different response. Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, need immediate medical help: get medical help straight away. Do not wait to see if it settles.

Other serious but uncommon effects also need urgent attention. These include severe, persistent stomach pain that may spread to your back, and pain in the upper right abdomen with fever or yellowing of the skin or eyes: get medical help straight away.

09 · Feelings about appearance

How you feel about the way you look

Seeing your face change can affect your mood, even when the weight loss itself is welcome. This is common and worth taking seriously.

Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 That said, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

How do you know fat is leaving your body?

The most reliable sign is a steady trend over time, such as clothes fitting differently and measurements changing, not any single physical sensation. There is no dependable feeling that tells you fat is being burned.

Because the medicine reduces appetite, you may notice that you feel fuller sooner and are less hungry between meals.1 That reflects how the medicine works, and it is what supports a reduced-calorie diet. It is not proof of where fat is coming from.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
