Mounjaro Duration of Action: How Long It Works in Your Body (October 2026)

Mounjaro (tirzepatide) has a long duration of action, which is why it is injected once weekly. It has a half-life of about five days, so after your final dose it takes roughly four to five weeks to clear the system.1 Nothing speeds that up, and nothing needs doing while it clears. Current as of October 2026.

A quick overview

What to know
before you start.

  • Mounjaro is a once-weekly injection because tirzepatide stays active in the body between doses.
  • After a final dose, clearance takes about four to five weeks, and nothing speeds it up.
  • Appetite effects build with regular weekly use, so judge the treatment over weeks rather than days.
  • Missing or delaying injections changes how steady your levels are, so ask your prescriber how to handle a late dose.
In this guide

01 · The core answer

What duration of action means for Mounjaro

Duration of action is how long one injection keeps affecting your body. For Mounjaro, the answer is a full week, which is why the pen is used once weekly.1 Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1

The reason it lasts so long is its half-life. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Half-life means the time it takes for the amount in your body to fall by half. Because that time is long, the medicine does not vanish between weekly doses, and it also does not vanish the day you stop.

The Mounjaro onset of action guide covers what you may notice early on.

02 · Your decisions

What this means for your weekly routine

The practical answer is to inject on the same day each week and judge progress over weeks, not days. Because the medicine lingers, you should not improvise if a dose is late. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13

StageWhat is happeningWhat to do
Starting outEach weekly dose adds to what is already in your body, so effects build gradually.Stay on the dose your prescriber sets and note how you feel.
Between dosesThe medicine remains active across the week, which supports a steady routine.Keep to the same injection day.
Dose increasesYour prescriber can step the dose up after at least 4 weeks on each dose, depending on how you respond.Attend your monthly review and report side effects.
After stoppingLevels fall slowly and clearance takes four to five weeks.Nothing needs doing; speak to your prescriber about what comes next.

If you are weighing up whether Mounjaro suits you, our weight loss clinic explains the wider treatment pathway. Ready to start? Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99), and you can see all doses on the Mounjaro pen page. Prices checked 12 October 2026.

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

03 · Half-life and clearance

Half-life and clearance explained

Half-life is the clearest measure of how long Mounjaro stays active. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

It helps to picture what half-life means in practice. After an injection, the amount of tirzepatide in your blood starts to fall gradually. When the time equal to one half-life has passed, about half is left. After another half-life, half of that remains, and so on. The amount never drops off a cliff. It tapers, which is why clearance after the last dose is measured in weeks, not hours.

This slow taper has two consequences. First, there is no need for daily injections, unlike liraglutide, a daily injection. Second, side effects may take a while to settle even after you stop, because the medicine is still present in your system. If you are worried about a symptom, contact your prescriber, and you can report suspected side effects through the Yellow Card scheme.19

People sometimes ask whether drinking more water, exercising harder or taking supplements will flush Mounjaro out faster. Nothing speeds up clearance, and supplements marketed with dramatic claims are not a route to faster clearance.

Clearance also matters for planning. If you are considering a pregnancy, preparing for a procedure, or changing treatment, tell your prescriber early, because the medicine remains in your system for weeks. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Do not stop or restart treatment on your own because of something you read online.

Finally, clearance is not the same as the effect wearing off in your appetite. Appetite and eating habits often change at their own pace after stopping. Some people notice hunger returning gradually. That is a good reason to plan the next phase of your weight management with your prescriber before your last dose, not afterwards. The guide to how long treatment lasts covers that decision in more depth.

In short, the half-life explains the weekly schedule, the slow wash-out and the reason a single late or missed dose is something to discuss rather than panic about.

04 · Onset and build-up

Onset and build-up over the first weeks

Mounjaro starts acting after the first injection, but most people notice its effects build gradually over the first few weeks. That fits the way the medicine works. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Because the medicine is long-acting and you inject weekly, each dose adds to what is still in your body from the last one. This is why the early weeks are a build-up period rather than an on-off switch.

Our dedicated page on the onset of action goes into the early-weeks experience.

During the build-up, side effects can also appear, particularly after a dose increase. Because the stomach empties more slowly, some people notice digestive changes. If symptoms are troublesome or do not settle, speak to your prescriber rather than changing your dose yourself. Our aftercare includes ongoing monthly reviews, dose titration and side-effect support from the prescribing team.

A few habits help you judge the early weeks fairly:

  • Inject on the same day each week so your routine stays consistent.
  • Note your appetite, fullness and any side effects in a diary or the free GLPTrackr companion app.
  • Attend each review so your prescriber can decide whether to increase the dose.
  • Avoid judging the medicine on a single day or a single week.

It is also worth being realistic about the lowest strength. The 2.5mg pen is a starting dose, not a maintenance dose, and the SURMOUNT-1 results quoted for the trial were for higher strengths. Your prescriber decides whether and when to step up, using your response and tolerance.2

If you want to understand the biology behind the build-up, see our guide to the mechanism of action. For the formal dosing instructions, follow the patient information leaflet supplied with every pen.13

05 · Peak and weekly rhythm

Peak levels and your weekly rhythm

Mounjaro levels rise after each injection and then taper slowly until the next one, so you experience a weekly rhythm rather than a sharp spike and crash.

Because the medicine is long-acting, you are not racing against a clock between doses. The days after an injection are when many people pay closest attention to appetite and any digestive effects. As the week goes on and the next dose approaches, your routine matters more than any single day.

This is the main reason to keep to a regular injection day. A predictable schedule means each dose sits in a similar place relative to the last. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.

It is also normal for people to ask whether the effects fade towards the end of the week. Some people notice appetite creeping back before the next injection. If that happens to you, note it and raise it at your review.

Practical points for living with the weekly rhythm:

  • Choose an injection day that fits your week and stick to it.
  • Use a reminder, or the GLPTrackr app, so a dose is not forgotten.
  • Keep pens in the fridge at 2–8°C and never freeze them; once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13
  • Tell your prescriber about any change in appetite, digestion or mood.

Understanding the rhythm also helps with expectations. The medicine is not a quick fix delivered on one day of the week. It is a steady, ongoing effect that works alongside a reduced-calorie diet and increased physical activity. If you want a wider explanation, our guide to the mode of action sets out how the pieces fit together.

06 · Stopping and what follows

What happens after your last dose

After your last dose, Mounjaro stays in your system for four to five weeks, falling gradually as it clears. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

That slow fade has a practical meaning. You will not feel the medicine disappear overnight. Instead, any effect on appetite tends to change gradually as the levels drop, and any side effects may take a while to settle. If you stopped because of a side effect and it persists, contact your prescriber, and consider reporting it through the Yellow Card scheme.19

Our page on the duration of treatment covers how long people typically stay on therapy and how that decision is made.

Should you want to move between providers or between medicines, the long duration of action matters. Because tirzepatide lingers for weeks, your prescriber will want to know exactly when you last injected and at what dose. Keep your records, or use the GLPTrackr app, so that information is easy to share.

Things worth doing when you stop:

  • Tell your prescriber, so your record and any follow-up are accurate.
  • Keep eating and activity routines going, as treatment works alongside them, not instead of them.
  • Do not use leftover pens in ways the leaflet does not describe.
  • Put used needles in your sharps bin, never household waste.

Stopping is not a failure and restarting is a clinical decision, not a do-it-yourself one. If you restart after a gap, your prescriber will decide the right dose, because the starting point may differ from where you left off.

07 · Dose and duration

How dose strength relates to duration

Mounjaro is injected weekly at every strength. Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

So the schedule does not change as you go up. You still inject weekly. What changes is the dose, as your prescriber guides.

It is natural to ask what weight loss to expect at each strength. The SURMOUNT-1 trial of 2,539 adults found that people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

For a worked example, someone starting at 100 kg who matched the 15mg average of 20.9% would lose about 21 kg. That is simple arithmetic on a trial average, not a prediction for you, and it should not be read as a timetable.

The trial figures describe a long treatment period, not a quick course. Duration of action, in other words, is about each injection lasting a week, whereas duration of treatment is about how many months or years a person stays on therapy. They are separate questions, and our overview of Mounjaro duration helps separate them.

Higher is not automatically better. The right dose is the one that you tolerate and that your prescriber judges appropriate. Never move up on your own.

08 · Dual action and duration

Dual action: why the effect lasts

The long duration of action sits alongside a dual mechanism that sets Mounjaro apart from single-action GLP-1 medicines. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

Those receptors help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

It also explains why different medicines have different dosing patterns. Wegovy (semaglutide) is also once weekly, with pens at 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg; the dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17. Liraglutide, by contrast, is a daily injection. If you are weighing options, see our guide on how tirzepatide's dual action works, or ask your prescriber.

When comparing medicines, avoid setting numbers from different trials side by side as though they were head to head. Only SURMOUNT-5 supports a direct tirzepatide versus semaglutide comparison, and results from other trials cannot be read against each other.15

One more practical point involves oral contraception. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. Ask your prescriber about contraception arrangements.7 Likewise, if you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so discuss this before starting.

09 · Storage and pen life

Storage, pen life and why it matters

Store unopened pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date.13 Once a pen is in use, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1

Do not use a pen that has been above 30°C or has been frozen. If you are unsure about an unopened pen that has been out of the fridge, do not guess: check the patient information leaflet and contact the pharmacy.

Our deliveries use temperature-controlled packaging and arrive by free next-day tracked delivery, with same-day dispatch when ordered by 3pm. Needles, a sharps bin and injection guidance are included with every order. Saturday delivery is £14.99. When your parcel arrives, put your pens in the fridge and follow the pen's instructions for each injection.

A few storage habits help:

  • Keep pens away from the freezer compartment and from direct heat.
  • Write the date you start a pen on the label so you can track the in-use period.
  • Do not share pens or needles with anyone else.
  • Dispose of used needles in the sharps bin provided.

Authenticity also matters for a medicine with such a long duration. Counterfeit and unregulated products circulate online, and the MHRA publishes alerts about them.7 Be wary of anyone offering to supply without a consultation or prescription. We are a GPhC-registered pharmacy (registration 9012878) and supply genuine KwikPens through official LillyDirect supply, direct from Eli Lilly's UK distribution. You can check our registration on the GPhC register.12

Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Repeat orders re-check suitability, and there is no auto-shipping or subscription. Start from the Mounjaro pen page, where doses are currently in stock.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

11 · NHS and private routes

NHS and private routes to treatment

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you are eligible for the NHS, your GP or a specialist service is the place to start. The NHS guidance explains the medicine and who it is for.5 NHS prescriptions in England carry the standard prescription charge of £9.90 per item, unless you are exempt.

For private treatment, the route is a free online consultation reviewed by a GPhC-registered prescriber. Unsuitable orders are refunded automatically, and the price shown is the price paid, with no hidden fees. Ongoing monthly clinical reviews are included, and our team handles dose titration and follow-up assessments. That is relevant for a long-acting medicine, because the decisions you make about dose and duration deserve regular clinical input.

Costs are worth thinking about over the long term, since this is not a one-off purchase. A new customer gets £40 off their first order at any dose with code FIRST40, including people switching to us from another provider at a higher dose. After that, an automatic £25 reorder discount applies to every future order (code DOSE25). We offer Klarna and pay-monthly options, with no subscription lock-in.

Which route is right depends on your eligibility, your timing and your budget. Neither route changes how the medicine works: the same weekly dose, the same slow build-up and the same four to five weeks to clear after the last dose.1

Whichever route you take, the safest approach is the same: use the medicine as prescribed, alongside a reduced-calorie diet and increased physical activity, keep in touch with your prescriber, and report any side effects. For support, our team is available Monday to Friday, 9am to 5pm, by email at support@everydaymeds.co.uk, through the account portal chat, or by phone on 0333 207 0413. The free GLPTrackr app can help you keep track of doses and symptoms as you go.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
How long does it take for a Mounjaro shot to kick in?
Mounjaro starts acting after the first injection, but most people notice its effects build gradually over the first few weeks rather than all at once. It is long-acting and you inject weekly, so each dose adds to what is still in your body. Judge the early weeks patiently, keep a diary, and discuss your response at your monthly review.
When does Mounjaro hit its peak?
Levels rise after each injection and then taper slowly until the next dose, so you experience a weekly rhythm rather than a sharp spike. What matters in practice is keeping to the same injection day each week and telling your prescriber about any change in appetite or side effects.
Can you lose weight on 2.5mg of Mounjaro?
Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity.
How can I speed up weight loss on Mounjaro?
Mounjaro works alongside a reduced-calorie diet and increased physical activity, so sticking to both consistently is the sensible focus. If you are worried about progress, raise it at your monthly review, where your prescriber can consider whether a dose change is appropriate.
What happens if you don't drink enough water on Mounjaro?
Drinking enough matters more on Mounjaro than usual, because reduced appetite means less fluid from food and vomiting or diarrhoea can dehydrate you quickly. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away. You can report side effects through the Yellow Card scheme.
What foods should I avoid on Mounjaro?
No food is banned on Mounjaro. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better. Pay attention to what disagrees with you and discuss it with your prescriber, who can give personalised guidance based on your health and your dose.
What happens if I eat chocolate on Mounjaro?
The medicine is used alongside a reduced-calorie diet, so treats still count towards your overall intake. Because stomach emptying slows, rich foods may feel heavy for some people. If you notice digestive discomfort after particular foods, make a note of it and discuss it with your prescriber at your next review for tailored advice.
Can I drink diet Coke while on Mounjaro?
Because the medicine slows stomach emptying, fizzy drinks may feel uncomfortable for some people. Pay attention to how you feel, and if you have concerns about drinks, diet or digestive side effects, ask your prescriber, who can advise based on your own circumstances and treatment.
Can I eat eggs on Mounjaro?
No food is banned on Mounjaro, and the advice is to prioritise protein at each meal, alongside vegetables, fibre and regular fluids. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, and your prescriber can help you plan meals that suit your health, your dose and your appetite. If a particular food upsets your stomach, note it down and mention it at your monthly review, where personalised dietary guidance can be discussed.
