A quick overview

What to know
before you start.

  • Diarrhoea is among the most common Mounjaro side effects, and it clusters around starting treatment and dose increases.
  • For most people it settles within days to a few weeks; persistent or severe symptoms need your clinical team.
  • Hydration comes first. If you cannot keep fluids down, dehydration needs medical help straight away.
  • Check with a pharmacist before using Imodium, and follow the leaflet rather than any figure from a web page.
  • Your prescriber can hold your current dose for longer if a step up is hard to tolerate.
In this guide

01 · The core answer

What to do when Mounjaro gives you diarrhoea

Start with fluids and plain food, then decide whether you need anything else. Diarrhoea is one of the most common digestive effects of Mounjaro, and for most people it eases within days to a few weeks as the body adjusts.1

Practical steps that help most people with digestive effects are: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, and stay well hydrated.13 If you cannot keep fluids down, contact your clinical team, because dehydration needs medical help straight away.13

Our longer guide on how to help diarrhoea on Mounjaro goes through the day-to-day detail.

02 · The decision

Imodium or let it run its course?

Dose, duration limits and who should avoid it are printed on the Imodium pack and explained by any pharmacist.

Many people find mild, short-lived loose stools are manageable with fluids and plain food alone. If symptoms are disrupting work, travel or sleep, a pharmacist can talk through an over-the-counter option with you in person. Our detailed page on whether you can take Imodium for Mounjaro diarrhoea expands on this.

03 · How common it is

How common is diarrhoea on Mounjaro?

It is one of the most frequent side effects. The most common effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1

In the weight-management trials, diarrhoea was reported by 18.7% of people on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.1 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1 Individual results vary, and these are trial averages that do not predict your own experience.

It helps to keep the other side of the picture in view. 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.1 Most people who get diarrhoea carry on with treatment once it settles. Our page on Mounjaro diarrhoea covers the basics too.

04 · Why it happens

Why does Mounjaro cause diarrhoea?

Mounjaro acts on the gut and on appetite signalling, so digestive effects are expected. Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties.1

Because the digestive system is being asked to work differently, your bowel habit can change, particularly in the early days of a new dose. Side effects are most likely when starting treatment or moving up a dose.1

Interestingly, constipation also appears on the list of common effects. Some people swing between the two, which is one more reason not to treat an isolated loose day as a crisis.

05 · How long it lasts

How long does diarrhoea last on Mounjaro?

For most people it lasts from days to a few weeks. Side effects generally appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts.13 For most people they ease within days to a few weeks as the body adjusts.1

That pattern is the reason Mounjaro uses a gradual dose ladder. Everyone starts on 2.5mg once a week, and the starting dose is about letting your body adjust rather than about weight loss itself.13 After at least four weeks the dose can increase to 5mg, then step up in 2.5mg stages, again at a minimum of four-week intervals.1

So a realistic expectation is a bumpy patch after each increase, easing as that dose beds in. If you want a fuller walkthrough, see our guides on how long Mounjaro diarrhoea lasts and what to expect week by week, plus the side-effects timeline guide.

If yours is not following the pattern, that is useful information for your prescriber.

06 · Early weeks

Is diarrhoea normal in the early weeks?

Yes, the early weeks are exactly when it is most expected. Side effects are most likely when starting treatment or moving up a dose, and the first weeks include both.1

Do not assume the third or fourth week should be trouble-free. Each increase restarts the adjustment, so a settled stomach on one dose can wobble again on the next. That is normal and not a sign that you are doing something wrong.

What matters more than the exact timing is the direction of travel. Improving, even slowly, is reassuring. Getting worse is a reason to contact your clinical team, and if you cannot keep fluids down, contact them because dehydration needs medical help straight away.13

07 · Practical relief

How do you stop diarrhoea on Mounjaro?

You manage it rather than switch it off, using the steps that help most people with digestive effects.13

  • Drink regularly. Fluid loss is the main risk, so sip water through the day. A pharmacist can suggest oral rehydration products if you want them.
  • Eat smaller meals slowly. Stop at the first feeling of fullness.
  • Favour plain, lower-fat foods while your body adjusts.
  • Avoid lying down straight after eating.
  • Do not push through severe symptoms. If a new dose is hard to tolerate, prescribers can hold the current dose for longer before stepping up.13

Holding a dose is a real option. If you are on a prescription with us, our prescribing team can discuss it as part of your monthly review, and side-effect support is part of our aftercare.

08 · About Imodium

Does Imodium help with Mounjaro diarrhoea?

Imodium (loperamide) is a widely sold antidiarrhoeal, and many people ask whether it is acceptable alongside Mounjaro. Dose, number of days and any interaction with your other medicines are best checked with a pharmacist.

Our page on how Imodium works gives more context.

Constipation is also a recognised effect of Mounjaro, so it is worth being careful about overshooting. If you notice your bowels swinging from loose to hard, tell your pharmacist, because that changes the advice.1

09 · If Imodium fails

Why diarrhoea might continue after Imodium

If an antidiarrhoeal is not working, it is a reason to ask for advice, not to take more than the leaflet allows. Never exceed the pack instructions.

There are several possible explanations, and only a clinician can sort them out. The diarrhoea may be linked to a recent dose increase and simply still be settling. It may have another cause unrelated to Mounjaro, such as an infection or a food intolerance. Or you may be dehydrated, which can make you feel worse regardless.

The useful next steps are:

  • Contact your clinical team and tell them what you have tried and for how long.
  • Ask whether your current dose should be held rather than increased.
  • Speak to a pharmacist about alternatives to what you are already using.

10 · Other digestive symptoms

Other digestive symptoms

Eating smaller, plainer, lower-fat meals is the general advice for digestive effects, and it is a sensible place to start.

If a symptom is new, troubling or unexplained, raise it with your prescriber. You can also report suspected side effects through the Yellow Card scheme.19

Sickness often travels with diarrhoea. Our guide on how long sickness lasts on Mounjaro covers that side of things.

11 · When to get help

When diarrhoea needs medical attention

If you cannot keep fluids down, contact your clinical team, because dehydration needs medical help straight away.13 Contact your clinical team if symptoms are severe or not settling.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis), or pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems).13 Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, need immediate medical help.13

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion), as your prescriber may need to adjust those medicines.13
